May 15, 2026Facility evaluation reportReport on file
Type of visit: Required - 1 Year
On May 15, 2026, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by care giving staff after explaining the purpose for the visit. Administrator (AD) Mark Cruz was notified via telephone and later arrived to assist with the inspection. LPA observed that Mark Cruz has a valid Administrator certificate which expires on December 2, 2026. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents and has a hospice waiver for four. The facility is a single story home which consist of five resident bedrooms, one of which is shared, one staff bedroom, three bathrooms, a living room, a dining room, a kitchen, and an attached two car garage. LPA, accompanied by the AD, conducted a tour of the interior portions of the facility. On today's visit, the facility currently has a census of six residents, however, there were only five residents physically present. There were also three care giving staff present during the visit. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall by the entryway of the facility. LPA inspected the five resident bedrooms and observed them to be clear of any hazards. LPA observed resident bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA observed additional linens to be stored in a hallway closet. LPA observed the staff bedroom to be kept locked and inaccessible to residents in care. LPA inspected the two shared resident bathrooms and observed them to be clean. Resident bathrooms were equipped with grab bars and non-skid floor mats. Faucets and toilets were operational. Hot water temperature measured between 105.2 and 105.9 degrees Fahrenheit. CONTINUED ON LIC809-C LPA observed the facility has a two day perishable and a seven day non-perishable food supply in the kitchen. LPA observed kitchen knives and sharps to be stored in a locked box stored under the kitchen sink. LPA observed a fire extinguisher to be mounted on the wall in the kitchen and it was observed to be charged and serviced as of March 9, 2026. LPA tested the wired smoke detectors/carbon monoxide detectors which tested operational. LPA observed the facility conducted their most recent emergency disaster drill on January 22, 2026. LPA observed the centrally stored medication to be kept in a locked closet. LPA observed there is also a first aid kit stored near the kitchen and it has all the required components. LPA inspected the attached two car garage and observed it to be used for storage and laundry. LPA observed chemicals and toxins to be stored in a locked cabinet located in the garage. LPA observed the facility also has a three day emergency food and water supply stored in the garage. LPA, accompanied by the AD, conducted a tour of the exterior portions of the facility. LPA observed the exterior to be clear of any obstructions or hazards. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gate of the facility to be self-latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all six resident files. All the required documentation were present and current in the resident files reviewed. LPA reviewed the residents' medication and medication administration records. LPA reviewed three staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, no deficiencies are being cited per Title 22 of the California Code of Regulations. May 12, 2025Facility evaluation reportReport on file
Type of visit: Required - 1 Year
On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by facility caregiving staff after after introducing himself and stating the purpose of the visit. Administrator Mark Ryan Cruz was notified of the visit via telephone and arrived later to assist. There are currently five residents in care, three of which are receiving hospice care at this time out of an authorized hospice waiver capacity of four residents. LPA observed residents relaxing in the facility's common living areas. LPA accompanied by facility staff toured the physical plant. The facility is a one-story house with a detached garage. The facility has four private and one shared bedrooms along with one staff room that is verified to be locked. There are two shared bathrooms throughout the facility, one of which designated for use by staff, as well as one en-suite bathroom in the shared bedroom. Bedrooms appear clean and sanitary. Some of the residents are using half rails or full rails for postural supports. Adequate orders present in all files or hospice plans of care reviewed. All resident bedrooms have the required furnishings. Bathrooms appear clean and sanitary. Hot water temperature measured at 110F in two bathrooms with faucets used for personal grooming. LPA observed the kitchen has a minimum two (2) day perishable and seven (7) day non-perishable food supply. Sharp items, cleaning supplies and the medication central storage are verified to be secured. CONTINUED ON FORM LIC809-C CONTINUED FROM LIC809 The mounted fire extinguisher is charged, with up-to-date maintenance tag attached. LPA tested the smoke and carbon monoxide detectors which were found to be operational. The garage is inaccessible to residents and used for laundry as well as additional storage of food and supplies. LPA and facility staff toured the outside of the facility. LPA observed an shaded outdoor seating area with furniture for resident use. There are self-latching gates on both sides of the property and the routes of egress are free of clutter and obstructions. There are no bodies of water on the