On April 24, 2026, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of a required annual inspection. LPA explained the purpose for the visit and was greeted and granted entry by staff on duty. During the visit, staff on duty contacted the facility administrator (AD) Dennis De La Cruz about the visit. For this visit, there are two staff members on duty, both of which are background cleared and associated. AD was unable to assist with the inspection. The PUB475 ‘See Something, Say Something’ poster was observed to be located near the front door. LPA observed that Dennis De La Cruz has a valid Administrator certificate expiring on January 18, 2028. The facility is a Residential Care facility for the Elderly (RCFE) licensed for six residents, six of which may be non-ambulatory, none of which may be bedridden, and a hospice waiver for four. LPA toured the interior and exterior portions of the facility with staff. For this visit, there are a total of five non-ambulatory residents in care, two of which are on hospice, and none are bedridden. The facility is a two story home. The upper level is only for staff. There are a total of five bedrooms, three of which are shared resident bedrooms and two are staff bedrooms. LPA toured each bedroom with staff and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and free of any hazards. LPA observed the staff rooms are kept locked and inaccessible to residents in care. Smoke and carbon monoxide detectors as well as auditory exit alarms were tested and operational. There are a total of three bathrooms, two are for residents and one is for staff. Continued on LIC 809-C. Bathrooms were observed to be in good repair, toilets and faucets were operational and showers were equipped with grab bars and non-skid floor mats. Water temperature in the bathrooms were measured to be 111.7 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked in a kitchen cabinet and inaccessible to residents in care. Fire extinguisher was charged, mounted and located in the kitchen. Fire extinguisher was dated 2026. LPA observed all the required postings in the hallway. LPA observed the facility conducted their last emergency disaster drill on January 2, 2026. Facility had back-up emergency food and water supply, located in the garage. LPA observed that the First Aid kit had all the required components. Medications were observed to be locked in a medication cabinet in the kitchen, inaccessible to residents in care. Chemicals were observed to be locked in the garage and underneath the kitchen sink. LPA observed the door leading to the attached two car garage is kept locked and inaccessible to residents in care. The garage is used for storage and laundry. For the exterior portion, LPA observed patio furniture under shading, and the grounds were free of any hazards or obstructions. There are two exit gates in the backyard that can be opened in case of an emergency. A pool was observed in the backyard. However, there is a fence surrounding it and a padlock on the gate. The pool has been drained. During this visit, five resident files and three staff files were reviewed. All staff are background cleared and associated with the facility. LPA reviewed residents’ medication and medication records and one resident interview was conducted. The facility has valid liability insurance that expires in May 1, 2026. Based on today's observations, there are no deficiencies being cited per Title 22 of the California Code of Regulations. An exit interview was conducted with the house manager. On 4/28/25 at 1 PM, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by Adminstrator (AD) Dennis DeLa Cruz after explaining the purpose for today's visit. LPA observed that Administrator Dennis DeLa Cruz has a valid Administrator certificate which expires January 18, 2026. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents on the first floor, and has a hospice waiver for four. The facility is a two story home with three shared resident bedrooms on the first floor, two shared resident bathrooms on the first floor, a living room, a dining room, a kitchen, two staff bedrooms upstairs, one staff bathroom upstairs, and an attached two car garage. LPA, accompanied by the AD conducted a tour of the interior portion of the facility. On today's visit, LPA observed five residents in care, one of which is on hospice, and three staff present. LPA observed residents relaxing in their respective bedrooms and in common areas. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall by the entryway of the facility. LPA inspected the three shared resident bedrooms and they were observed to be free of hazards. LPA observed resident bedrooms had the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA observed additional linens to be stored in a hallway closet. LPA inspected the two residents bathrooms and they were observed to be clear of hazards. Resident bathrooms were equipped with grab bars and nonskid floor mats. Faucets and toilets were operational. Hot water temperature measured between 113.5 and 114 degrees Fahrenheit. LPA observed the second story of the facility is for staff use only and is off limits to residents in care. CONTINUED ON 809-C LPA observed the kitchen has a two day perishable and a seven day nonperishable food supply on hand. LPA observed kitchen appliances to be clean and operational. LPA observed the four burner gas stove lights unassisted. