Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator Rebekah Buboi and explained the reason for the visit. The Administrator's certificate expires on September 26, 2027. Facility is licensed for 6 non-ambulatory residents and has a hospice waiver for 3. Facility is a single story home with 7 bedrooms, 1 is for staff, 2 full bathrooms, 3 half bathrooms, kitchen, dining room, living room with a screened fireplace and a family room with a TV. Staff reported the fireplace is never used. LPA and the Administrator toured the facility. LPA observed the kitchen is clean and organized. LPA observed the fire extinguisher in the kitchen is fully charged. LPA observed sharp objects and knives are kept locked in a kitchen drawer. LPA observed a 7 day non-perishable food supply in the kitchen. LPA observed the refrigerator in the kitchen is non-operational. The Administrator reported the refrigerator is going to be repaired later in the week. Staff reported they use the refrigerators in the garage. LPA observed a 2 day perishable food supply in the 2 refrigerators in the garage. There is also a freezer in the garage with food. The garage is used for storage. LPA observed cleaning supplies and furniture stored in the garage. The laundry room door leading to the garage is kept locked and is inaccessible to residents. Smoke detectors/Carbon Monoxide detectors tested operational. The last emergency disaster drill was on June 6, 2026. LPA observed all resident rooms had the required furnishings. Extra linens are stored in the hallway cabinets. LPA observed all bathrooms are clean and operational. Hot water measured 119.0 degrees Fahrenheit. LPA inspected the first aid kit. The first aid kit did not have a current edition first aid manual. LPA reviewed 3 resident records. LPA observed R1 did not have a current appraisal. LPA reviewed 3 resident medications, no discrepancies observed. LPA reviewed 2 staff files, no discrepancies observed, both staff members are background cleared and associated to the facility and have the required training. LPA observed the facility did not have a dedicated internet device for resident use. LPA consulted with Administrator regarding reporting requirements. Deficiencies are being cited per Title 22, Division 6 of the California Code of Regulations. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Ruth Martinez is conducting this unannounced visit for the purpose of completing an annual required inspection. LPA arrived at the facility and was greeted and granted entry by staff, and explained the nature of the visit. Rebekah Buboi, Administrator arrived at the facility and met with LPA. There are six (6) residents at the facility and there is two (2) residents receiving hospice services currently. LPA began the tour of the inside and outside of the facility. LPA observed required department postings throughout the facility. Facility stays within the capacity limitations. There is a minimum of one week of non-perishables foods and two days of perishables foods available. There is additional food storage in a spare refrigerator and freezer located in the garage. The facility is maintained at a comfortable temperature. LPA inspected that medication is centrally stored in a safe locked storage cabinet located in the dining room. LPA reviewed medication and observed medication was labeled and stored inaccessible to residents in care. LPA inspected the facility bathrooms and LPA measured the hot water temperature which measured 118.7 Fahrenheit degrees. All bathrooms observed to have a supply of soap, toilet paper and towels. Bathrooms are equipped with required safety measures such as non-skid mats and grab bars. Lighting is sufficient to ensure safety and comfort. The facility is equipped with sufficient hand hygiene, cleaning, and disinfecting supplies. LPA observed that toxic chemicals, cleaning solutions and disinfectants are locked in storage cabinet in the garage. The facility has an available clean supply of linens. LPA inspected residents’ bedrooms which had sufficient lighting to ensure safety and comfort. All bedrooms are observed to have all the required components. Storage space is provided for residents in their bedroom. Smoke detectors/carbon monoxide detectors were tested and found to be operational. LPA toured the outside of the facility and observed outdoor passageways are free of obstructions. LPA observed there are shaded seating areas for residents’ enjoyment in the backyard of the facility. LPA observed a fire extinguisher with service date of August 15, 2024, charged and mounted in the kitchen, LPA reminded Administrator to service fire extinguisher by August 15, 2025, to stay in compliance. Fire drills are conducted every three months, LPA verified fire drill logs are kept. LPA began reviewing records. LPA reviewed three resident records. All the required documentation was present and current in the residents’ files reviewed. LPA reviewed two employee records. All employees present have a criminal record clearance and are associated with the facility. LPA observed records reviewed have a current First Aid certificate. LPA as a reminder provided annual fee dues information. Based on the observations made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations. LPA arrived at the facility and was greeted and granted entry by caregiver and LPA explained the nature of the visit. Rebekah Buboi, Administrator arrived shortly after and met with LPAs. There are five residents at the facility and there is one resident receiving hospice services currently. LPAs began the tour of the inside and outside of the facility. LPA observed required department postings throughout the facility. Facility stays within the capacity limitations. There is a minimum of one week of non-perishables foods and two days of perishables foods available. There is additional food storage in storage a refrigerator and a freezer located in the garage. LPA Martinez observe a fire extinguisher mounted on the wall of the kitchen with service date of 4/26/23. The facility is maintained at a comfortable temperature. LPA inspected that medication is centrally stored in a safe locked storage cabinet located in dining room. LPA reviewed medication and observed medication was labeled and stored inaccessible to residents in care. LPA inspected the bathroom and LPA measured the hot water temperature which measured 113.1 Fahrenheit degrees. All bathrooms observed to have a supply of soap, toilet paper and towels. Bathrooms are equipped with required safety measures such as non-skid mats and grab bars. Lighting is sufficient to ensure safety and comfort. The facility is equipped with sufficient hand hygiene, cleaning, and disinfecting supplies. LPA observed that toxic chemicals, cleaning solutions and disinfectants are stored and locked in a storage cabinet located in the garage. The facility has an available clean supply of linens. LPA inspected residents’ bedrooms which has sufficient lighting to ensure the safety and comfort. All bedrooms observed to have all required components. Storage space is provided for residents in their Continued on LIC809-D bedroom. Smoke detectors were tested and found to be operational. LPA toured the outside of the facility and observed outdoor passageways are free of obstructions. LPA observed there are shaded seating areas for residents’ enjoyment. LPA began review of records. LPA reviewed three resident records. All the required documentation was present and current in the residents’ files reviewed. LPA reviewed two employee records. All employees present have a criminal record clearance and are associated to the facility. LPA observed records reviewed have a current First Aid certificate. Based on this inspection, one deficiency was observed at this time in the areas evaluated per Title 22 Division 6 of the California Code of Regulations. See LIC 809-D for deficiencies. This report was reviewed with facility representative and a copy of this LIC809, LIC809-D report was provided and left at facility.
