On 8/7/26, Licensing Program Analyst (LPA) Kyle Wellington arrived unannounced to conduct an annual inspection. LPA met with Executive Director (ED), Bree Lofvendahl, who was informed of the purpose of the visit. The census at the facility is thirteen (13) residents. LPA received a resident roster from ED. LPA toured the inside and outside of the facility with Resident Services Director (RSD), May Ann Valledor. LPA conducted an observation and record review for the inspection. Facility Overview: Facility is a one story building with seven (7) resident bedrooms, seven (7) resident bathrooms, one (1) staff/visitor restroom, medication room, kitchen, dining room, commons area, activity room and attached garage. There are no pools, bodies of water or firearms at the facility. Facility has a fire clearance for fifteen (15) non-ambulatory seniors of which fifteen (15) may be bedridden. Hospice waiver was granted to the facility for six (6) residents. Infection Control: LPA observed hand sanitizers and soap dispensers throughout the facility. Cleaning equipment and cleaning supplies were kept in the locked garage and available for regular facility maintenance. LPA reviewed the facility’s infection control plan which met the department’s requirements. Physical Plant: LPA observed the inside and outside of the facility to be clean, safe and well kept. The floors, windows and doors were clean and well maintained. Common area and dining room furniture was in good repair. Residents’ bedrooms contained the required bedding, lighting and furniture. Residents' bathrooms had paper towels, soap, grab bars and non-slip floors in the showers. Laundry equipment appeared to be in good working condition. Laundry supplies were kept in the locked garage. The outdoor area was free of hazards and contained furniture and shaded area for the residents. Kitchen/Food Service: LPA observed the kitchen to be clean, organized, and well maintained. The kitchen had the ability to prepare and store food in a safe and clean environment. Kitchen appliances appeared to be in good working condition. All sharp objects were kept in the locked kitchen inaccessible to residents. Cleaning supplies were kept in a locked cabinet under the kitchen sink inaccessible to residents. Facility has over a two day supply of perishable foods and over a seven day supply of non-perishable foods. Care & Supervision: LPA observed two (2) staff members to supervise the thirteen (13) residents at the facility. Facility has sufficient staff to supervise the residents and operate the facility. Administration: LPA observed emergency evacuation plan (sketch), complaint procedures, long-term care ombudsman information, business license, facility license, and Administrator Certificate posted in the facility. Admin holds a current Administrator Certificate, CPR/First Aid Certificate and a Criminal Record Clearance. Record Review and Resident/Staff Files: LPA reviewed the records of three (3) resident files and three (3) staff files. Staff present have criminal record clearance and are associated with the facility. The files contained all the required documentation and paperwork. The staff and client files were kept in the facility office inaccessible to unauthorized individuals. Health Related Services/Incidental Medical Services: LPA observed clients' medications were centrally stored in a locked cabinet in the locked medication room inaccessible to residents. First aid kit was kept in the locked medication room. LPA reviewed two (2) clients' medications to the facility’s medication log to make sure all medication was accounted for and dispensed correctly. Disaster Preparedness and Safety: LPA reviewed the facility’s emergency and disaster plan. It is current and up to date. LPA observed pull fire alarm systems, smoke and carbon monoxide detectors and charged fire extinguishers throughout the facility. The four (4) fire extinguishers were charged and last serviced on 3/2/26. The facility's last fire and safety inspection was performed by the Escondido Fire Department on 11/25/25. Fire drills are done quarterly and last conducted on 7/15/26. Facility's property and liability insurance is current and expires on 4/1/27. All facility entrances and exits were clear of obstructions. No deficiencies were cited during this visit. On 7/9/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by Caregiver, Dulce Ruiz who was informed of the purpose of the visit. Administrator, Brigitta Lofvendahl was also notified of the purpose of the visit. The facility has a fire clearance to serve 15 non-ambulatory or bedridden elderly residents and an approved hospice waiver for 6. LPA was informed two (2) residents are currently receiving hospice services at the facility. LPA toured the facility with Licensed Vocational Nurse (LVN), Veronica Delval and observed two (2) care staff present. Resident bedrooms had the required bedding, furniture, and lighting. Bathrooms had grab bars and non-skid mats in the showers. No bodies of water were observed on the premises. Indoor and outdoor pathways were free of obstruction. The facility had a two-day supply of perishable foods and seven-day supply of non-perishable foods. Medications are secured in the locked medication room. Maintenance Staff, Antonio Torres tested one (1) of the smoke alarms/carbon monoxide detectors and LPA heard them to be hardwired and operational. LPA also observed a charged fire extinguishers mounted throughout the building last serviced on 3/27/2025. The facility's certificate of liability insurance expires on 4/1/2026. Staff present have a criminal record clearance and are associated with the facility. Resident files reviewed had updated physician's reports, signed admission agreements and Departmental required records. Staff files reviewed had all Departmental required records as well. LPA observed several board games and puzzles in the activity room available for resident leisure. Long Term Care Ombudsman's contact information, evacuation plan, and facility sketches are visibly posted. During today's visit, LPA did not observe any issues or concerns. On today's date 07/02/24 at 11:47am Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility for the purpose of conducting a 1 year required visit/annual inspection. LPA met with Executive Director Brigitta Lofvendahl and explained the purpose of the visit. At the time of the inspection there were 3 staff and 13 residents present. The facility is licensed to serve residents age 60 and over, 15 non ambulatory of which 15 may be bedridden. The facility has an approved hospice waiver for six (6). There is currently one (1) resident receiving hospice services. Below are the observations made during today's visit: The facility is a single story structure with 7 combined bedrooms and bathrooms, dining area, front and back patio, kitchen, living room and medication room. The facility was observed to be clean and clutter and odor free. The resident bedrooms were observed to have the required furniture, the call lights were observed to be operable.The facility food supply was sufficient. LPA observed all toxic chemicals and other hazards/sharps to be inaccessible to residents. The hot water temperature was tested and found to be within regulatory limits. The facility has fire extinguishers fully charged with the tags in tact. The emergency drills are conducted quarterly, the last drill was conducted on 4/16/24. The last fire inspection was conducted on 12/14/23. Records review: Staff present were observed to have obtained criminal record clearance and to be associated to the facility. Staff present were observed to have current CPR/First Aid training, and the Administrator certificate which expires on 04/16/2025. Resident files had the required documentation such as physician's report, appraisal, and admissions agreement. No deficiencies were observed during today's annual inspection.