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. LPA observed chemicals and toxins to be stored in a locked kitchen cabinet under the sink. LPA observed a fire extinguisher to be mounted on the wall in the kitchen. The fire extinguisher was observed to be charged and up to date on service. LPA tested the wired smoke detectors/carbon monoxide detectors which tested operational. LPA observed the facility does not keep a log of the emergency disaster drills it conducts. LPA observed the centrally stored medication to be kept in a locked cabinet in the kitchen. LPA observed a First Aid Kit to be stored in a locked kitchen cabinet and it had all the required components. LPA observed a fireplace in the living room and it was observed to be adequately fenced and not in operation at time of visit. LPA observed the door leading to the attached two car garage to be kept locked and inaccessible to residents in care. LPA observed the garage to be used for storage and laundry. LPA observed chemicals and toxins to be stored in the garage. LPA observed the facility has a three day emergency food and water supply stored in the garage. LPA, accompanied by the AD, conducted a tour of the exterior portion of the facility. LPA observed the exterior portion to be free of obstructions and hazards. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gates on the north side and west side of the facility to be self latching and can be opened in an evacuation. LPA observed a pool in the backyard. LPA observed the pool to be adequately fenced and kept locked for resident safety. LPA reviewed all five resident files. LPA observed the facility did not have a Pre-Admission Agreement on file for Resident #1, Resident #2, Resident #3, Resident #4, and Resident #5. LPA observed the facility did not have a Reappraisal on file for Resident #3 and Resident #4. LPA observed the Reappraisals for Resident #1, Resident #2, and Resident #5 to be outdated. LPA reviewed residents' medication and medication records. LPA reviewed five staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, deficiencies are being cited on the attached 809-Ds. An exit interview was conducted with Administrator Dennis DeLa Cruz. Licensing Program Analysts (LPAs) Celine De Perio and Faith La conducted an unannounced required annual inspection. LPA De Perio explained reason for visit and was greeted and granted entry by staff on duty and facility administrator (AD) Denis Dela Cruz. The PUB475 "See Something, Say Something" poster was observed to be at the entrance of the facility. LPAs observed the Administrator's Certificate for Denis Dela Cruz, which expired on 1/18/24, however provided proof to LPAs that a payment was made to obtain the renewed certificate and that certificate is pending. LPAs toured the interior and exterior portions of the facility with AD Dela Cruz. The facility is a two level structure and is licensed for 6 non-ambulatory residents, of which 0 may be bedridden and 4 may be on hospice. For this visit, there are a total of 4 residents in care, of which none are on hospice. On the first floor, there are a total of 3 shared resident rooms. On the second floor, there are 2 bedrooms, which are only designated for staff. LPAs verified that there are no residents residing on the second floor. LPAs toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detector and auditory exit alarms were tested and operational. There are a total of 3 restrooms of which 2 are for residents on the first floor, and 1 is for staff on the second floor. Restrooms were observed to be in good repair, toilets were operational, and grab bars and non-skid floor mats were provided. Water temperature in restrooms were measured to be at 107.0 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked and inaccessible to residents in care, and located in a kitchen drawer. Fire extinguisher was charged, mounted and located in the kitchen. LPAs observed the emergency disaster and evacuation plan, which is posted at the entrance of the facility. Facility had back-up emergency food and water supply, located in the kitchen and in the garage. LPA De Perio observed that First Aid Kit had all the required components. Medications and toxins were observed to be locked and inaccessible to residents in care For the exterior portion, LPA De Perio observed patio furniture under shading, and the grounds were free of any hazards. There are 2 gates in the backyard, which were self-closing and self-latching. LPAs observed an empty pool in the backyard, that is locked by a gate, and is made inaccessible to residents in care. For today's visit no deficiencies were issued per Title 22 Division 6 of the California Code of Regulations. No citations were issued. An exit interview was conducted with AD Dela Cruz.
