A&B at Home Health Home Care Facility RCFE

On this day Licensing Program Analyst (LPA) Fred Arias made an unannounced visit to conduct a required annual visit. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. Facility is licensed for six non-ambulatory residents of which all may be bedridden. Facility has an approved hospice waiver for four residents and the home currently has six residents, with one resident on hospice. Administrator (AD) Joiey Ong arrived shortly to assist with visit. LPA along with staff toured the facility at 2:30 PM. LPA toured the physical plant, checked food service, and facility documentation. The home consists of six resident bedrooms, staff room, living room, dining room, kitchen, six bathrooms and an attached garage. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured 110.8 degrees F. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. Auditory exit alarms were operational during today's visit. LPA toured the kitchen and observed sharps locked in a cabinet during today's visit. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Kitchen appliances were operational during today's visit. Smoke detectors tested operational during today's visit. Fire extinguishers were fully charged. LPA reviewed the infection control and emergency disaster plans and plans are complete and thorough. Facility conducts quarterly emergency drills with the last drill conducted on 1/05/2026. Outside grounds were toured. Walkways around the home were clear of hazards. There is shaded outdoor seating for residents. Exit gates are unlocked and operational. LPA observed the emergency food and water supply. First aid kit contained all required items including tweezers, scissors and thermometer. LPA reviewed four resident files and two staff files. Resident files contained required documentation including admission agreements, physician reports, and resident appraisals. Staff files reviewed contained required documentation including required annual training, medical assessment/ TB, criminal record clearance and proof of CPR training. LPA reviewed medication storage and administration. Medications are stored in a locked cabinet. Medications are being administered per physician order. Based on the observations made during today’s visit, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Jessica Cho arrived at the facility unannounced for the purpose of conducting the Required 1-Year annual evaluation using the CARE Inspection Tool. LPA was greeted and granted entry by Caregiver Catherine Ranando and explained the reason for the visit. Administrator (Admin) Joiey Ong arrived shortly after. LPA observed two caregivers on duty providing care to six residents of which one is under hospice care. The Administrator's Certificate expires June 18, 2026 for Administrator Joiey Ong. All staff were fingerprint cleared and associated at the time of the visit. The following was observed during the inspection: This is a single story structure located in a residential neighborhood. Facility operates within the conditions and limitations specified on the license. There are six private resident bedrooms, six resident bathrooms, and one private staff bedroom. All common areas were inspected including the attached two car garage. The residents' bedrooms were appropriately furnished. Beds and bedding supplies were in good condition, adequate lighting was provided, sufficient storage space for each residents' personal belongings were observed. Hospital bed and bed rail orders were present for five out of five residents. Bathrooms were found to be in compliance, clean, and operational. The hot water temperature measured at 110.6, 109.5, 105.1, 106.1, 106.5, and 107.4 degrees Fahrenheit in the resident bathrooms. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed sufficient two-day supply of perishables and seven-day supply of non-perishable food. The kitchen pantry appeared unorganized. LPA toured the exterior portion of the facility. LPA observed the outdoor passageway is free of obstruction. The exit gates were operational and there were sufficient seating and shading. The fire extinguisher was mounted, charged, and purchased 8/11/25. The auditory devices and smoke/carbon monoxide detectors were tested and operational. LPA observed the emergency food/water in the closet behind the piano. Fire drills were conducted quarterly with the last date on June 28, 2025, however drills are not conducted accounting various emergency scenarios. The Emergency Disaster Plan (LIC610E) is outdated. LPA observed the required 'See Something, Say Something' (PUB475) poster posted in the correct size in the entry way. LPA conducted resident/staff interviews and audited six residents' files and two personnel files in which no discrepancies were found. Medications were audited for two out of six residents. No discrepancies noted. LPA consulted the following: to update the LIC610E using the current form released on (3/19), to conduct drills accounting for different emergency scenarios, and to organize the pantry. Based on the observations made during today's visit, no deficiency is being cited. Advisory Notes are being issued. Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced visit to A & B At Home Health Care Facility RCFE. The purpose of today’s visit was to conduct the Annual Required inspection. LPA was allowed entry into the facility and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents, all of which may be bedridden. Facility has an approved hospice waiver for 4 residents and the home currently has 5 residents, with one on hospice. LPA Lyman along with Administrator Joiey Ong toured the facility at 12:56 PM. LPA toured the physical plant, checked food service, and the first aid kit. Facility appears to be clean, safe, and sanitary. The home consists of six resident bedrooms, 6 resident bathrooms, one staff room, living room, dining room, and kitchen. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. LPA observed five residents with a half bed rails. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 107.6 and 111 degrees F in all facility bathrooms. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, doorways were free of obstructions. First aid kit had all the elements including thermometer, and scissors. LPA observed a locked storage area for cleaning supplies under the kitchen sink and toxins secured in the garage. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. LPA observed sharps locked in a kitchen drawer. Smoke detectors and Carbon Monoxide detectors are hardwired and tested operational during today's visit. Fire extinguisher is fully charged. Kitchen appliances are operational during today's visit. LPA toured the outside grounds and there is ample seating for residents. Exit gates are unlocked. LPA observed ample emergency food and water supply. LPA reviewed the emergency disaster plan during the visit. Plan is thorough and complete. Facility provided documentation of last fire drill conducted on 11/22/2023. CONT ON LIC809-C DATED 06/11/2024 Facility provides activities in the form of exercise, music and bible studies. At 1:30 PM, LPA reviewed five resident files and three staff files. Resident files contained required documents including admission agreements, physician reports and resident appraisals. Staff files reviewed contained required documentation of annual training and criminal record clearance. At 2:15 PM, LPA reviewed medication storage and administration. Medications are stored in a locked cabinet. Medications are being administered per physician order. Based on the observations made during today's visit, the following violations are being cited per California Code of Regulations, Title 22, Division 6, Chapter 8.
