Licensing Program Analyst (LPA) Jessica Cho arrived at the facility unannounced for the purpose of conducting the Required 1-Year annual inspection. LPA was greeted and granted entry by Caregiver Tamara Barrera after stating the reason for the visit. Administrator (Admin) Pilar Layton arrived approximately 9:30am. Admin has a valid certificate expiring December 23, 2026 per the Active Administrator Certificate Holders list. The following was observed during the inspection: This is a two story property located in a residential neighborhood. Facility operates within the conditions and limitations specified on the license. LPA observed six residents in care of which none are receiving hospice services. LPA observed two caregivers on duty and verified fingerprint clearance and association statuses. There are two private resident bedrooms, two shared resident bedrooms, two resident bathrooms, and one private staff bedroom with a separate entry way from the patio. The staff bedroom has a private bathroom and a kitchenette. Across the staff bedroom in the backyard, a new additional dwelling unit (ADU) was constructed within the past year. At the time of the inspection, the required fire clearance had not been completed or obtained. All common areas were inspected including the office which doubles as a medication room, the attached two car garage, and the second floor occupied by the licensee's family member consisting of one bedroom, bathroom, living area, and kitchen. LPA toured the first floor and observed the residents' bedrooms were appropriately furnished. Beds and bedding supplies were in good condition, adequate lighting was provided, sufficient storage space for each residents' personal belongings were observed. Bathrooms were found to be in compliance, clean, and operational. The hot water temperature measured at 117.3 and 113.7 degrees Fahrenheit in the resident bathrooms. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed sufficient two-day supply of perishables and seven-day supply of non-perishable food. LPA toured the exterior portion of the facility. LPA observed the outdoor passageway is free of obstruction. The exit gates were operational and there were sufficient seating and shading. The three fire extinguishers were mounted, charged, and serviced on September 15, 2025. The auditory devices and smoke/carbon monoxide detectors were tested and operational on both levels. LPA observed the emergency food/water and supplies that were centrally stored in the garage. Emergency drills were conducted quarterly with the last date of July 20, 2026. The Emergency Disaster Plan (LIC610E) was reviewed and updated annually as required. LPA observed the required 'See Something, Say Something' (PUB475) poster posted in the correct size in the entry way. LPA audited six residents' files and two personnel files in which no discrepancies were found. Medications were audited for two out of six residents. No discrepancies found. LPA consulted the following: to ensure a fire clearance is obtained for any structural modifications. Based on the observations made during today's visit, a deficiency is being cited. An exit interview was conducted with Administrator Pilar Layton, and a copy of this report was provided at exit. LPA was greeted and granted entry by Caregiver Amalia Tenoriotello followed by Administrator (Admin) Pilar Layton and explained the reason for the visit. LPA verified Admin met all course requirements per review of the certificate of completion. The following was observed during the inspection: This is a two story property located in a residential neighborhood. Facility operates within the conditions and limitations specified on the license. LPA observed six residents in care of which one is receiving hospice service. LPA observed two caregivers on duty and verified fingerprint clearance and association statuses. The upper level is a private unit with a separate access door. Facility is licensed to operate for six (6) non-ambulatory residents and maintains a hospice waiver for four (4). There are six residents in care with one in hospice and two caregivers on duty during today's visit. LPA observed the facility to be clean and sanitary. There are four resident bedrooms and two resident bathrooms. All common areas were inspected including the attached two car garage and the sitting room which is utilized as a staff bedroom. The residents' bedrooms were appropriately furnished. Beds and bedding supplies were in good condition, adequate lighting was provided, sufficient storage space for each residents' personal belongings were observed. Bathrooms were found to be in compliance, clean, and operational. The water temperature measured at 112.7 and 114.9 degrees Fahrenheit. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed sufficient two-day supply of perishables and seven-day supply of non-perishable food available. LPA toured the exterior portion of the facility. LPA observed the outdoor passageway free of obstructions. The exit gates were self-closing and self-latching. LPA observed sufficient seating and shading. Facility maintains a fire extinguisher that was mounted, charged, and serviced on September 14, 2023. The auditory devices and smoke/carbon monoxide detectors were tested and operational. LPA observed the emergency disaster supplies including food/water in the garage. Facility is conducting emergency drills however is not maintaining a log. Administrator was advised to maintain a log. The first aid kit contains all required elements. LPA observed the required 'See Something, Say Something' (PUB475) poster in 8.5"x11" posted in the hallway. Administrator was reminded to post the PUB475 in the size of 20"x26" in the entry way as required per regulation. The Administrator's Certificate for Pilar Layton expires on December 23, 2024. LPA conducted an audit of six residents' files and two personnel files. No discrepancies were noted. Staff and one resident interview were conducted as the remaining residents were either occupied or sleeping. Medications were audited for six residents. No discrepancies noted. Based on the observations made during today's visit, no deficiency is being cited today. Advisory Notes were issued.
