On June 22, 2026, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of a required annual inspection. LPA explained the purpose for the visit and was greeted and granted entry by staff on duty. During the visit, staff on duty contacted the facility administrator (AD) Jennifer Perez about the visit. For this visit, there are three staff members on duty, all of which are background cleared and associated. AD later arrived to assist with the inspection. The PUB475 ‘See Something, Say Something’ poster was observed to be located near the front entrance. LPA observed that Jennifer Perez has a valid Administrator certificate which expires on January 1, 2027. The facility is a Residential Care facility for the Elderly (RCFE) licensed for six residents, five of which may be non-ambulatory, one of which may be ambulatory, none may be bedridden, and a hospice waiver for three. LPA toured the interior and exterior portions of the facility with AD. For this visit, there are a total of four non-ambulatory and one ambulatory residents in care, one of which is on hospice, and none are bedridden. The facility is a single story home. There are a total of six bedrooms, all of which are private resident bedrooms. LPA toured each bedroom with the AD and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and free of any hazards. Smoke and carbon monoxide detectors as well as auditory exit alarms were tested and operational. There are a total of two bathrooms for residents and staff. Continued on LIC 809-C. Bathrooms were observed to be in good repair, toilets and faucets were operational and showers were equipped with grab bars and non-skid floor mats. Water temperature in the bathrooms were measured to be between 108.1 and 109.5 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked in a kitchen sink and inaccessible to residents in care. Fire extinguisher was charged and located in the kitchen. Fire extinguisher was purchased on February 2, 2026. LPA observed all the required postings in the kitchen. LPA observed the facility conducted their last emergency disaster drill on March 7, 2026. Facility had back-up emergency food and water supply, located in the garage. LPA observed that the First Aid kit had all the required components. Medications were observed to be locked in a medication cabinet in the hallway, inaccessible to residents in care. Chemicals were observed to be locked in the laundry room. LPA observed the door leading to the attached two car garage is kept locked and inaccessible to residents in care. The garage is used for storage. For the exterior portion, LPA observed patio furniture under shading, and the grounds were free of any hazards or obstructions. There is one self-closing gate in the backyard that can be opened in case of an emergency. There is a pool in the backyard surrounded by a fence that is 5 feet tall. The fence is locked and inaccessible to residents in care. During this visit, five resident files and two staff files were reviewed. All staff are background cleared and associated with the facility. LPA reviewed residents’ medication and medication records and one resident interview was conducted. Based on today's observations, there are deficiencies being cited per Title 22 of the California Code of Regulations. An exit interview was conducted with Administrator. Licensing Program Analyst (LPA) Celine Rodriguez conducted an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPA Rodriguez explained reason for visit and was greeted and granted entry by staff on duty. For this visit, there are 6 residents in care, of which 3 are on hospice. For this visit, LPA Rodriguez met with house manager (S1) Efrain Matheus. LPA Rodriguez observed the facility administrator (AD) certificate for Hermelinda Perez which expires on 10/30/2026. LPA Rodriguez observed that the PUB475 "See Something, Say Something" poster was posted at the entrance of the facility. The facility has a total of 6 bedrooms, of which all 6 are private resident rooms. LPA Rodriguez toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Facility was observed to have 3 bathrooms, of which 2 are located inside the facility, and 1 outside, however it was verified that the bathroom located outside is not for resident use. Bathrooms were observed to be in good repair, toilets were operational, and grab bars and non-skid floor mats were provided. Smoke and carbon monoxide detector and auditory exit alarms were tested and operational. Hot water was measured at 112.3 degrees Fahrenheit. LPA Rodriguez observed that the facility had a two-day supply of perishables and a seven-day supply of non-perishable food. LPA Rodriguez observed hallways were free of obstructions. For the exterior portion, LPA Rodriguez observed patio furniture under shading, and the grounds were free of any hazards. There is 1 gate in the backyard, which is self-closing and self-latching. LPA Rodriguez observed that there is a body of water located in the backyard, however it is secured by a locked gate. LPA Rodriguez observed that the facility has fire extinguishers that were charged, mounted and located in the kitchen with a maintenance tag dated for 2025. Sharp items, and medications were observed to be locked and inaccessible to residents in care. Cleaning supplies were observed to be locked in the garage and laundry room. LPA Rodriguez observed the Emergency and Disaster Plan posted in the facility binder which is located at the facility entrance, and that the facility has emergency supplies located in the garage. LPA Rodriguez reviewed all six resident files. LPA Rodriguez reviewed staff files, and observed that current staff members are cleared and associated to the facility. For today's visit no deficiencies were issued per Title 22 Division 6 of the California Code of Regulations. No citations were issued. An exit interview was conducted with S1 Matheus. Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA met with the Administrator and explained the reason for the visit. The Administrator, Hermelinda Perez's Administrator's certificate expires on October 30, 2024. LPA and the Administrator toured the facility. LPA observed the See Something, Say Something poster (PUB 475) posted in the entry way of the facility. The facility is a single story home with 6 bedrooms, 2 bathrooms, living room, eat in kitchen and a 2 car garage. LPA observed the fireplace in the living room is screened. LPA toured the kitchen. The kitchen is clean and organized. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. The 4 burner gas stove lights unassisted. LPA observed the knives are kept locked in the garage. LPA observed the fire extinguisher in the kitchen is fully charged. LPA toured the resident rooms. LPA observed all resident rooms are clean and organized. All resident beds had the required linens. LPA observed both bathrooms are clean and operational. How water measured 109.0 degrees Fahrenheit in both bathrooms. LPA toured the garage. The garage is used for storage and kept locked. LPA observed extra furniture, supplies and perishable food stored in the garage refrigerator. The living room has chairs and a couch for residents to sit and there is a TV mounted above the fireplace for residents to watch TV. LPA observed the medication is stored in the locked in a hallway closet. LPA toured the backyard. LPA observed a covered patio in the backyard. There is a pool in the backyard which is surrounded by a fence that measures 60 inches tall. The pool gate is secured and the pool is inaccessible to residents. The backyard exit gate is operational and is self closing. There is storage room next to the garage that is used to store old furniture and wheelchairs. No obstacles or hazards observed inside or outside of the facility. LPA reviewed 2 staff files, no discrepancies observed. The 2 staff had the required 20 hours of annual training. LPA reviewed 6 resident files and medications, no discrepancies observed. Smoke detectors/carbon monoxide detectors tested operational. No deficiencies observed during the visit.
