Type of visit: Required - 1 Year
On April 21, 2026, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of a required annual inspection. LPA explained the purpose for the visit and was greeted and granted entry by staff on duty. During the visit, staff on duty contacted the facility administrator (AD) Baabak Sharifan about the visit. For this visit, there are two staff members on duty, both of which are background cleared and associated. AD was available via phone to assist with the inspection. The PUB475 ‘See Something, Say Something’ poster was observed to be located near the front entrance. LPA observed that Baabak Sharifan has a valid Administrator certificate which expires on August 6, 2027. The facility is a Residential Care facility for the Elderly (RCFE) licensed for six residents, four of which may be non-ambulatory, two of which may be bedridden, and a hospice waiver for two. LPA toured the interior and exterior portions of the facility with AD. For this visit, there are a total of five non-ambulatory residents in care, three of which are on hospice, and one is bedridden. The facility is a single story home. There are a total of five bedrooms, four of which are private resident bedrooms, one is a shared bedroom. LPA toured each bedroom with the AD and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and free of any hazards. There is no staff room. Smoke and carbon monoxide detectors as well as auditory exit alarms were tested and operational. There are a total of two full bathrooms, one is for residents and one is for staff. Continued on LIC 809-C. Bathrooms were observed to be in good repair, toilets and faucets were operational and showers were equipped with grab bars and non-skid floor mats. Water temperature in the bathrooms were measured to be between 108.6 and 110.6 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked underneath the kitchen sink and inaccessible to residents in care. Fire extinguisher was charged, mounted and located in the kitchen. Fire extinguisher was dated and tagged on May 14, 2025. LPA observed all the required postings in the hallway. LPA observed the facility conducted their last emergency disaster drill on January 13, 2026. Facility had back-up emergency food and water supply, located in the pantry. LPA observed that the First Aid kit had all the required components. Medications were observed to be locked in a storage room in the hallway, inaccessible to residents in care. Chemicals were observed to be locked in the garage. LPA observed the door leading to the attached two car garage is a laundry room, which is kept locked and inaccessible to residents in care. The garage is used for storage. For the exterior portion, LPA observed patio furniture under shading, and the grounds were free of any hazards or obstructions. There are two self-closing gates in the backyard that can be opened in case of an emergency. No bodies of water were observed. During this visit, six resident files and two staff files were reviewed. All staff are background cleared and associated with the facility. LPA reviewed residents’ medication and medication records and three resident interviews were conducted. The facility has valid liability insurance that expires on October 15, 2026. Based on today's observations, there are deficiencies being cited per Title 22 of the California Code of Regulations. An exit interview was conducted with lead caregiver. This report was reviewed with the authorized representative and a copy was provided at the end of the visit. Sep 17, 2025Complaint investigation reportUnsubstantiated
Allegation investigated: -Staff are mismanaging residents's medication. -Staff did not respond to resident in a timely manner. -Staff handled resident in a rough manner -Staff are threatening resident -Staff are not providing adequate food service to resident's. -Staff did not feed resident. -Staff di not safeguard resident's personal belongings.