premises. Facility does not utilize locked perimeters or delayed egress. LPA reviewed five resident records which included all necessary components. LPA reviewed resident medication records and prescription orders for all residents with no discrepancies observed. Oxygen is currently in use with adequate signs posted. There are no residents currently assessed as being bedridden at the time of the visit. Five resident interviews were attempted or conducted during the visit along with staff interviews. LPA reviewed staff records for the three caregivers present during the visit. All staff members listed on the roster form LIC500 are background cleared and associated to the licensed location accurately. Present staff members have current CPR certificates on file. 2025 annual training provided. Disaster drills are conducted quarterly with proof of attendance provided. Based on the observations made during today’s visit, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of conducting the Required Annual Inspection. LPA was greeted and granted entry by facility caregiving staff after stating the purpose of the visit. Administrator Mark Cruz was notified by telephone and arrived later to assist. During the inspection, LPA and facility staff conducted a tour of the physical plant and observed the following: The facility is a one-story home with five private resident bedrooms, three bathrooms and one locked staff room in addition to the common living areas. All resident bedrooms had the required furnishings. LPA observed all beds had linens and blankets and an adequate additional supply is present. There are orders on file for the postural supports in use. The backyard has a shaded area and the routes of egress are free of clutter and obstructions. There are currently six residents in care at the facility, two of which are receiving hospice care. Residents are observed to be clean and appear well taken care of. Bathrooms faucets and toilets were operational. Water temperature was verified to be within acceptable range. LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food as required. Combined smoke and carbon monoxide detectors tested operational. Fire extinguisher present is observed to be fully charged with up-to-date maintenance. Medication, sharp items and cleaning supplies were confirmed to be inaccessible throughout the physical plant. The medication central storage was also observed to be secure and reviewed for accuracy during the visit. LPA reviewed six resident files and three staff files. The two staff members on duty were interviewed during the visit. Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. One Technical Violation Advisory Note was also provided to the licensee. Feb 27, 2024Complaint investigation reportUnsubstantiated
Allegation investigated: Staff do not maintain facility in good repair.
On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of delivering findings into the investigation of the allegation listed above. LPA was greeted and granted entry by facility staff after stating the purpose of the visit. An initial complaint investigation took place on February 21, 2024. LPA accompanied by administrator conducted a tour of the facility's physical plant and observed the presence of Caution tape around the outdoor patio structure as well as wear and tear and rot on the patio structure. Administrator states that access has been restricted pending repairs. Following the visit, facility staff provided LPA with a copy of email and text messages between the licensee and the landlord for the property the facility is licensed at demonstrating that damage to the structure had been flagged on December 14, 2022. CONTINUED ON FORM LIC9099-C Unsubstantiated CONTINUED FROM FORM LIC9099 Regarding the allegation that Staff do not maintain facility in good repair, the following has been concluded: Based on observation, interviews and a review of records, it was established that there is currently extensive damage to parts of the backyard patio structure and roof carpentry. It appears however that facility staff did their due diligence by informing the landlord of the situation as early as December 12, 2022 and followed up when an extensive weather event worsened the damage in early February 2024. A review of the current lease agreement did not evidence clearly the chain of liability for the damage. As a result, the allegation is found to be Unsubstantiated, meaning that although the allegation listed may have happened or is valid, there is not sufficient evidence to demonstrate that the alleged violation occurred. Type of visit: Case Management - Deficiencies
On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of conducting a case management inspection documenting a deficiency observed during the investigation of complaint #22-AS-20240214122109. Due to the presence of structural damage to the outside patio, the backyard of the facility is currently found to be inaccessible to residents in care. A type B deficiency is cited on the attached form LIC809-D. Per CCR 87219(h)(2) on Planned Activities: "Facilities shall provide sufficient space to accommodate (...) outdoor activities. Activities shall be encouraged by provision of: Outdoor activity areas which are easily accessible to residents and protected from traffic. This requirement is not met as evidenced by: Based on observation and interviews, the backyard is currently inaccessible to residents in care due to damage sustained to the patio shade structure.
