On 04/06/2026, Licensing Program Analyst (LPA) Jacqueline Shaw-Ross, arrived at the facility to conduct a annual required visit. LPA was greeted and granted entry by Administrators Arni and Hamid Montazer where LPA explained the purpose of the visit. The facility is approved for (6) non ambulatory residents of which 1 may be bedridden. At the time of the visit there were (5) residents present of which one resident was asleep during LPAs visit. The facility is licensed to serve age range 60 and over and an approved hospice waiver for (6) with (2) resident currently receiving hospice services. During today's visit LPA observed annual fees were paid up to date, and the governing body was observed to be active and in good standing. The facility interior and exterior areas of the facility were observed to be clean, and free of any obstructions. The food supply was observed to be sufficient as the requirement was met. The facility has a clean pool that is housed behind a locked fence. There are no known guns or ammunition. The emergency disaster drills are being conducted on a quarterly basis with the last drill being on 04/01/2026. The fire extinguishers are fully charged and operable. The hot water measured to be 126 degrees Fahrenheit which is over the regulatory limit, however Administrator was able to adjust the temperature at the time of visit. Hot water retest measured at 118 which is within regulatory limits. The medications, sharps, chemicals and other hazardous items were observed to be locked and in accessible to residents in care. The facility is armed with a security alarm and video surveillance in common areas. Medications are being administered per physicians orders. All staff present had the required training and possessed valid CPR certification. Staff have criminal record clearance and are associated to the facility. The Administrator Arni Montazer's administrator certification expires on 08/12/2026. Resident files had the required documents such as admissions agreement, medical assessment and appraisals. Based on today's inspection no citations were issued. LPA was greeted and granted entry by Administrator Arni Montazer where LPA explained the purpose of the visit. (6) non ambulatory residents of which 1 may be bedridden. At the time of the visit there were (5) residents present of which 4 were asleep during LPAs visit. The facility is licensed to serve age range 60 and over and an approved hospice waiver for (6) with (1) resident currently receiving hospice services. During today's visit LPA verified contact information and will update accordingly. The facility annual fees that were due by 4/4/25 were observed to be paid, and the governing body was observed to be active and in good standing. The facility interior and exterior areas of the facility were observed to be clean, and free of any obstructions. The food supply was observed to be sufficient as the requirement was met. The facility has a clean pool that is housed behind a locked fence. There are no known guns or ammunition. The hot water measured to be within regulatory limits at 113 degrees Fahrenheit. The medications, sharps chemicals and other hazardous items were observed to be locked and in accessible to residents in care. The facility is armed with a security alarm and video surveillance. All staff present had the required training and possessed valid CPR certification. Staff have criminal record clearance and are associated to the facility. On 04/24/24 Licensing Program Analyst (LPA) Javina George conducted an unannounced 1 year required visit. LPA met with Administrator Arni Montazer. The facility is licensed for 5 non-ambulatory residents and 1 bedridden resident. The facility does not have a resident that is bedridden at this time. The facility has an approved hospice waiver for six (6), there are zero (0) residents receiving hospice services at this time. At the time of the visit there were four (4) residents and four (4) staff present. All staff were observed to have obtained criminal record clearance and are associated to the facility. LPA conducted a tour of interior and exterior of the facility. The facility was observed to be clean, clutter and odor free. The facility was observed to have a 2 day supply of perishable and a 7 day supply of non perishable food items. The carbon monoxide and smoke detectors were tested and were observed to be operable. The emergency disaster drills are being conducted on a quarterly basis, the last drill was conducted on 3/30/24. There are no known guns or ammunition on the premises. The facility utilizes video surveillance in the common areas. There is a secured/locked gated pool inside the backyard, with a shaded area for sitting. The facility has activities available for residents to promote socialization. Hazardous items such as, cleaning and disinfecting supplies are locked inside cabinets located inside of the laundry room. The knives, and other sharps are locked and inaccessible to residents inside a drawer inside of the kitchen across from the refrigerator. The medications are locked inside a closet next to bedroom #1 that requires a code to be entered on a key pad in order to entry/access. LPA conducted a review of resident files and observed for the required documentation such as the Emergency Identification form, Admission agreement video surveillance acknowledgment form, appraisals needs and services plan to be present. LPA conducted a review of staff files which are all housed in one binder. The staff were observed to have received their initial and annual training's, as well as possess current Cardio Pulmonary Resuscitation (CPR) cards. The Administrator certificate expires on 8/12/24. Based on today's inspection there were no deficiencies observed.
