Allegation investigated: -Facility did not meet the residents needs -Facility is not abiding by the admission agreement -Facility did not ensure that resident falls were reported to the Department.

Licensing Program Analyst (LPA) Ruth Martinez visited the facility to deliver findings for the investigation into the above identified complaint allegations. LPA arrive at facility was greeted at the door and granted entry. LPA spoke with Shahin Mokhtarzad, Administrator and explained the purpose of the visit. Findings are based upon this investigation which included facility file review, interviews conducted with staff, and residents. It is alleged the facility did not meet residents’ needs. Based on file review resident (R1) expired at the facility on October 28, 2024, and LPA was unable to interview. Record review for R1 revealed that LIC602 physicians report page 4 number 14i. R1 was able to communicate needs. Allegation investigated: Staff are not available to assist residents at night

Licensing Program Analyst (LPA) Jenifer Tirre conducted an unannounced inspection visit to deliver findings for complaint investigation into the above allegations. LPA explained the reason for the visit with Administrator Shahin Mokhtarzad During the course of the investigation LPA toured facility, reviewed records, conducted interviews, made visual observations and requested pertinent documentation such as Resident Roster, Admission Agreements, staff trainings, Functional Capability Assessment and Physician’s reports. During investigation LPA reviewed facility records such as Personnel records, training, and resident records. Records reviewed revealed that Personnel files of staff had training in importance & techniques of personal care, Dementia training in RCFE, Resident Rights, and Direct care staff orientation. Licensing Program Analyst (LPA) Celine De Perio conducted an unannounced visit to the facility to deliver the findings for the complaint received on 04/27/23. LPA De Perio explained the purpose of today's visit, was greeted, and granted entry by facility administrator (AD) Shahin Mokhtarzad about visit. It was alleged that staff do not assist resident with toileting in a timely manner. LPA De Perio conducted a total of 8 interviews which consisted of staff and residents. 5 out of the interviews conducted did not corroborate with the allegation, of which one of the interviews stated "they do help us" and further explained that staff will assist with toileting in the morning, afternoon, in the evening, and whenever the resident calls for staff. 3 out of the 8 interviews were unable to be conducted due to declining of wanting to be interviewed. LPA De Perio conducted a tour of the facility on 05/02/2023, and observed staff on duty assisting a resident with toileting in the restroom.
