Licensing Program Analyst (LPA) Ruth Martinez is conducting this unannounced visit for the purpose of completing an annual required inspection. LPA arrived at the facility and was greeted and granted entry by Claudia Pulida, Administration and LPA explained the nature of the visit. There are three residents at the facility and there are no residents on hospice. LPA accompanied by the Administrator, began the tour of the inside and outside of the facility. LPA observed required department postings throughout the facility. Facility stays within the capacity limitations. There is a minimum of one week of non-perishables foods and two days of perishables foods available. There is additional food storage in a spare refrigerator and freezer located in the garage. The facility is maintained at a comfortable temperature. LPA inspected that medication is centrally stored in a safe locked storage cabinet located in the kitchen. LPA reviewed medication and observed medication was labeled and stored inaccessible to residents in care. LPA inspected the bathroom. LPA measured the hot water temperature which measured 115.8 Fahrenheit degrees. All bathrooms observed to have a supply of soap, toilet paper and towels. Bathrooms are equipped with required safety measures such as non-skid mats and grab bars. Lighting is sufficient to ensure safety and comfort. The facility is equipped with sufficient hand hygiene, cleaning, and disinfecting supplies. LPA observed that toxic chemicals, cleaning solutions and disinfectants are stored underneath the kitchen sink and in a locked storage cabinet in the garage. The facility has an available clean supply of linens. LPA inspected residents’ bedrooms which has sufficient lighting to ensure the safety and comfort. All bedrooms observed to have all required components. Storage space is provided Continued on LIC809-C for residents in their bedroom. Smoke/carbon monoxide detectors were tested and found to be operational. LPA toured the outside of the facility and observed outdoor passageways are free of obstructions. LPA observed there are shaded seating areas for residents’ enjoyment in the front patio of the facility and throughout the corridor of the yard. LPA observed a swimming pool adjacent to the house with a fence around it. LPA observed the pool has a main entry gate; the entry door opens away from the pool. The fence has a padlock at the gate door for inaccessibility. LPA measured the pool fence which measured at least 5ft from base of the floor to the top of the fence and it was observed to enclose the entire pool area. LPA observed a fire extinguisher with service date July 14, 2025, in the kitchen. Fire drills are conducted every three months and LPA reviewed logs with last drill conducted on November 7, 2025. LPA began review of records. LPA reviewed three resident records. All the required documentation was present and current in the residents’ files reviewed. LPA reviewed one employee records. All employees present have a criminal record clearance and are associated to the facility. LPA observed records reviewed have a current First Aid certificate. Based on the observations made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Nancy Guillen made an unannounced visit for the purpose of conducting a required annual Inspection. LPA was greeted and granted entry by Licensee Alphonso Pulido after explaining the purpose of the visit. This is a Residential Care Facility for the Elderly (RCFE) licensed to six non-ambulatory residents with a hospice waiver for one. The facility is a two-story home. The second floor of the facility is used for staff only; the first floor consists of two resident bedrooms, a staff bedroom, a half bath, and a shared bathroom, living room, dinning room, office, kitchen and attached garage. During the inspection, LPA and Licensee conducted a tour of the inside and outside of the facility, common areas, resident rooms, kitchen, dining room, garage and observed the following: LPA observed residents resting in their respective bedrooms. LPA observed two residents present at the facility and one resident was away at their day program. During todays visit there were two staff present. LPA observed the See Something Say Something Poster (PUB 475) mounted on the wall by the entry way. All resident bedrooms had the required furnishings. LPA observed all resident beds had linens and blankets with additional linens stored in a garage closet. LPA observed bathrooms were clean and equipped with grab bars and non-skid floor mats. LPA observed all windows were appropriately screened. Bathrooms were observed to be free of debris and mildew, faucets and toilets were operational. Water temperature tested between 108.6 and 110.9 degrees Fahrenheit. LPA toured the outside of the facility and observed outdoor passageways were free of obstruction. LPA observed the backyard had a shaded sitting area with furniture for resident use. LPA observed a swimming pool with a fence around it. LPA observed the pool gate has a self-latching entry door. The fence has a key lock at the gate door for inaccessibility. LPA observed the facility had a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Continued on LIC 809C Smoke detectors and carbon monoxide detectors tested operational. Fire extinguisher was observed to be fully charged with a service date of May 14, 2024 and located by the kitchen. Gas stove, microwave, washer, and dryer were all inspected and observed to be operable. The garage is used for storage and is kept locked and inaccessible to residents. Toxic chemicals, cleaning solutions, and disinfectants were observed to be locked and inaccessible to residents in the garage and under the kitchen sink. Medication cabinet was observed to be locked and centrally stored the kitchen. LPA observed the First Aid Kit had all the required components. LPA observed the facility conducted their last emergency disaster drill on October 29, 2024 and is conducted every 3 months. LPA began review of the records. LPA Guillen reviewed three resident records. All the required documentation were present and current in the residents’ files reviewed. LPA reviewed three employee records. All employee’s present have a criminal record clearance and were associated to the facility. LPA observed staff to have current First Aid certificates. Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit for the purpose of conducting a Required/Annual Inspection. LPA was greeted and granted entry by Licensee Alphonso Pulido and explained the purpose of the inspection. During the inspection LPA and Licensee conducted a tour of the inside and outside of the facility, common areas, client rooms, kitchen, garage and observed the following: This is a two-story home with three client bedrooms and two bathrooms on the first floor. The second floor is used solely as staff quarters. All client bedrooms had the required furnishings. LPA observed all client beds had linens and blankets. The back yard has a shaded sitting area. LPA observed two clients present, an additional client was away at day program. Bathrooms were observed to be free of debris and mildew, faucets and toilets were operational. Water temperature tested at 119.8 F degrees. LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted at the entrance of the facility. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Smoke detectors and carbon monoxide detectors tested operational. Fire extinguisher was observed to be fully charged. Medication cabinet lock was observed to be inoperable, and medication is accessible; a Deficiency was cited on today’s date. Record review of disaster drill log indicated last disaster drill was conducted in December 2022. Per Licensee, drills are conducted quarterly, however, not documented; a Technical Advisory was issued on today’s date. LPA reviewed three client files and three staff files. LPA interviewed clients and staff present. Based on the observations made during today’s inspection, one deficiency is being cited per Title 22 Division 6 of the California Code of Regulations.