Licensing Program Analyst (LPA) Ruth Martinez conducted an unannounced visit to the facility to conclude the investigation to the above identified complaint allegations. LPA arrived at the facility and was greeted at the door and granted entry. LPA spoke with Baabak Sharifan, and explained the purpose of the visit. Findings are based upon this investigation which included tour of the facility, facility file review, resident file review and interviews conducted. It is alleged that staff are mismanaging residents’ medication, specifically to seroquel. LPA Martinez visited the facility on January 13, 2022, and residents (R1) had already moved out and medication was not available for review. Record review revealed that on January 4, 2022, the medication release form signed by R1’s POA to have received 7 medications upon move out. Medication list does not list Seroquel as one of the Continued on LIC9099-C Unsubstantiated medications that was released. Interview with staff stated that medication was given as ordered by the doctor and only medication that was prescribed to residents was given. It is alleged that staff did not respond to residents in a timely manner. The interview with 4 of 4 residents stated that they always got responses from staff when needing help and that they waited about 5-10 minutes the most. Residents stated that they got help and assistance from all staff at the facility when they needed it and never had an issue with not getting help. Resident in question was not available for interview due to the resident had moved out of the facility on January 4, 2022. It is alleged that staff handled resident in a rough manner. The interview with 4 of 4 residents stated that they had never witnessed staff being rough with another resident. They had never seen any yelling, treating or being rough with any resident. Residents stated there was a previous resident who would scream and yell and be loud all the time and disrupt residents’ peace. Staff were very good to them and never rough. Interview with 2 of 2 staff stated that R1 had behavior issues and yell and scream all the time. Staff would transfer R1 from wheelchair to bed and R1 gave staff a hard time, but regardless staff did the best they could to transfer R1. It is alleged that staff are threatening resident, specifically by giving 30-day notice. Interview with staff (S1) stated that they had brought to the attention of R1’s POA that R1 was having behavioral issues and that S1 was concerned about it on multiple occasions. S1 stated that after several conversations with the POA about the concerns of the wellbeing and quality of life of R1, R1’s POA gave S1 a 30 day notice. S1 on December 30, 2021, received a 30 day notice of move out for R1, however POA informed S1 that on January 4, 2022, R1 would be moving out. It is alleged that staff are not providing adequate food service to residents. LPA conducted a facility visit on January 13, 2022, and observed there to be a minimum of one week of non-perishables foods and two days of perishables foods available, and fresh fruit available. An interview with 4 of 4 residents stated that they get 3 meals and snacks every day. They are happy with the food service, which is of good and of quality. It is alleged that staff did not feed residents. LPA conducted a facility visit on January 14, 2022, observed Continued on LIC9099-C lunch being prepared, served and given to residents. The interview with 4 of 4 residents stated that they get all the meals they should at the facility. Records review for R1 revealed that per LIC602 physicians report page 4 section 15 resident is able to feed self. The interview with 2 of 2 staff stated that R1 did struggle at time with feeding and POA expressed that they wanted the food to be puree, but staff informed POA that a doctors order was needed. State stated that regardless of the situation when R1 would struggle with feeding they accommodated R1 as best as they could in order for R1 to eat their meals. It is alleged that staff did not safeguard resident’s personal belongings. Record review revealed that LIC621 resident personal property and valuable was blank. The interview with 2 of 2 staff revealed that when residents moved in with personal property LIC621 would be completed and there were times when family brought items to residents and didn’t let staff know in order to update the form or take inventory. It wasn’t uncommon for families to bring items and place them with the residents belonging in their bedroom. R1 moved out and their POA came and removed all of R1’s personal belongings and based on observations they may have taken items of clothing from R1’s roommate since R1 was in a shared room. Based on the information mentioned above, the Department is unable to ascertain if the allegation occurred as reported. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove or refute the alleged violation occurred; therefore, this allegation is deemed Unsubstantiated. Type of visit: Required - 1 Year
