On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by facility caregiving staff after introducing himself and stating the purpose of the visit. Administrator Jennifer Perez was also present on the premises and available to assist. There are currently six residents in care, two of which are receiving hospice care at this time. Residents are observed relaxing in their respective bedrooms and in the common areas. LPA accompanied by facility staff toured the physical plant. The facility is a two-story house with three shared bedrooms used by residents in addition to one staff room. The second level of the house is for the exclusive use of the licensee and their family. There are two bathrooms used by residents. None of the residents are bedridden at this time. All occupied bedrooms appear clean and sanitary. Postural supports orders are verified to be on file. One resident has full rails in place but has been discharged from hospice. Consultation provided. All resident bedrooms have the required furnishings. Bathrooms appear clean and sanitary and are all equipped with grab bars and slip mats. Hot water temperature measured at 114F and 118F in the shared bathrooms. LPA observed the kitchen has a minimum two (2) day perishable and seven (7) day non-perishable food supply. Sharp items, cleaning supplies and the medication central storage located in the kitchen and living room are verified to be secure. CONTINUED ON FORM LIC809-C CONTINUED FROM FORM LIC809 A wall-mounted fire extinguisher is present and verified to be charged, with a purchase receipt dated August 2025. Carbon monoxide and smoke detectors are present and operational. Emergency water supplies are stored in the garage. LPA and facility staff toured the outside of the facility. LPA observed a shaded outdoor seating area with outdoor furniture for resident use in the backyard. The identified routes of egress is free of clutter and obstructions. There are self-latching gates on both sides of the premises. There are no bodies of water on the premises and the facility does not utilize either locked perimeters or delayed egress. LPA reviewed six resident records which were found to include all necessary elements of documentation. LPA reviewed resident medication records and prescription orders with no discrepancies observed. LPA reviewed staff records for three staff members during the visit. All staff members are background cleared and associated. Proof of current CPR training reviewed. Disaster drills are conducted quarterly. The administrator certificate is current. Based on the observation conducted during the present visit, no deficiencies are being cited per Title 22 of the California Code of Regulations. LPA was greeted and granted entry by the caregiving staff after introducing himself and stating the purpose of the visit. Administrator Jennifer Perez was notified of the visit via telephone and arrived later to assist. LPA accompanied by facility staff conducted a tour of the physical plant and observed the following: the facility is a two-story home with an attached garage. The upper level is only for use by licensee and their family and staff and is kept inaccessible to residents as verified during the visit. The facility's ground level has three shared bedrooms and two shared bathrooms. All bathrooms are observed to be equipped with grab bars and slip mats. All resident bedrooms have the required furnishings. Bathrooms faucets and toilets are operational. Water temperature was measured at 130.5F and 135.2F in two bathrooms used for personal hygiene. Type B citation issued. LPA observed all beds have linen and blankets. There are half rails used for postural supports for three residents. Physician orders for two residents reviewed. A third resident received half rails ordered upon their admission, physician orders pending from skilled nursing. There are currently six residents admitted to the facility with one of these residents currently admitted to rehabilitative care. Two residents are receiving hospice care out of a waiver capacity of two possible hospice residents. LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted. One occurrence of an evacuation drill has been conducted in 2024 for both shifts. Type B citation issued due to the quarterly frequency not being followed. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Smoke and carbon monoxide detectors tested operational. Fire extinguisher present is fully charged and has been purchased in 2024 as demonstrated by the receipt on display. CONTINUED ON FORM LIC809-C CONTINUED FROM FORM LIC809 There is adequately shaded outside space with outdoor furniture present. There are self-latching gates on each side of the property. The route of egress on one side is free of obstructions, however there are construction materials partially blocking the path to the second gate. Technical Violation Advisory note issued. Cleaning products are observed to placed in a lockable cabinet under the sink which staff state is locked at night. Consultation provided to remind licensee of the need to lock the cabinet when not attended directly by staff. The medication central storage was also observed to be secure and reviewed to be accurate and up to date with the resident's prescription orders. LPA reviewed six resident files along with two staff files. Resident records include all necessary components. All staff members on the facility's roster are confirmed to be cleared and associated with this particular licensed location. Training verified to be up to date. CPR stated to have been conducted. Proof of training to be sent to LPA as soon as possible. Health screenings are on file for both staff members. Proof of liability insurance coverage could not be provided. Licensee states that finding an insurance company to renew their previous contract has been challenging. Type B citation issued. Based on the observations made during today’s inspection, three deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations and four Advisory Notes issued. Allegation investigated: Uncleared staff providing care and supervision to residents. Staff did not safeguard resident’s money.

On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of conducting the initial investigation into the allegations listed above. LPA was greeted and granted entry by caregiving staff after stating the purpose of the visit. Administrator Jennifer Perez was notified of the visit via telephone and arrived later to assist with the visit. LPA listed the allegations investigated during the initial phone conversation. During the visit, LPA requested and obtained records for former resident R1. LPA reviewed the facility's admission agreement and its section on theft and loss, the facility's theft and loss policy as well as the inventory established upon admission. The list of all facility employees was additionally provided by the facility administrator. CONTINUED ON FORM LIC9099-C Unfounded CONTINUED FROM LIC9099 Regarding the allegation that Uncleared staff providing care and supervision to residents, the following has been concluded: Based on interviews with administrator and staff members along with a review of the current facility roster, all individuals involved in providing care for residents were shown to be cleared and associated to the facility. The allegation is therefore determined to be Unfounded, meaning that the allegation is false, could not have happened and/or is without a reasonable basis. Regarding the allegation that Staff did not safeguard resident’s money, the following has been concluded: Based on a review of the terms of the admission agreement, facility theft and loss policy and the inventory of safeguarded cash and valuables filled by resident R1 upon admission, it was determined that the alleged missing funds were not placed under the facility's safeguarding responsibility. Additionally, the terms of the admission agreement indicate that "The Company is not responsible for any cash, valuables or personal property brought into the facility unless these items are delivered to the licensee/administrator for safeguarding". As evidenced by the three pages of inventorized property present in R1's records, cash funds were not placed under the facility's responsibility during the period the resident was admitted at the facility. As a result, the allegation is found to be Unfounded, meaning that the allegation is false, could not have happened and/or is without a reasonable basis. This agency has investigated this complaint.
