On May 18, 2026, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by care giving staff after explaining the purpose for the visit. Licensee (LI) Dina Lucaciu was notified via telephone and later arrived to assist with the inspection. LPA observed that Dina Lucaciu has a valid Administrator certificate which expires on November 28, 2026. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents, of which one may be bedridden, and has a hospice waiver for four. The facility is a single story home which consist of six private resident bedrooms, one staff bedroom, seven bathrooms, a living room, a dining room, a kitchen, a medication room, a family room, and an attached two car garage. LPA, accompanied by the LI, conducted a tour of the interior portions of the facility. On today's visit, LPA observed three residents in care and two care giving staff present. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall in the dining room. LPA inspected the six resident bedrooms and observed them to be clear of any hazards. LPA observed resident bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA observed additional linens to be stored in a hallway closet and hallway cabinets. LPA inspected the seven resident bathrooms and observed them to be clean. Resident bathrooms were equipped with grab bars and non-skid floor mats. Faucets and toilets were operational. Hot water temperature measured between 110.4 and 115.8 degrees Fahrenheit. LPA observed the staff bedroom to be kept locked and inaccessible to residents in care. LPA observed the facility has a two day perishable and a seven day non-perishable food supply on hand in the kitchen. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. CONTINUED ON LIC809-C LPA observed a fire extinguisher to be mounted on the wall in the kitchen and it was observed to be charged and purchased on April 21, 2026. LPA tested the wired smoke detectors/carbon monoxide detectors which tested operational. LPA observed the facility conducted their most recent emergency disaster drill on February 10, 2026. LPA observed the centrally stored medication to be kept in the locked medication room. LPA observed there is also a first aid kit stored in the medication room and it has all the required components. LPA observed the door leading to the attached two car garage to be kept locked and inaccessible to residents in care. LPA observed the garage to be used for storage and laundry. LPA observed chemicals and toxins to be stored in the garage. LPA observed the facility has a three day emergency food and water supply stored in the garage. LPA, accompanied by the LI, conducted a tour of the exterior portions of the facility. LPA observed the exterior to be clear of any obstructions or hazards. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gates of the facility are self-latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all three resident files. LPA observed that the Reappraisals on file for all three residents were outdated and have not been updated within the last twelve months as required. LPA reviewed the residents' medication and medication administration records. LPA reviewed two staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, a deficiency is being cited on the attached LIC809-D page. An exit interview was conducted with Licensee Dina Lucaciu. On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by facility administrator Dina Lucaciu after stating the purpose of the visit. There are currently four residents in care, two of which are receiving hospice care at this time. LPA observed residents relaxing in the facility's common living areas or in their respective bedrooms as well as receiving toileting care or hospice care during the visit. LPA accompanied by facility staff toured the physical plant. The facility is a one-story house with an attached garage. The facility has six private rooms along with one locked staff room. There are two shared bathrooms accessed in the hallways as well as multiple en-suite full and half bathrooms throughout the facility. Bedrooms appear clean and sanitary. One resident is currently using full length rails for postural support. Hospice plan of care reviewed and verified to include an order for the support. All resident bedrooms have the required furnishings. Bathrooms appear clean and sanitary. Hot water temperature measured at 110F in two bathrooms with faucets used for personal grooming. LPA observed the kitchen has a minimum two (2) day perishable and seven (7) day non-perishable food supply. Sharp items, cleaning products and the medication central storage all verified to be secured. The mounted fire extinguisher is charged, with up-to-date maintenance receipts present. LPA tested the smoke and carbon monoxide detectors which were found to be operational. The attached garage is inaccessible to residents and used for laundry as well as additional storage of food and supplies. CONTINUED ON FORM LIC809-C CONTINUED FROM FORM LIC809 LPA and facility staff toured the outside of the facility. LPA observed an shaded outdoor seating area with furniture for resident use. There are self-latching gates on both sides of the property and the routes of egress are free of clutter and obstructions. There are no bodies of water on the premises. Facility does not utilize locked perimeters or delayed egress. LPA reviewed four resident records which included all necessary components. LPA reviewed resident medication records and prescription orders for all residents with no discrepancies observed. Oxygen is not currently in use. There is one bedridden resident present on the premises and verified to be located in the bedroom currently identified as bedridden on the fire clearance. Another resident stays mostly in bed but is not bedridden at this time. LPA provided licensee with a consultation on updating the fire clearance to include two bedridden residents in the event of an unforeseen change in condition. LPA reviewed staff records for two caregivers on staff. All staff are background cleared and associated to the licensed location accurately, with current CPR certificates on file. 2024 annual training provided. 2025 training was recently conducted, proof of training to be provided by licensee at the earliest convenience. Disaster drills are conducted quarterly with proof of the two most recent drills provided. Based on the observations made during today’s visit, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Amy Rodgers conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was greeted and allowed entry into the facility by care giver Roberto DeLa Cruz to whom LPA discussed the purpose of the visit.. LPA also discussed the purpose of the visit over the phone with Licensee/Administrator Dina Lucaciu. According to the facility’s license, the facility has a maximum capacity of six (6) non-ambulatory clients of which 1 may be bedridden. Facility also has a hospice waiver for 4. LPA, accompanied Care Giver Roberto DeLa Cruz, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. The facility’s ambient internal temperature was compliant. Hot water temperature at taps accessible to clients were all compliant. There was at least 2 days of perishable food, and at least 7 days non-perishable food present, all safely stored. Cooking/dining equipment and utensils were present. There were no sharp objects, toxic chemicals/poisons, or open-faced heaters accessible to clients. Centrally stored medications were properly stored and locked in medication room. Medication logs and medications reviewed were current and medications appear to be administered according to the label instructions. [Continued on 809-C] [Continued form 809] No pools or bodies of water on the premises. Per Care Giver Roberto DeLa Cruz, no firearms or ammunition are kept at the facility. Smoke alarms, carbon monoxide detectors, emergency lighting, and facility telephone were all working. Fire extinguisher(s) were serviced within the last 12 months. First aid kit was complete and readily accessible. Required licensing postings were observed in visible areas of the facility. LPA reviewed the theft and loss policy and procedures. Transportation procedures are compliant. LPA interviewed staff members and Residents. LPA interviews did not raise any licensing concerns. Staff records review verified that all staff records are complete and compliant. LPA observed that residents were being treated with dignity by staff, and there were sufficient staff on duty to meet resident’s needs. Administrator’s certification is current. LPA Reviewed resident records and confirmed most required documents were on file. During the client file review it was discovered that 2 of 6 tuberculosis test results are missing from files. LPA issued a Technical Violation for missing documentation in the residents files. According to Licensee the documentation is present but they are unable to locate.
