On 03/19/2026, Licensing Program Analyst (LPA) Jacqueline Shaw-Ross arrived at the facility to conduct the required annual visit. LPA was greeted at the door by Salvacion Bagtas, Caregiver, and the purpose of the visit was explained. Present at the facility were one staff member, one hospice worker, and three residents. The home is approved for six elderly residents ages 60 and above. All of whom may be non-ambulatory, three are approved for bedridden. Hospice waiver is approved for four residents. The facility is a single story structure with five (5) bedrooms and three and a half (3.5) bathrooms, with one bedroom designated for a live in caregiver. LPA conducted a tour of the interior and exterior of the facility. The home was observed to be clean, free of clutter, and free of odors. The food supply was observed to be sufficient. Hot water temperature tested in two (2) resident bathrooms measured 113 degrees Fahrenheit, which is within regulatory limits. Medications, sharps, and cleaning chemicals were locked and inaccessible to residents in care. Smoke detectors and carbon monoxide detectors were tested and found to be operational. Quarterly emergency disaster drills are being conducted as required; the most recent drill occurred on 01/13/2026. No pools, bodies of water, or firearms/ammunition were observed on the premises. Staff present were observed to have criminal record clearance, facility association, and valid CPR certification. Annual licensing fees are up to date. Emergency drills are conducted quarterly, with the last drill conducted on 01/13/2026. The Emergency Disaster Plan was reviewed and is updated. During the visit LPA was informed by Caregiver that they did not have access to client records and the owner was away on medical appointment. LPA spoke with the Licensee by telephone and informed them of the importance of having all records readily available for review. Citation issued. During the employee file review, LPA observed that all three (3) staff files contained current CPR certification, background clearance, medical clearance, and current/ongoing training. On 03/06/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit. LPA was greeted and granted entry by Caregiver Amelia Perlow, where explained the purpose of the visit. The administrator Carlo Munar was unable to come to the facility due to being out of town. The facility has an approved hospice waiver for (3), with (2) residents currently receiving services. LPA conducted a tour of the interior and exterior areas of the facility. The facility was observed to be clean, clutter and odor free. The medications, sharps, and chemicals were observed to be locked and inaccessible to residents in care. The smoke and carbon monoxide detectors were tested and observed to be operable. The emergency disaster drills are being conducted on a quarterly basis, the last drill was conducted on 12/11/24, with a drill being due this month (March 2025). There are no pools, bodies of water or known guns and ammunition on the premises. The facility is a single story structure consisting of (5) bedrooms and (3.5) bathrooms. There is one room designated for a live in caregiver. The food supply was observed to be sufficient. The water temperature was tested and measured to be 105 degrees Fahrenheit in the (2) resident bathrooms. All staff present were observed to have obtained criminal record clearance and were associated tot he facility with valid CPR certification. The administrator on record needs to be changed and was observed to have submitted their renewal application. The facility annual fees are due on or before 03/17/25, LPA provided PIN 746115, should the Licensee wish to pay electronically. The governing body was observed to be active and in good standing. The facility is to submit the following: updated LIC610E, Emergency Disaster Plan as well as an updated Centrally Stored Medication And Destruction Record for all (3) residents in care to the department no later than 5pm on 03/02/25. During the employee file review LPA observed for the (3) staff files to not contain proof of ongoing/current training(s) received. Deficiency cited. Licensing Program Analyst (LPA) Jacqueline Shaw Ross made an unannounced visit to the facility for the purpose of an annual review. LPA was greeted by Administrator, Amelia Perlow and explained the purpose of the visit. A tour of the facility was conducted inside and out. Present at the facility were two clients and one additional staff was available. The facility is a four (4) bedroom four (4) bathroom one story home. During the tour the following was observed: Clients bedrooms had the required furnishings and were observed to be in good condition. Bathrooms had required signage, hand rails, non-slip mats. Night-lights were observed in the hallways. Fixtures and furniture for an operational facility are present and in good repair. All passageways were free of obstructions, charged fire extinguishers and the fire alarm system was operable, medications are kept centralized and locked, hazardous items are kept inaccessible to clients. Hot water was tested at 118.5 degrees Fahrenheit. Backyard area is free from obstructions. Kitchen/Food Service: LPA observed the entire kitchen, food is stored properly and dishes are clean and in good condition. There is a sufficient supply of perishable and non-perishable foods. Area was observed to be clean and functional. Care & Supervision: Facility has sufficient care staff employed. Administration: Emergency exiting plans, telephone numbers and Ombudsman information and other required signage are posted throughout the facility. The following deficiency was observed: The Administrator could not recall nor have record of the last emergency drill conducted. LPA advised Administrator drills are to be conducted quarterly. Deficiency cited. Record Review and Client/Staff Files: LPA reviewed current staff and all staff have has Criminal Background Clearance and trainings are current, however the following deficiency was observed: -The facility staff did not have current CPR/First Aid certification. Certification for all staff expired on 6/7/2023. Deficiency cited. Client records were reviewed and contained required documents that included Admission Agreement, Pre-Appraisal, current Physician's Report. Medication Review: LPA reviewed medication and medications appear to be dispensed according to physician's orders, however the following deficiency was observed: - The facility did not have an updated medication record keeping system of medication dosages given to residents. Administrator stated they are in the process of updating the system. Deficiency cited.
