Licensing Program Analyst (LPA) Jessica Cho arrived at the facility unannounced for the purpose of conducting the Required 1-Year annual evaluation using the CARE Inspection Tool. LPA was greeted and granted entry by Caregiver Marjorie De Los Reyes after stating the reason for the visit. Administrator (Admin) Estela Kramer arrived on premise approximately 10am and remained at the facility to assist with the inspection. Admin has a valid administrator's certificate expiring on September 12, 2026. The following was observed during the inspection: This is a single story property located in a residential neighborhood comprised of five resident bedrooms, one staff bedroom, and two resident bathrooms. Facility operates within the conditions and limitations specified on the license. LPA observed five residents in care of which none are receiving hospice services. LPA observed two caregivers on duty and verified fingerprint clearance and association statuses including the administrator. LPA toured the interior portion of the facility. All common areas were inspected which includes the attached two car garage which is utilized as storage. The fireplace is appropriately screened in the living room. LPA inspected all the resident bedrooms. The resident bedrooms' were appropriately furnished, beds and bedding supplies were in good condition, adequate lighting was provided, and sufficient storage space for each residents' personal belongings were observed. All bathrooms were found be in compliance, clean, and operational. Slip resistant mats were available. The hot water temperature in the bathrooms measured at 111.9 and 111.0 degrees Fahrenheit. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed ample two-day supply of perishables and seven-day supply of non-perishable food. LPA toured the exterior portion of the facility. The outdoor passageway is free of obstruction. The two exit gates on each side of the property are operational, and there were sufficient seating and shading in the patio area. The three fire extinguishers were charged and serviced on April 15, 2025. The auditory devices and dual functioning smoke/carbon monoxide detectors were tested and operational. Facility maintains emergency food and water in the dining area. The first aid manual and first aid kit had all necessary elements. LPA reviewed five out of five resident files and two personnel files in which no discrepancies were found. Medications were audited for three out of five residents and no discrepancies were found. Based on the observations made during today's visit, no deficiencies are being cited. Licensing Program Analyst (LPA) Jessica Cho arrived at the facility unannounced for the purpose of conducting the Required 1-Year annual evaluation using the CARE Inspection Tool. LPA was greeted and granted entry by Caregiver Robinie Martin and explained the reason for the visit. Administrator Estela Kramer later joined to assist LPA with the visit. The facility is a single story structure located in a residential neighborhood. Facility is licensed to operate for four non-ambulatory, two bedridden, and maintains a hospice waiver for six residents. There are six residents in care with one receiving hospice care. Two caregivers were present and on duty. LPA observed the facility to be clean, sanitary, and operational. There are five resident bedrooms and two resident bathrooms. There is an additional bedroom occupied by two live-in staff next to the kitchen. All common areas were inspected including the laundry area within the attached two car garage. The residents' bedrooms were appropriately furnished. Beds and bedding supplies were in good condition, adequate lighting was provided, sufficient storage space for each residents' personal belongings were observed. Bathrooms were found to be in compliance, clean, and operational. The hot water temperature measured at 116.2 and 116.4 degrees Fahrenheit. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed sufficient two-day supply of perishables and seven-day supply of non-perishable food available. The fireplace was screened. LPA toured the exterior portion of the facility. The outdoor passageway is free of obstruction. The exit gates were self-closing and self-latching. LPA observed sufficient seating and shading. The outdoor shed was secured. Two fire extinguishers were observed, charged, and serviced on April 23, 2024. The auditory devices and dual functioning smoke/carbon monoxide detectors were tested and operational. The auditory device was not tested in one resident's room per their request. LPA observed the emergency food/water in the pantry room and garage. Emergency evacuation drills are being conducted quarterly. The first aid kit contains all necessary elements. The first aid manual is available however is outdated. The facility land line number, (949) 380-7379, was tested and remains available. The liability insurance is also current. LPA observed the required 'See Something, Say Something' poster (PUB475) in the correct size. The Administrator's Certificate for Estela Kramer expires on September 12, 2026 and October 27, 2025 for Paul Kramer. LPA conducted an audit of six residents' files and two personnel files. No discrepancies noted. Medications were audited. No discrepancies noted. Interviews were attempted and conducted for residents/staff. The following items were noted and discussed with Admin Estela Kramer: to review and amend the Emergency Disaster Plan (LIC610E), to obtain the current edition of the first aid manual, to include the subject(s) covered on the evacuation drill logs, and to organize/prepare your emergency supplies/equipment(s). Based on the observations made during today's visit, no deficiencies are being cited. Advisory Notes (LIC9102s) are being issued.
