Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with Facility Administrator, Sara Post Continual Administrator's Certification expires 08/29/2026. There are currently 167 residents who reside at this home and there is 14 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector and performs disaster drills as required. Water temperature was tested at 116 degrees in room 132, 115 degrees room 152, and 113 degrees in room 326. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. LPA reviewed a sample of staff and resident files, Plan of Operation, Emergency Disaster Plan, and Infection Control Plan. The facilities Emergency Disaster Plan does not have Licensing Fresno Regional Office phone number listed. Resident 1's medication is not documented correctly on Centrally Stored Medication record. There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Type of visit: Case Management - Incident

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit to the facility to conduct a Case Management/Incident visit. LPA met with facility Executive Director Sara Post and explained the purpose of this visit. On December 25, 2025, the Executive Director was notified by the daughter of the Resident 1 regarding a fraudulent charge Resident 1's bank account. Resident 1's daughter, expressed concern that her father’s checkbook had been left out on the dining room table in his apartment, potentially leading to unauthorized access. On December 26, 2025, Resident 1's daughter informed the Executive Director that Chase Bank had provided her with copies of the checks in question and identified the individual to whom the checks were made out. Facility Staff 1 was the individual listed on the checks. Facility Executive Director conducted an internal investigation and placed Staff 1 on suspension pending further inquiry. Staff 1 had written two checks, each in the amount of $2,000. The Executive Director reported the incident to local law enforcement, and Salinas Police Department Officer arrived on scene, referencing case number 25-120827. Following the suspension on 12/16/2025, Staff 1 was arrested with charges of embezzlement, forgery, and elder theft by caretaker. The Executive Director called and left a voicemail for the local ombudsman and faxed the SOC 341 on 12/27/25. The Executive Director called and left a voicemail for DSS and faxed the SOC 341 on 12/27/25. The Executive Director conducted an in-service training session for staff covering the following topics: Resident Rights, Theft and Loss Policy, Mandated Reporter Policy, and Safeguarding Resident Personal Property. Resident 1's bank restored the amount of the stolen checks, and Resident 1 will no longer retain checks in their apartment , as the daughter will handle payment to the beautician. Resident 1 will be reminded about safeguarding items in their apartments at the upcoming resident council, and a resident letter regarding safeguarding resident personal property will be sent. Allegation investigated: Staff did not assist residents during an emergency alarm event. Licensee did not ensure elevator was maintained in good repair.

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to investigate the allegations listed above. LPA met with facility Administrator Sara Post, and explained the purpose of today's visit. Regarding the allegation staff did not assist residents during an emergency alarm event. Based on interviews conducted there was no actual fire or emergency, no residents were trapped in the elevator, and no resident required evacuation or staff intervention. While the Reporting Party expressed concern that resident was startled by the alarm, there is no evidence that staff failed to respond appropriately or that the facility neglected to maintain the elevator in good repair. Therefore, this complaint is deemed unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Unsubstantiated Regarding the allegation Licensee did not ensure elevator was maintained in good repair. Based on interviews conducted, the investigation revealed that a maintenance technician was working on a separate alarm system when the fire alarm was accidentally triggered. As designed, the elevator ceased operation in response to the alarm; however, the elevator was in good working order outside of the alarm activation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. There was no emergency or fire inside the facility. The facility staff did not come to the third floor to reassure residents there was no emergency inside the facility building. LPA met with Facility staff Andrea Ramirez, Continual Administrator's Certification expires for Administrator Sara Post is 08/29/2026. There are currently 167 residents who reside at this home and there is 15 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. LPA reviewed 3 facility resident hospice care plans. 2 of the 3 facility hospital binders did not have required hospice care plan. There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. Licensing Program Analyst (LPA) Sarah Hurt arrived at the facility unannounced on October 21, 2024 at 01:00 p.m. to investigate the above allegations. LPA met with facility Business Office Manager, Andrea Ramirez and explained the purpose for today’s visit. Regarding the allegation resident was billed for services not rendered. Resident 1's assesment stated they need assistance with 6 to 10 medications. Resident 1's "medication clarification list" signed by Physician on 09/03/2924 lists 6 as needed medications in addition to the 5 routine medications. Resident 1 is being billed for assistance with 6 to11 medications including PRN's. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Allegation investigated: Staff do not provide daily activities for residents

Licensing Program Analyst (LPA) Sarah Hurt arrived at the facility unannounced on July 26,2024 at 5:00 p.m. to investigate the above allegation. LPA met with Facility Medication Technician, Jewel Sanchez ,and explained the purpose for today’s visit. Regarding the allegation staff do not provide daily activities. LPA Hurt spoke with four facility residents who all stated the facility has regular activities including on weekends. LPA Hurt observed several postings throughout the facility informing residents of acitvities being held. LPA Hurt observed a large activities calendar posted in the hallway during visits to the facility on 05/31/2024, and 07/26/2024. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with Administrator Sara Post Continual Administrator's Certification expires 08/29/2024. There are currently 158 residents who reside at this home and there is 8 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector and performs disaster drills as required. Water temperature was tested between 105 and 120 degrees (several resident rooms tested.) First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. LPA Hurt reviewed facility 610E Emergency Disaster Plan. LPA interviewed several facility staff and residents. LPA reviewed several facility resident files, and staff files. LPA confirmed staff present is background cleared. There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. Allegation investigated: Facility did not seek medical attention in a timely manner. Facility did not provide adequate supervision resulting in resident eloping from the facility.

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to deliver findings on the above allegations. LPA met with facility staff Administrator, Sara Post and explained the purpose of today's visit. Regarding the allegation Facility did not seek medical attention in a timely manner. Resident 1 fell on their bathroom floor on 08/03/2023 at approximately 04:30 a.m. Resident 1 told facility staff they were haivng pain in their legs which caused them to lose balance and fall. Resident 1 was assesed and given medication for pain at the facility. Resident 1 was taken to the hospital at approximately 12:40 p.m., and was diagnosed with a fractured femur. Based on records reviewed, and interviews which were conducted , the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED Substantiated Continued.. Regarding the allegation Facility did not provide adequate supervision resulting in resident eloping from the facility. Resident 1 eloped from the facility on 06/30/2023. Facility staff heard the Memory Care area door alarm alert and went to look out the door but did not see any residents outside. Facility staff cleared the alarm. Resident 1 was did leave the facility and was already around the corner out of site, and was eventually recovered down the street. Based on records reviewed, and interviews which were conducted , the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED The following Deficiencies are being cited Per Title 22 Regulations. Type of visit: Required - 1 Year

Licensing Program Analysts (LPA's) Sarah Hurt and Lisa Salazar conducted an unannounced visit today for the facility’s annual inspection. LPA met with Administrator Sara Post Continual Administrator's Certification expires 08/29/2024. There are currently 163 residents who reside at this facility. LPA's inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. LPA's will return at a later date to review files, and conclude annual inspection. There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22.
