Allegation investigated: Staff are not meeting resident's dietary needs.

On 08/20/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to commence a complaint investigation in regards to the allegation listed above. The LPA met with Brigitta Lofvendahl, Executive Director and explained the elements of the allegation. The investigation consisted of interviews and records review. On 08/11/25 Community Care Licensing received a complaint alleging staff are not meeting resident's dietary needs. It was alleged that Resident #1 (R1)s dietary needs are not being met. During today's visit the LPA conducted a records review of the facility census, which revealed R1 does not reside at the facility. The census revealed that R1 is a residing in another building/facility. Per an interview with Brigitta Lofvendahl, Executive Director, confirmed that R1 is in fact residing in another building. Therefore the allegation is unfounded. A finding that the complaint is unfounded means that the allegation is false, could not have happened, and/or is without a reasonable basis. Allegation investigated: Staff served expired food to a resident in care.

Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to commence a complaint investigation for the allegation listed above. LPA met with Brigitta Lofvendahl, Administrator and explained the purpose of the visit and the elements of the allegation. The allegation was investigated, and the investigation consisted of observations interviews and records review. On 01/27/2025 Community Care Licensing received a complaint alleging that staff served expired food to a resident in care. Regarding the allegation of staff served expired food to a resident in care. It was alleged that the staff had served expired yogurt to resident's in care. LPA observed for the lunch that was served (BBQ chicken, salad and rice) to be fresh (vibrant in color, and fresh aroma). Per an interview with Executive Director Brigitta Lofvendahl the facility is apart of the food program and receives food donations on a daily basis. In addition there is a weekly food order that the facility places from a commercial food supply company. Per Executive Director, the lead caregiver is responsible for checking the food each shift. Unsubstantiated LPA conducted interviews with multiple residents who could not corroborate what was alleged, as they could not recall being served expired food, hearing anyone saying that the food was expired or the food tasting or smelling like it was spoiled. Residents shared that there are not any concerns with the food, as they are served 3 meals a day as well as 2 snacks and are given more when they asked. Per interviews with staff food deliveries are received daily, and checked at the initial time of receipt. Staff denied receiving reports from residents and having being served expired food, in addition if there is expired food is noticed it is discarded. During today's visit LPA inspected the facility's food supply and observed for the food supply to be sufficient LPA observed for there to be two (2) containers of yogurt with a date stamp of 1/15/25 and 1/19/25. There were several jugs of milk and loaves of bread that had a sell by date that extended back to 1/21/25. According to the USDA Food and Safety and Inspection Service there are commonly used phrases such as A "Best if used by/Before" date indicates when a product will be best flavor or quality. It is not a purchase or safety date. A"Sell-by" date date tells the store how long to display the product for sale inventory management. It is not a safety date. A "Use-By" date is the last date recommended for the use of the product while at peak quality. It is not a safety date except when used on infant formula... Based on observation and interviews the allegation of staff served resident expired food is unsubstantiated. A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation(s) occurred. Type of visit: Required - 1 Year

On today's date 07/29/24 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility for the purpose of conducting a 1 year required visit/annual inspection. LPA met with Executive Director Brigitta Lofvendahl and explained the purpose of the visit. The facility is licensed to serve residents age 60 and over, 15 non ambulatory of which 15 may be bedridden. The facility has an approved hospice waiver for six (6). There is (1) resident receiving hospice services. The facility is a single story structure with 8 combined bedrooms and bathrooms, dining area, front and back patio, kitchen, living room, medication closet, and garage which is used to store the laundry appliances, generator and extra food. The facility was observed to be clean and clutter and odor free. The call lights were tested and observed to be operable. LPA observed all toxic chemicals and other hazards/sharps to be inaccessible to residents. The medications are locked inside the mediation closet, that is accessed via a key pad. The facility food supply met the requirements of a 2 day supply of perishable and a 7 day supply of non perishable food items. There are no pools or bodies of water on the premises, are known guns or ammunition. LPA tested the pull cord/signal system and observed for bedroom#5 pull cord to be inoperable, the cord was replaced during LPAs visit, therefore no citation was issued. LPA conducted a records review and observed the following: Staff present were observed to have obtained criminal record clearance and to be associated to the facility. Staff present were observed to have current CPR/First Aid training, and possess a valid Administrator certificate which expires on 04/16/2025. Resident files were observed to have the required documentation such as physician's report, appraisal, and admissions agreement. The hot water temperature was tested and found to be within regulatory limits. The facility was observed to have (3) fully charged fire extinguishers fully charged. The emergency drills are conducted quarterly, the last drill was conducted on 4/16/24. LPA conducted a file review prior to inspection and observed for there to be outstanding annual fees, during today's visit, proof was provided confirming that the fees have been paid. Based on today's inspection no deficiencies were issued.