Licensing Program Analysts (LPA) Samer Haddadin conducted an unannounced visit for the purpose of conducting a required annual inspection. LPA met with care staff, Jose Cando and explained the reason of the visit. LPA toured the interior and exterior of the facility and noted the following: is a single-story facility comprised of five resident bedrooms and two resident bathrooms. Common areas include the kitchen, two dining areas, living room, laundry room, and a two-car garage. The facility is a single-story home with five resident bedrooms and two resident bathrooms. Common areas include the kitchen, two dining areas, living room, laundry room, and a two-car garage. At the time of the visit, the facility had 4 residents in care. Facility appeared clean, safe, and sanitary. All residents’ rooms had required elements, including bed, chair, closet space, and ample lighting. Facility had extra linens and hygiene supplies for all residents. Restrooms were stocked with soap and paper towels. Hot water measured at 112.1 degrees Fahrenheit in both bathrooms. LPAs observed the facility had a two-day supply of perishables and a seven-day supply of non-perishable. LPA observed hallways and walkways were free of obstruction. LPA observed the fire extinguisher was charged based on the arrow pointing into the green zone. The service tag indicated the extinguisher was last serviced on May2nd, 2024. Facility staff tested smoke and carbon monoxide detectors, and all were operational. First aid kit had all the required elements including bandages, tweezers, thermometer, scissors, and manual. LPA reviewed 2 residents’ files and observed all files had all required documentation. LPA also reviewed 2 staff files and observed all had the required documents. Chemicals and toxins are locked and secured in the laundry room. LPA noted the facility's knives and sharps were secured and locked in a kitchen cabinet. Medication for each resident was kept locked and secured and inaccessible to resident in care. LPA toured the exterior of the facility and observed a shaded seating area and space for activities; all outside emergency exists were free of tripping hazards and or obstructions or barriers Based on the observations made during today’s inspection NO deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Type of visit: Required - 1 Year
Licensing Program Analysts (LPAs) Jessica Cho, Edward Kim, and Faith La arrived at the facility unannounced for the purpose to conduct the Required 1 Year Annual Inspection. LPAs were greeted and granted entry by Caregiver Edgardo Sevilla and explained the purpose of the visit. Administrator (Admin) Baabak Sharifan was advised of the visit by telephone and arrived on premise approximately 1:00pm. The facility is licensed for 6 and maintains a hospice waiver of 2. As of today, the resident census is 6 of which 2 are ambulatory, 3 are non-ambulatory, and no residents are receiving hospice care. At or approximately 9:34am, LPAs conducted a tour of the physical plant accompanied by the caregivers, and the following were observed: This is a single-story facility comprised of five resident bedrooms and two resident bathrooms. Common areas include the kitchen, two dining areas, living room, laundry room, and a two-car garage. LPAs observed signs of personal belongings in the garage evidenced by (2) beds, clothing items, and personal hygiene supplies. The resident bedrooms had all the required elements with ample lighting. The residents' bathrooms were checked. Toilets and water faucets worked properly, and the grab bars were secure. Showers were free of mold/mildew, and the non-skid mats were in place. The hot water temperature measured within range at 105.2 and 106.3 degrees Fahrenheit. LPAs inspected the kitchen and the dining areas. Facility had adequate supply of two-day perishables and seven-day non-perishables but was lacking in quantity and variety of fruits to meet the two-day supply. LPAs observed during the visit groceries including fruits were dropped off at 2:33pm. LPAs observed 2 expired non-perishable food items in the pantry which were disposed during the visit. LPAs also observed the emergency food and water. Sharps were observed unsecured underneath the kitchen cabinet as well as the laundry detergent in the laundry room which were also corrected during the visit. The fire extinguishers were mounted, fully charged however was last serviced by the Fire Marshall on 12/30/2021. The auditory devices, smoke detectors, carbon monoxide detectors were tested and in operating condition. LPAs toured the outside grounds. There were sufficient seating and shading for the residents, and the walkways were clear of hazards. One out of the two exit gates were not self-closing and self-latching. LPAs reviewed six residents’ files. No pre-appraisals and re-appraisals were observed for four out of the six residents. One Physician's Report was not present and the second was not current. Interviews were conducted with six residents and two staff. The medications and the Medication Administration Records (MARs) were reviewed for six residents. No discrepancies noted. Staff files were not available for review at the time of inspection. The following items were reviewed: Complaint Poster (PUB475) in the required size, food menu, activities, resident's rights, Emergency Disaster Plan (LIC610D), and the Infection Control Plan. The following items were advised: to purchase or service the fire extinguishers annually, ensure toxins/sharps are secured, audit/organize/label the expiration dates of the pantry items, repair one exit gate to ensure it self-latches unassisted, to maintain, organize, the resident files and the required documents annually or as needed upon change of condition with respect to the Physician's Reports, Reappraisals, and to obtain a doctor's order for the half rails for Resident #1 (R1). Based on LPAs' observations, one deficiency will be cited today as per the Title 22, Division 6, Chapter 8 of the California Code of Regulations. The remaining deficiencies will be addressed on the continuation inspection at a later date due to time constraints in addition to the review of staff files.
