On May 13, 2026, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of a required annual inspection. LPA explained the purpose for the visit and was greeted and granted entry by staff on duty. During the visit, staff on duty contacted the facility administrator (AD) Stephen Venanzi about the visit. For this visit, there are two staff members on duty, both of which are background cleared and associated. AD later arrived to assist with the inspection. The PUB475 ‘See Something, Say Something’ poster was observed to be located near the front entrance. LPA observed that Stephen Venanzi has renewed his Administrator's certificate on April 28, 2026. The facility is a Residential Care facility for the Elderly (RCFE) licensed for six residents, six of which may be non-ambulatory, none of which may be bedridden, and a hospice waiver for three. LPA toured the interior and exterior portions of the facility with AD. For this visit, there are a total of five non-ambulatory residents in care, one of which is on hospice, and none are bedridden. The facility is a single story home. There are a total of four bedrooms, two of which are private resident bedrooms and two are shared bedrooms. LPA toured each bedroom with the AD and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and free of any hazards. Smoke and carbon monoxide detectors as well as auditory exit alarms were tested and operational. There are a total of two full bathrooms, both are for residents and staff. Continued on LIC 809-C. Bathrooms were observed to be in good repair, toilets and faucets were operational and showers were equipped with grab bars and non-skid floor mats. Water temperature in the bathrooms were measured to be between 113.9 and 114.9 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked in a kitchen cabinet and inaccessible to residents in care. Fire extinguisher was charged, mounted and located in the kitchen. Fire extinguisher was purchased in 2025. LPA observed all the required postings in the hallway. LPA observed the facility conducted their last emergency disaster drill on April 6, 2026. Facility had back-up emergency food and water supply, located in the garage. LPA observed that the First Aid kit had all the required components. Medications were observed to be locked in a medication cabinet in the kitchen, inaccessible to residents in care. Chemicals were observed to be locked in the garage and underneath the kitchen sink. LPA observed the door leading to the attached two car garage is kept locked and inaccessible to residents in care. The garage is used for storage and laundry. For the exterior portion, LPA observed patio furniture under shading, and the grounds were free of any hazards or obstructions. There is one exit gate in the backyard that can be opened in case of an emergency. No bodies of water were observed. During this visit, five resident files and two staff files were reviewed. All staff are background cleared and associated with the facility. LPA reviewed residents’ medication and medication records and two resident interviews were conducted. The facility has valid liability insurance that expires on August 6, 2026. Based on today's observations, there are no deficiencies being cited per Title 22 of the California Code of Regulations. A Technical violation was issued. An exit interview was conducted with the licensee. Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Stephen Venanzi and explained the reason for the inspection. There are currently five residents at the facility of which all were seen engaging in musical therapy with the musical therapist that comes to the facility. The facility is a one-story home with four resident bedrooms, one caregiver bedroom, two bathrooms, kitchen, living room, dining room, two car garage, and backyard. Facility appears clean, safe, and sanitary. LPA observed the kitchen to be free of debris and vermin. LPA observed a two-day perishable and seven-day non-perishable food supply ready and on hand. LPA observed the toxins to be stored under the locked kitchen sink made inaccessible to residents in care. LPA observed the knives and sharps to be locked in a drawer in the kitchen located by the stove made inaccessible to residents in care. LPA observed the centrally stored medication to be stored in a locked cabinet located in the kitchen and made inaccessible to residents in care. LPA observed a fire extinguisher located in the kitchen to be fully charged and serviced. LPA observed all resident bedrooms to have the required components and furnishings. LPA observed that all doors leading to the outside had alarmed exits. LPA observed an extra clean linen supply located in the hallway for resident use. LPA observed resident bathrooms to have toilet paper, paper towels, and non-slip mats. LPA tested the water to be 116.6 degrees Fahrenheit in the resident bathrooms. LPA observed the staff bedroom to be locked made inaccessible to residents in care. LPA observed the emergency food and water supply to be located in the garage. Continue on 809-C LPA observed extra food storage and toxins to be stored separate in the locked garage made inaccessible to residents in care. LPA observed the backyard to have a shaded seating area for resident use and is free of obstructions. LPA observed birds in bird cages in the backyard that are well maintained and appear to be clean and free of odor. LPA reviewed resident files, no discrepancies were observed. LPA reviewed resident medications, no discrepancies were observed. LPA reviewed staff files, no discrepancies were observed. LPA tested the carbon monoxide and smoke detectors and they were found to be operational. All staff present have been background cleared and are associated to the facility. Based on today’s inspection no deficiencies are being noted per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Amy Rodgers conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was greeted and allowed entry into the facility by care giver Romuald Adona to whom LPA discussed the purpose of the visit..Administrator Stephen Venanzi Jr. and Licensee/Administrator Stephen Venanzi later join the visit. According to the facility’s license, the facility has a maximum capacity of six (6) non-ambulatory clients with a hospice waiver for 3. LPA, accompanied by care giver and administrator Stephen Venanzi Jr. toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. The facility’s ambient internal temperature was compliant. Hot water temperature at taps accessible to clients were all compliant. There was at least 2 days of perishable food, and at least 7 days non-perishable food present, all safely stored. Cooking/dining equipment and utensils were present. There were no sharp objects, toxic chemicals/poisons, or open-faced heaters accessible to clients. Centrally stored medications were properly stored and locked in medication cabinet. Medication logs and medications reviewed were current and medications appear to be administered according to the label instructions. [continued on 809-C] [Continued form 809] No pools or bodies of water on the premises. Per Licensee/Administrator Venanzi, no firearms or ammunition are kept at the facility. Smoke alarms, carbon monoxide detectors, emergency lighting, and facility telephone were all working. Fire extinguisher(s) were serviced within the last 12 months. First aid kit were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. LPA interviewed staff members and reviewed the staff records/files. LPA interviews did not raise any licensing concerns. LPA interviews with clients did not raise any licensing concerns. Staff records review verified that all staff records are complete and compliant. All direct care staff have First Aide/CPR certificates, and staff training. Resident records reviewed and confirmed compliant. Administrator’s certification is current. LPA reviewed the theft and loss policy and procedures. Transportation procedures are compliant. LPA observed that residents were being treated with dignity by staff, and there were sufficient staff on duty to meet resident’s needs An exit interview was conducted with Licensee/ Administrator Stephen Venanzi , to whom copies of this report, and the Licensee/Appeal Rights (LIC9058 03/22) were provided at the conclusion of the visit.the state’s words, verbatim · CDSS document, Aug 9, 2024
