Allegation investigated: -Facility is in disrepair -Staff did not provide a safe and comfortable environment for residents
This unannounced visit conducted by Licensing Program Analyst (LPA) Ruth Martinez is being conducted to initiate the 10-day visit to investigate the above mentioned complaint allegations. LPA arrived at the facility and was greeted and granted entry by staff. LPA spoke to Mark Cruz, Administrator, and explained the nature of the visit. During the course of the investigation, findings are based upon interviews conducted, a tour of the physical plant of the facility conducted and copy of pertinent documents obtained. It is alleged that the facility is in disrepair. Based on interviews with 2 of 2 staff indicated that on February 18, 2022, it was reported to landlord that there was a leak in the garage ceiling.
