Licensing Program Analyst (LPA) Robert Campbell conducted an unannounced annual required visit. Upon entry, LPA was greeted by Jelou Rosal, Caregiver, and informed them of the purpose of the visit. At the time of the visit, there was two (2) staff member and 4 (4) residents present. Heddy Oyson/Administrator, arrived later during the visit. Facility Overview: The facility is a one-story home with three (3) bedrooms and three (3) bathrooms, including an attached garage. There are no pools or known firearms on the premises. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility has infection control plan in file. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The outdoor area was free of hazards. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked and inaccessible to residents. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was 115°F. Fire extinguisher located at kitchen had current inspection tag (8/27/2025). Food Service: The facility’s kitchen was clean and equipped to prepare food. The facility maintained the required two-day supply of perishable foods and a seven-day supply of non-perishable foods. Continued on LIC809-C.... Care & Supervision/Administration: Adequate staff were present to supervise clients during the visit. The administrator holds a current administrator’s certificate. Record Review and Resident/Staff Files: LPA reviewed files for six (6) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. Four (4) resident files were reviewed and contained all the required documents. Health-Related Services/Incidental Medical Services: All resident medications were securely locked in hallway closet. LPA reviewed medications for four (4) residents, confirming that all medications were listed on the Medication Administration Record (MAR) and accounted for. Personal & Incidental Funds: All monies are not handled by the facility. Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, including documentation of the last emergency drill conducted on 1-05-2025, which met department requirements. All facility exits were clear of obstructions. No deficiencies were cited during the visit. Type of visit: Case Management - Deficiencies

On 08/18/2025, Licensing Program Analyst (LPA) Valerie Flores conducted a case management visit to issue a deficiency observed during the initial visit for complaint control number 18-AS-20250815131027. During this visit, LPA met with Administrator, Alyssa Legaspi. LPA conducted a record review for (4) four out of (4) four residents. Upon record review, (2) two out of (4) four residents maintained an ambulatory status of bedridden. Per the facility's fire clearance conducted on 11/2/23, the facility is permitted to retain a total of (6) six residents to which (4) four may be non-ambulatory and (0) zero bedridden residents. Administrator did not properly notify the fire department with the required 48 hours nor submit an application for an updated fire clearance. The facility will be cited in accordance with Title 22 Section 87202. This requirement was not met with evidence by: Based on interviews and record review, (2) two out of (4) four residents maintained a bedridden ambulatory status to which the facility can maintain (0) zero bedridden residents per the fire clearance. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Kathleen Banrasavong arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by facility staff and granted entry. LPA began inspection with introduction and visit purpose. Upon arrival LPA learned that five (5) residents live at this facility. However, there was three (3) residents present at the facility during the inspection. There was two (2) staff members present. The Licensee, Jason Legaspi was advised of the annual and came to conduct and completed the facility tour. Client Records/Incident Reports/Clients Rights Information: LPA reviewed client records. Five (5) records were reviewed. LPA reviewed for identification and emergency information, admission agreement, medical assessment, and TB test results, needs and service plans, placement, functional assessment, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. Personnel Records/Training/ Staffing/ Administration: LPA reviewed employee records. Two (2) records were reviewed. LPA reviewed employee records for first aid certification, criminal record clearance or an exemption, health screening and TB test results, employee rights, training verification, and current administrative organization. Gerramy Legaspi, Administrator’s certificate expiration date was 02/13/2025. Alyssa Grace Legaspi, Administrator's certificate expiration date is 10/30/2025. Food Service: Food prep areas are clean and organized. Food supply meets the requirement of one (1) week supply of nonperishable and two (2) day supply of perishables. Emergency food and water supply is present. There is a location for sharps in the kitchen. Physical Plant and Safety of Environment/Operational Requirements: LPA toured the facility inside and outside. LPA observed the facility to be clean and in good repair. The facility is maintained at 75 degrees for the client’s comfort. Lighting is sufficient for safety. Water temperature measured 108.0 degrees F. Laundry is done in the garage. There is a locked location for storing laundry soap, cleaning supplies and chemicals in the closet in the laundry room, cabinets. All outdoor and indoor passageways are free of obstruction. Emergency lighting is available. There is a telephone working at this location. LPA dialed the facility’s landline number, which rang and was operable. The LIC 610, emergency disaster plan is maintained. There are no firearms at this facility. There is one (1) secured fireplace at this facility. There are zero (0) pools at the facility. There are two (2) secured gates that has a self-latching lock located on the northeast and northwest sides of the house. LPA observed emergency supplies and two (2) first aid kits. The last emergency fire drill was conducted on 01/25/2025. Infection Control: The LPA observed the hand washing stations in the facility restrooms and kitchen had hand hygiene supplies and hand washing signs. LPA observed PPE equipment and cleaning supplies to do regular cleaning of the facility. LPA reviewed the facility's infection control plan which met department requirements. LPA reviewed staff records and found that staff had infection control training. Medications/Health Related Services/Incidental Medical Services: The medications are centrally stored. There is a locked cabinet allocated for medication storage. Centrally stored medication and destruction logs are maintained separately. LPA reviewed medication logs and observed that they were dispensed accurately. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. LPA observed five (5) dual smoke detectors and carbon monoxide detectors throughout the facility. There were one (1) fire extinguishers on site, date charged was 12/30/2024. Pursuant to Title 22 of The California Code of Regulations Division 6, there are zero (0) deficiencies observed. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Janira Arreola, made an announced visit to the facility in order to conduct a prelicening inspection. LPA met with applicants Jason, Alyssa and Gerramy Legaspi. The facility is seeking an initial license. The facility will be licensed as a residential care facility for the elderly, with capacity of (6). Approved for clearance for (4) non ambulatory and hospice waiver for (2) residents. The facility does not have a pool or firearms. The home is a one story home with (3) bedrooms and (3) bathrooms. LPA conducted a walk through of the interior and exterior of the facility. The bedrooms have all the required furniture, and required hygiene supplies, and linens. LPA observed the hallway lights and the carbon monoxide detectors were in good working condition. The outdoor area was free of any hazards and had a shaded area for residents and an emergency exit. The kitchen had the ability to prepared food is a clean and safe environment and possessed the required food items . LPA observed areas were the staff and resident files would be kept as well as locked areas designated for medication, sharp objects, and cleaning supplies. The hot water temp was measures at 120F. The facility has a land line at (760) 975-3119. There are no objections for the applicant to proceed in the prelicensing process.
