Licensing Program Analyst (LPA) Ruth Martinez is conducting this unannounced visit for the purpose of completing an annual required inspection. LPA arrived at the facility and was greeted and granted entry by staff. LPA met with Richard Mendoza, Administrator explained the nature of the visit. There are five (5) residents at the facility and there are no residents receiving hospice services currently. LPA accompanied by Administrator began the tour of the inside and outside of the facility. LPA observed required department postings throughout the facility. Facility stays within the capacity limitations. There is a minimum of one week of non-perishables foods and two days of perishables foods available. There is additional food storage in a spare refrigerator in the garage. The facility is maintained at a comfortable temperature. LPA inspected that medication is centrally stored in a safe locked storage closet located in the hallway. LPA reviewed medication and observed medication was labeled and stored inaccessible to residents in care. LPA inspected the facility bathrooms and LPA measured the hot water temperature which measured 105.9 Fahrenheit degrees. All bathrooms observed to have a supply of soap, toilet paper and towels. Bathrooms are equipped with required safety measures such as non-skid mats and grab bars. Lighting is sufficient to ensure safety and comfort. The facility is equipped with sufficient hand hygiene, cleaning, and disinfecting supplies. LPA observed that toxic chemicals, cleaning solutions and disinfectants are stored locked underneath kitchen sink and locked storage cabinet in the laundry unit. The facility has an available clean supply of linens. LPA inspected residents’ bedrooms which had sufficient lighting to ensure the safety and comfort. All bedrooms observed to have all required components. Storage space is provided for Continued on LIC809-C residents in their bedroom. Smoke detectors/carbon monoxide detectors were tested and found to be operational. LPA toured the outside of the facility and observed outdoor passageways are free of obstructions. LPA observed there are shaded seating areas for residents’ enjoyment in the front yard as well as the backyard of the facility. LPA observed a fire extinguisher with service date of August 20, 2026, charged and mounted in the kitchen and in the garage. Fire drills are conducted, LPA verified fire drill logs are kept and last drill was conducted June 29, 2026. LPA began reviewing records. LPA reviewed three resident records. All the required documentation was present and reviewed in the residents’ files. LPA reviewed two employee records. All employees present have a criminal record clearance and are associated with the facility. LPA observed records reviewed have a current First Aid certificate. LPA as a reminder provided annual fee dues information. Based on the observations made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Jessica Cho arrived at the facility unannounced for the purpose of conducting the Required 1-Year annual evaluation using the CARE Inspection Tool. LPA was greeted and granted entry by Caregiver Glenda Lopez and stated the reason for the visit. Administrator (Admin) Richard Mendoza arrived on premise to assist with the inspection. The administrator's certificate for Richard Mendoza is valid expiring on June 12, 2026. The following was observed during the inspection: This is a single story property located in a residential neighborhood comprised of six private resident bedrooms and two resident bathrooms. Facility operates within the conditions and limitations specified on the license. LPA observed six residents in care of which one resident is receiving hospice service. LPA observed two caregivers on duty and verified fingerprint clearance and association statuses including the administrator. All common areas were inspected which includes the laundry room and attached two car garage. LPA toured the interior portion of the facility. LPA observed and inspected the six private resident bedrooms. The resident bedrooms' were appropriately furnished, beds and bedding supplies were in good condition, adequate lighting was provided, and sufficient storage space for each residents' personal belongings were observed. All bathrooms were found be in compliance, clean, and operational. The hot water temperature in both bathrooms measured at 109.4 and 113.9 degrees Fahrenheit. Toxins, disinfectants, sharps, and medications were secured and inaccessible. The fireplace is barricaded by the sofa. LPA observed sufficient two-day supply of perishables and seven-day supply of non-perishable food. LPA toured the exterior portion of the facility. The outdoor passageway is free of obstruction. The two exit gates on each side of the property were operational, and there were sufficient seating and shading in the patio area. The fire extinguishers were mounted, charged, and serviced on August 25, 2025. The auditory devices and smoke/carbon monoxide detectors were tested and operational. LPA observed the emergency food/water and supplies in the kitchen. The first aid kit had all necessary elements. Emergency drills were conducted on September 5 & 10, 2025. The required Complaint Poster (PUB475) poster was posted in the entry way in the size of 16.75" x 26." LPA reviewed six out of six resident files and two personnel files in which discrepancies were found in the resident files as five out of six residents did not have an updated medical assessment after a change in condition. Three out of those five residents, facility did not maintain record for the Tuberculosis (TB) testing. There were no bed rail orders for three out of six residents as well as no needs and services plan for the current year in five out of six residents. Additionally, facility did not have documentation of the current medication list for five out of six residents. Medication was audited for one out of six residents since the only medication list was available for Resident #2 (R2). The following was addressed during the visit: to ensure the medical assessments are updated annually, to complete the needs and services annually, to ensure facility maintains documentation of TB test results, to obtain bed rail orders, to maintain a complete and current medication list, to replace the two toilet seat covers, to ensure the PUB475 meets the size requirement required per regulation, and to maintain/organize complete staff/resident records. Based on the observations made during today's visit, deficiencies are being cited. Advisory Notes are also being issued. On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of conducting the Required Annual Inspection. LPA was greeted and granted entry by facility caregiving staff after introducing himself and stating the reason of the visit. Administrator Richard Mendoza arrived later to assist with the visit. During the inspection, LPA and staff conducted a tour of the physical plant and observed the following: The facility is a one-story home with four private and one shared bedrooms in addition to a staff room. There are two bathrooms throughout the facility. All bathrooms are observed to be equipped with grab bars and slip mats. All resident bedrooms have the required furnishings. LPA observed all beds have linen and blankets. Postural supports with corresponding orders are observed, however one bedroom is equipped with full length rails in spite of the resident not currently receiving hospice care. Type B citation issued. There are currently five residents admitted to the facility, none of which are receiving hospice care. Bathrooms faucets and toilets are operational. Water temperature was measured to be within acceptable temperature range. LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted. Drills are conducted quarterly however they are not systematically documented. Consultation provided. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Smoke and carbon monoxide detectors tested operational. Fire extinguishers present are fully charged and have been maintained in 2024. There is adequately shaded outside space with outdoor furniture present. There are self-latching gates on both sides of the property. The routes of egress are free of obstructions. There are no bodies of water on the premises. CONTINUED ON FORM LIC809-C CONTINUED FROM FORM LIC809 Medication, cleaning products and sharp items are confirmed to be inaccessible throughout the physical plant. The medication central storage was also observed to be secure and reviewed to be accurate and up to date with the resident's prescription orders. Medication is observed to be prepared two to three days ahead of dispensation. Consultation provided on pre-pouring medication as a Technical Violation Advisory Note. LPA reviewed five resident files and two staff files. Resident records include all necessary components. All staff members on the facility's roster are confirmed to be cleared and associated with this particular licensed location. Training records are maintained at an administrative location off-site. Training and CPR stated to be up to date. Physician reports need to be updated for three residents with a diagnosis of dementia. Type B citation issued. Based on the observations made during today’s inspection, two type B deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Three Advisory Notes issued along with consultations provided to the licensee.
