On December 1, 2025, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of a required annual inspection. LPA explained the purpose for the visit and was greeted and granted entry by staff on duty. During the visit, staff on duty contacted the facility administrator (AD) Josephine Teehankee about the visit. For this visit, there are two staff members on duty, both of which are background cleared and associated. AD later arrived to assist with the inspection. The PUB475 ‘See Something, Say Something’ poster was observed to be located by the front entrance. LPA observed the Administrator's Certificate for Rafael Teehankee, which expires on January 27, 2027. The facility is a Residential Care facility for the Elderly (RCFE) licensed for six residents, six of which may be non-ambulatory, one of which may be bedridden, and a hospice waiver for four. LPA toured the interior and exterior portions of the facility with AD. For this visit, there are a total of six non-ambulatory residents in care, five of which are on hospice, and one is bedridden. The facility is a single story home. There are a total of seven bedrooms, six of which are private resident bedrooms and one staff bedroom. LPA toured each bedroom with the AD and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and free of any hazards. LPA observed the staff room is kept locked and inaccessible to residents in care. Smoke and carbon monoxide detectors as well as auditory exit alarms were tested and operational. There are a total of five bathrooms. Continued on LIC 809-C. Bathrooms were observed to be in good repair, toilets and faucets were operational and showers were equipped with grab bars and non-skid floor mats. Water temperature in the bathrooms were measured to be between 129.2 and 130.6 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked in the kitchen cabinet and inaccessible to residents in care. Fire extinguisher was charged, mounted and located in the kitchen. LPA observed the emergency disaster and evacuation plan, which is posted in the living room. LPA observed the facility conducted their last emergency disaster drill on September 3, 2025. Facility had back-up emergency food and water supply, located in the garage. LPA observed that the First Aid kit had all the required components. Medications were observed to be locked in a medication cabinet in the dining room, inaccessible to residents in care. Chemicals were observed to be locked underneath the kitchen sink and in the garage. LPA observed the door leading to the attached two car garage is kept locked and inaccessible to residents in care. The garage is used for storage and laundry. For the exterior portion, LPA observed patio furniture under attached shading, and the grounds were free of any hazards or obstructions. There are two exit gates in the backyard that can be opened in case of an emergency. There is a gazebo in the backyard for seating. No bodies of water were observed. During this visit, six resident files and two staff files were reviewed. All staff are background cleared and associated with the facility. LPA reviewed residents’ medication and medication records and two resident interviews were conducted. Based on today's observations, there are deficiencies being cited per Title 22 of the California Code of Regulations. A Technical Violation was issued at the time of the visit. An exit interview was conducted with staff. This report was reviewed with staff and a copy was provided at the end of the visit. Type of visit: Case Management - Incident

Licensing Program Analyst (LPA) Alvaro Ramirez, Jr made an unannounced case management visit in conjunction with Unusual Incident/Injury Report (UIIR) dated February 9, 2024. LPA was greeted and granted entry by Caregiver Michael Lopez. LPA explained the reason for the visit. Administrator (AD) Josephine Teehankee arrived shortly after. Per UIIR on February 8, 2024, Licensee was notified by a resident's family member of potential financial abuse by former Administrator Assistant. During today's visit LPA obtained a copy of staff schedule and Resident Roster. LPA also conducted interviews with residents and staff. A Health and Safety inspection was conducted, and LPA Ramirez observed no Health and Safety concerns during today's visit. Based on observations and records reviewed during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Manager (LPA) Fred Arias made an unannounced visit to conduct a plan of correction visit. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. LPA verified correction for 1 deficiency has been made from cited section CCR 87309(a). LPA printed Plan of Correction clearance letter for deficiency cleared. On this day Licensing Program Analyst (LPA) Fred Arias made an unannounced visit to conduct a required annual visit. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents which one may be bedridden. Facility has an approved hospice waiver for 4 residents and the home currently has 4 residents, with 4 residents on hospice. Administrator (AD) Josephine Teehankee arrived shortly to conduct facility tour. AD Teehankee has a valid certificate that expires on 11/19/2026. LPA along with AD toured the facility at 9:40 AM. LPA toured the physical plant, checked food service, facility documentation and the first aid kit. The home consists of 6 resident bedrooms, staff room, living room, dining room, and kitchen as well as 5 bathrooms. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 114.6 degrees F and 116.4 degrees F in all restrooms. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. Auditory exit alarms were operational during today's visit. LPA toured the kitchen and observed sharps locked in a cabinet during today's visit. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Kitchen appliances were operational during today's visit. At 10:00am, LPA observed locks under the kitchen sink non-operational housing cleaning supplies. Smoke detectors tested operational during today's visit. Fire extinguishers were fully charged. LPA reviewed the infection control and emergency disaster plans and plans are complete and thorough. Facility conducts quarterly emergency drills with the last drill conducted on 3/1/2025. Outside grounds were toured. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises. First aid kit contained all required items including tweezers, scissors and thermometer. Facility conducts activities in the form of exercise. There is shaded outdoor seating for residents. Continued on LIC809-C dated 3/3/2025 Exit gates are unlocked and operational. LPA observed the emergency food and water supply. LPA reviewed four resident files and two staff files. All resident files contained required documentation including admission agreements, physician reports, resident appraisals, and physician orders for bed rails as indicated. Staff files reviewed contained required documentation including required annual training, medical assessment/ TB, criminal record clearance and proof of CPR training. LPA reviewed medication storage and administration. Medications are stored in a locked closet. Based on the observations made during today’s visit, the following deficiency is being cited per Title 22 Division 6 of the California Code of Regulations.
