On February 9, 2026, Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit for the purpose of conducting the Required- 1 Year evaluation using the Care Inspection Tool. LPA was greeted and granted entry after explaining the reason for the visit to Caregiver John Montolalu. Administrator Maryam Alamoutinia arrived on site and remained at the facility for the duration of the evaluation. The administrator's certificate for Maryam expires on February 3, 2026, and all course work has been completed. The inspection is as follows: This facility is a single story residential home comprised of six private resident bedrooms, five resident bathrooms, and one shared staff bedroom. The resident census is six (6) and there are two (2) staff on duty. Facility is operating within the conditions and limitations specified on the license. LPA toured the premise accompanied by Caregiver Aina Tsou. LPA observed all resident bedrooms had the required elements with ample lighting. All bathrooms were clean and operational. The hot water temperature measured at 110.8, 109.2, 113.1, 112.4, and 111.9 degrees Fahrenheit. There were a clean supply of linens. LPA observed ample two-day supply of perishables and seven-day supply of non-perishable food. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed the fire extinguisher was mounted, charged, and serviced on April 11, 2025. The indoor temperature remains at a comfortable range. The smoke/carbon monoxide detectors were tested and operational. The fire place was screened, and there are no bodies of water on premise. Facility maintains emergency food, water, and disaster supplies as noted on the Emergency Disaster Plan including the first aid kit. LPA reviewed six of six resident files and two of two personnel files in which no discrepancies were found. Medications were audited for two of six residents, and no discrepancies were found. Annual staff training is currently in progress with completion of more than 50%. Present staff are First Aid/CPR certified, background cleared, and associated to the facility. Disaster drills are conducted on a quarterly basis. The Complaint Poster in the correct size is posted in the entry way. Based on the observations made during today's visit, no deficiency is being cited. Licensing Program Analyst (LPA) Jessica Cho arrived unannounced for the purpose of conducting the Required 1-Year annual evaluation using the Care Inspection Tool. LPA was greeted and granted entry by Caregiver John Moralida and explained the reason for the visit. Administrator (Admin) Maryam Alamoutinia arrived on premise to assist with the inspection. The facility is a single story structure and is licensed to operate for five non-ambulatory residents of which one may be bedridden and maintains a hospice approved for two. There are no residents on hospice during today's visit. During the tour of the physical plant, LPA observed six resident bedrooms and five resident bathrooms with one additional bedroom occupied by two staff. Resident bedrooms had all required furnishings. Bathrooms were found to be in compliance, clean, and operational, however the hot water temperature measured at 137.8, 134.7, 133.1, 135.6, and 129.2 degrees Fahrenheit. All common areas were inspected including the attached two car garage. LPA observed sufficient emergency food and water. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed sufficient two-day supply of perishables and seven-day supply of non-perishable food available in the kitchen and garage. LPA toured the exterior portion of the facility. The outdoor passageway is free of obstruction. The exit gate was self-closing and self-latching. LPA observed sufficient seating and shading. The fire extinguisher was serviced on March 8, 2024, The auditory devices and smoke/carbon monoxide detectors were tested and operational. Emergency evacuation drills are conducted quarterly with last drills on March 28-29, 2025. The first aid kit contains all necessary elements. The Complaint Poster, 'See Something, Say Something,' (PUB 475) was available and posted in the incorrect size. No discrepancies noted with resident/staff records. The following items were addressed during the exit interview: to enlarge the PUB475 to meet the required size of 20"x26," to assemble the emergency supplies and amend the Emergency Disaster Plan (LIC610D), to provide proof of last emergency drill, and expiration date of the administrator's certificate, and to maintain the hot water temperature in the resident bathrooms between 105-120 degrees Fahrenheit at all times. Based on the observations made during today's visit, a deficiency is being cited. Advisory Notes (LIC9102s) are also being issued. On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit for the purpose of conducting a Required Annual Inspection. LPA was greeted and granted entry by caregiving staff after introducing himself and stating the purpose of the visit. Administrator Maryam Alamoutinia was contacted by phone and arrived later to assist with the visit. During the inspection, LPA and administrator conducted a tour of the physical plant and observed the following: The facility is a one-level home with six resident bedrooms, one staff room and three full bathrooms. All resident bedrooms have the required furnishings. LPA observed all beds had linens and blankets and an adequate additional supply is present. The backyard has a shaded sitting area and the route of egress is free of clutter and obstructions. There are currently six residents in care at the facility, none of which are receiving hospice care. Bathrooms faucets and toilets were operational. Water temperature tested at approximately 120F degrees. LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted. Food menu was also posted and visible. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Smoke detectors and carbon monoxide detectors tested operational. Fire extinguisher was observed to be fully charged with up-to-date maintenance. Sharps were observed locked in a drawer in the kitchen. LPA observed cleaning supplies to be stored in a locked cabinet under the kitchen sink. The laundry area is also observed to be secured with a lock. The medication central storage was observed to be locked. LPA reviewed six resident files and four staff files. Based on the observations made during today’s inspection, four type B deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations.
