On 01/02/2026 Licensing Program Analyst (LPA) Gorban arrived at the facility unannounced to conduct a Case Management regarding an incident that was reported on 11/05/25. Report stated that multiple residents claimed that during the period of time from August to November of 2025 personal belongings and valuable items went missing from resident rooms. During this visit LPA met with senior executive director Ryan Banner and reviewed the facility records. Based on records review, sixteen residents reported the missing items. Based on records review, prior to the event, eleven out of sixteen residents refused to file personal property inventory list and signed declination form. Additional files requested for review. During this visit LPA was notified that construction is taking place by The Core Group remodeling administration offices, lobby space, clinic, and adding new bistro. Anticipated completion date is August, 2026. Executive director explained the remodeling is not interfering with residents daily living functions. On 11/19/2025 Licensing Program Analyst (LPA) Gorban arrived at the facility unannounced to conduct a Case Management regarding an incident that was reported on 11/05/25 by Administrator Ryan Banner reporting that multiple residents reported to administrator of missing personal belongings and valuable items from resident rooms. During this visit LPA met with senior executive assistant Geoven Snaer and followed up with the facility investigation. LPA requested facility files for review and follow up. Mean time, facility implemented visitors badges last week and extra video cameras monitoring facility perimeters for security measures. During this visit LPA was notified that due to construction by California American Water services shutting off water to the facility tomorrow for one day, on 11/20/25 from 8AM to 530 PM. Administration is working on providing an alternative ways for residents and staff have full access to water supply, included showers, hand washing stations hands and etc. Administrator also working on providing notification to residents and Licensing office regarding water shut off. LPA requested copy of police report and medical records regarding this incident. LPA will return at a later date if necessary regarding this incident. On 09/04/2025, Licensing Program Analyst (LPA) V Gorban arrived at the facility unannounced to conduct Required Annual Inspection. LPA met with Senior Executive Assistant (SEA) Geoven Snaer. LPA conducted tour inside and out of facility with SEA. Residents observed at the facility after lunch time. The facility was observed to be at a comfortable temperature of 76 degrees, clean, in good repair, and no passageway obstructions or fire hazards observed. Fire extinguisher was observed with a service date of 05/20/2025. Last disaster drill recorded on June 12,2025 Dining area and Kitchen were toured. An adequate supply of perishable and non-perishable food was observed to be properly stored in freezer, refrigerator, and pantry. Food is delivered by Sisco Foods twice a week. Refrigerator temperature was maintained at 39.0-degree F. and freezer was maintained at 2-degree F. Residents' rooms were toured and observed with adequately furnished with bed, dresser, and adequate lighting. Hot water temperature tested and within regulation requirements. LPA observed securely fastened grab bar and non-skid mat in shower area. Medications were stored in a locked medication room in a medication cart. Medications records were reviewed. First Aid Kit was stored in medication room and observed with all required items. Adequate PPE supplies was observed. LPA toured laundry room and observed chemicals were stored and locked for staff use only. Facility courtyard was toured and observed to be free from debris. There was outdoor seating available for the residents. Report continues on attached LIC809-C Residents’ files were reviewed to have updated emergency contact, Admission agreement, Needs and Services Plan and Pre-Appraisal Plan. A sample of staff files were reviewed. Staff files were observed to have current First Aid/CPR, Health screening, and Personnel record. On 08/19/2024, Licensing Program Analyst (LPA) V Gorban arrived at the facility unannounced to conduct Required Annual Inspection. LPA met with Administrator (AD) Elvyra Abare. LPA conducted tour inside and out of facility with AD. Residents observed at the facility during quite time resting before dinner. The facility was observed to be at a comfortable temperature of 74 degrees, clean, in good repair, and no passageway obstructions or fire hazards observed. Fire extinguisher was observed with a service date of 05/29/2024 Dining area and Kitchen were toured. An adequate supply of perishable and non-perishable food was observed to be properly stored in walk-in freezer, walk-in refrigerator, and pantry. Food is delivered twice a week. Refrigerator temperature was maintained at 42.0-degree F. and freezer was maintained at -5-degree F. LPA toured resident bedrooms. Residents' rooms were toured and observed with adequately furnished with bed, dresser, and adequate lighting. Hot water temperature tested at 110 degrees F. LPA observed securely fastened grab bar and non-skid mat in shower area. Medications were stored in a locked medication room in a medication cart. Medications records were reviewed. First Aid Kit was stored in medication room and observed with all required items. Adequate PPE supplies was observed. LPA toured laundry room and observed chemicals were stored and locked. Facility courtyard was toured and observed to be free from debris. There was outdoor seating available for the residents.
