On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of conducting the initial investigation into the two allegations listed above. LPA was greeted and granted entry by caregiving staff after introducing himself and stating the purpose of the visit. Administrator Rozina Amlani was notified of the visit via telephone and provided with the allegations under review before coming to the facility to assist. During the visit, LPA conducted staff and resident interviews, toured the physical plant and reviewed resident and hospice records present at the facility. Additional documentation was presented by facility staff in the form of photographs, texts and emails. Witness interviews were also conducted via telephone. This report was amended. On 07/22/2022, Licensing Program Analyst (LPA) Kevin Saborit-Guasch conducted an unannounced visit to the facility. LPA was greeted and granted entry by caregiving staff who called Rozina Amlani, Administrator, who arrived later to assist with the visit. It was alleged that Resident sustained injuries while in care. During the investigation it was discovered that Resident 1 (R1) sustained an unwitnessed fall on 02/01/2022. Interviews conducted with staff and facility Administrator confirmed that upon finding resident on the floor resident was observed able to stand up unassisted. R1’s responsible party was notified. No medical attention was sought at that time, however, R1 was later that day sent to the hospital after becoming unresponsive to staff. Medical records obtained and reviewed confirmed R1 sustained bruising to face. Per review of medical records submitted my Mission Hospital under subpoena conducted, R1 was noted to be a fall risk. On 06/09/2022 at 1:15pm, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of investigating complaint #22-AS-20220204135802. LPA was greeted and granted entry by facility administrator Rozina Amlani after being screened for COVID-19 and explaining the purpose of the visit. LPA requested and obtained staff training records as well as the personnel files for caregivers S1 and S2. Personnel files include all required components and documentation of mandatory training is included. Onboarding training includes a discussion of emergency procedures which does not include the conduct expectation for serious incidents involving the health and safety of residents. An interview with staff member S1 and administrator was also conducted by LPA.
