On March 5, 2026, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of a required annual inspection. LPA explained the purpose for the visit and was greeted and granted entry by staff on duty. During the visit, staff on duty contacted the facility administrator (AD) Lisa Gaitan about the visit. For this visit, there are two staff members on duty, both of which are background cleared and associated. AD arrived later to assist with the inspection. The PUB475 ‘See Something, Say Something’ poster was observed to be located near the front entrance. LPA observed that Lisa Gaitan has a valid Administrator certificate which expires on January 10, 2027. The facility is a Residential Care facility for the Elderly (RCFE) licensed for six residents, six of which may be non-ambulatory, none of which may be bedridden, and a hospice waiver for six. LPA toured the interior and exterior portions of the facility with AD. For this visit, there are a total of four non-ambulatory residents in care, one of which is on hospice, none are bedridden, and one ambulatory resident.. The facility is a single story home. There are a total of five bedrooms, four of which are private resident bedrooms, one is a shared bedroom. LPA toured each bedroom with the AD and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and free of any hazards. Smoke and carbon monoxide detectors as well as auditory exit alarms were tested and operational. There are a total of two bathrooms. Continued on LIC 809-C. Bathrooms were observed to be in good repair, toilets and faucets were operational and showers were equipped with grab bars and non-skid floor mats. Water temperature in the bathrooms were measured to be between 116.4 and 116.6 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked in a lockbox in the kitchen and inaccessible to residents in care. Fire extinguisher was charged, mounted and located in the kitchen. Fire extinguisher was dated and tagged on June 9, 2025. LPA observed the emergency disaster and evacuation plan, which is posted in the office. LPA observed the facility conducted their last emergency disaster drill on January 19, 2026. Facility had back-up emergency food and water supply, located in the garage. LPA observed that the First Aid kit had all the required components. Medications were observed to be locked in a medication room in the hallway, inaccessible to residents in care. Chemicals were observed to be locked in the garage. LPA observed the door leading to the detached two car garage is kept locked and inaccessible to residents in care. The garage is used for storage. For the exterior portion, LPA observed patio furniture under shading, and the grounds were free of any hazards or obstructions. There is one self-latching gate in the backyard that can be opened in case of an emergency. No bodies of water were observed. During this visit, five resident files and two staff files were reviewed. All staff are background cleared and associated with the facility. LPA reviewed residents’ medication and medication records. Based on today's observations, there is deficiency being cited per Title 22 of the California Code of Regulations. An exit interview was conducted with Lisa Gaitan. Allegation investigated: Facility retained a resident with a prohibited health condition.

Licensing Program Analyst (LPA) Jerome Haley made an unannounced visit to close the investigation into the complaint allegation above. LPA Haley was greeted by staff and explained the reason for the visit upon entry. During the investigation interviews were conducted with facility staff, a resident, a family member, and a nurse. 7 of 7 individuals denied the allegation or were unable to provide any information to support or corroborate the complaint allegation. According to S1, Resident 1 (R1) had some redness on their buttocks/back area, but there was only redness. According to S1,there is no wound. During an interview with a Home Health Nurse (HHN1), it was discovered R1 was recently released from a skilled nursing facility due to a fall. According to the HHN1, R1 has a wound on the right lower leg and a wound/rash on the buttocks area which is the reason HHN1 is providing wound care. HHN1 added, staff at the facility was advised to reposition R1 every two hours because the wound on the buttocks area is healing slowly because R1 is incontinent. The nurse stated the wound on the back area can be staged as a stage 2. The nurse stated the wound is red, it’s raw, but it’s being treated with barrier cream. Continued on LIC9099C Unsubstantiated Based on the information gathered during the investigation through interviews and document review, the Department is unable to ascertain if the allegation occurred as reported. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove or refute the alleged violation occurred; therefore, the allegation is deemed Unsubstantiated. Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Lisa Gaitan, Administrator. The facility is a five bedroom, two bathroom, single story residence with an approved fire clearance of six non-ambulatory residents with a hospice waiver for six. The facility currently has a census of six residents in care with two on hospice. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors and testing hot water temperature in two of two resident bathrooms. The hot water temperature measured between 119.1 to 119.7 degrees Fahrenheit and all smoke detectors were operational. Resident bathrooms had grab bars and non-skid mats with covered trash cans. The fire extinguisher is charged and was serviced on June 4, 2024. The facility’s last fire drill was conducted on January 8, 2025. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. Knives and cleaning solutions were secured in the garage LPA observed medication storage and reviewed the centrally stored medications. Per review of medications, it is not clear if they are being given as prescribed. LPA advised Administrator to use the digital Adivantus system to inventory and account for medications. An LIC 809-D will be given. LPA toured the exterior of the facility and noted a shaded seating area and various seating areas in the backyard. There are no obstructions in passageways and the exterior gate is self-latching. LPA observed residents in the common area watching a movie. Residents were going to celebrate a St. Patrick's day dinner later in the day. (Continued on LIC 809-C) (Continued from LIC 809) LPA reviewed three of three staff training and fingerprint records and conducted a complete review of resident records. Staff #1 was not fingerprint cleared and was told to leave the premises. Guardian documentation showed prior fingerprints were incomplete and expired on March 2, 2025. A Civil Penalty will be given for $100 per day X 5 days on a LIC 421-BG. Staff #1 will re-submit fingerprints and cannot return to work until they are cleared. LPA interviewed alert residents regarding their quality of care and spoke to staff present regarding care provided. LPA confirmed that administrator has a current administrator certificate, which is pending, and will expire on December 23, 2026. The following deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Jessica Cho arrived unannounced for the purpose to conduct the Required 1-Year Annual Inspection. LPA was greeted and granted entry after stating the purpose of the visit to Caregiver Perla Sanches. Administrator (Admin) Lisa Gaitan arrived on site to assist with the inspection around 11:00am. LPA conducted a tour of the physical plant accompanied by Caregiver Valeria Ochoa. The following was observed during the tour: This is a single story facility comprised of five resident bedrooms and two resident bathrooms, laundry room, living room, office, and a two-car garage. LPA toured the outside grounds. There was shading and sufficient seating for residents. The exit gate was self-closing and self-latching. LPA observed two residents and two caregivers on duty. The two resident bedrooms are spacious and easily accommodates the residents' furnishings. Furniture for each resident bedrooms were inspected. The bathrooms were clean, faucets, and toilets were operational. The hot water temperature initially measured at 124.3 in the shared bathroom and 123.2 degrees Fahrenheit in the private bathroom. The water heater was readjusted and corrected during the visit which then measured at 118.4 and 117.6 degrees Fahrenheit. There was sufficient supply of clean linens. LPA observed a two-day supply of perishables and a seven-day supply of non-perishable food as required by regulation. Carbon monoxide, smoke detectors, and the auditory devices were tested and operational. The fire extinguisher was serviced on 06/16/2023. Medications, toxins, and sharps were locked and inaccessible to the residents. Facility had ample supply of emergency supplies including food/water. LPA reviewed two out of the two resident and staff files. LPA reminded the following: Licensee to pay the licensing fees by 03/02/2024, to ensure at least one staff with a CPR/First Aid training on duty on premise at all times, and to accurately document the current list of medications on the Medication Administration Record. Based on the observations made during today's visit, no deficiency is being cited as per the Title 22 Division 6 Chapter 2 of the California Code of Regulations. Two Advisory Notes (LIC9102s) were issued during the visit.
