Type of visit: Required - 1 Year

On April 15, 2026, Licensing Program Analyst (LPA) Garlli Tat made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by care giving staff after explaining the purpose for the visit. Administrators (AD) Ryan Roche and Scott Messick were notified via telephone and later arrived to assist with the inspection. LPA observed that both administrators have valid Administrator certificates expiring on December 1, 2027. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents, none of which may be bedridden, and has a hospice waiver for six. The facility is a single story home with four private resident bedrooms, one shared resident bedroom, one staff bedroom, two full bathrooms, two living rooms, a dining room, a kitchen, and an attached two car garage. LPA accompanied by the ADs, conducted a tour of the interior portion of the facility. On today's visit, LPA observed four residents in care and two care giving staff present. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall by the front door. LPA inspected the five resident bedrooms, and they were observed to be free of any hazards. LPA observed the resident bedrooms had the required furnishings of a bed, a chair, a dresser, and a lamp. All resident beds had clean linens and blankets. LPA observed additional linens are stored in the hallway closet. LPA inspected the two full bathrooms. Resident bathrooms are clean. Bathrooms are equipped with grab bars and non-skid floor mats. Faucets and toilets were operational. Hot water temperature measured between 108.8 and 109.2 degrees Fahrenheit. LPA observed the staff bedroom to be kept locked and inaccessible to residents in care. LPA observed the kitchen has a two day perishable and seven day nonperishable food supply on hand. Continued on LIC809-C. LPA observed kitchen appliances to be clean and operational. LPA observed the five burner gas stove lights unassisted. LPA observed kitchen knives are locked underneath the kitchen sink. LPA observed fire extinguishers mounted on the wall in the kitchen and the garage and they were observed to be charged and tagged on March 26, 2026. LPA tested the wired smoke detectors and carbon monoxide detectors and auditory exit alarms, which tested operational. LPA observed the facility conducted their last emergency disaster drill on March 30, 2026. The centrally stored medication is kept in a locked storage closet in the hall. The First Aid kit is stored in the same storage closet and it was observed to have all the required components. LPA observed the facility has a three day emergency food and water supply stored in garage and the pantry. LPA observed the door leading to the attached two car garage is kept locked and inaccessible to residents in care. The laundry room was observed to be locked and it leads to the garage. LPA observed chemicals and toxins to be stored underneath the kitchen sink. LPA and ADs conducted a tour of the exterior portion of the facility. LPA observed the exterior portion to be clear of obstructions and hazards. LPA observed a shaded outdoor seating area with furniture for resident use. The perimeter gate is self-latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all four resident files. All the required documentation were present and current in the resident files reviewed. LPA reviewed residents’ medication and medication records. LPA reviewed two staff files. All staff are background cleared and associated to the facility. LPA observed that the facility has a valid Liability Insurance that expires on March 1, 2027. Based on today's observations, there are no deficiencies being cited per Title 22 of the California Code of Regulations. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. At around 9:10 AM, LPA Tea was greeted and granted entry into the facility by the caregiver staff and explained the reason for the visit. Administrators (AD) Scott Messick and Ryan Roche arrived shortly to assist with the visit. Facility is licensed for 6 non-ambulatory residents, with a hospice waiver for six. Currently there are four residents, and three are on hospice during today's visit. At 9:20 AM, LPA Tea reviewed four resident files and two staff files. Residents’ files and staff files contained all required documentation. Administrator certificates expire on December 1, 2025. LPA Tea along with AD Messick and Roche toured the facility at 10:06 AM. LPA toured the physical plant, checked food service, and the first aid kit. The home consists of 5 resident bedrooms, which one is a shared room, 1 staff room, 2 full bathrooms, living room, family dining room, kitchen, laundry room and attached garage. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 112.6 Fahrenheit degrees and 113.3 Fahrenheit degrees. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, doorways were free of obstructions. First aid kit had all the required elements including dressing, bandages, tweezers, thermometer, and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. LPA observed sharps secured in a drawer. LPA also observed toxin substances to be secured and locked and inaccessible to clients underneath the kitchen sink and garage. The fire extinguisher in the kitchen is fully charged. The facility’s last fire drill was conducted Annual Inspection continued on LIC809C on March 10, 2025. Kitchen appliances are operational during today's visit. LPA observed emergency food and water supply in the garage. LPA toured the outside grounds and there is ample seating with shade and an exit gate on the left side of the house that is self-latching and operational. There is a lovely garden and barbeque grill for grilling. Facility provides activities daily conducted by an activity’s coordinator. The residents play bingo and trivia, do arts and crafts. Also residents partake in musical therapy. At the time of the visit, LPA observed residents playing bingo and answering trivia questions. At 10:23 AM LPA reviewed medication storage and administration. Medications are stored in a locked closet by the kitchen. Medications are being administered per physician order. LPA interviewed residents regarding their quality of care and spoke to staff present regarding care provided. Based on the observation made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations.
