On 09/11/25, Licensing Program Analyst (LPA) Aziz Faizi made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by caregiver Fernanda Frias who was informed of the purpose of the visit. The facility is a one-story home with nine (9) bedrooms and (9) bathrooms, including a garage and a game room for residents. There are no pools or known firearms on the premises. LPA toured the facility's exterior and observed outdoor pathways were free of obstructions. Outdoor shaded seating area is available for the clients in care. The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. LPA toured the kitchen and observed the facility has a two-day supply of perishable foods and more than a seven-day supply of non-perishable foods, which are stored in a safe and healthy manner. LPA observed sharp and dangerous objects were securely locked and inaccessible to residents. Laundry room and equipment was in good working condition. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was 105.9°F. Fire extinguishers equipped throughout the building are maintained in accordance with the department's safety requirements expiring on 03/31/2027. Adequate staff were present to supervise clients during the visit. The administrator holds a current administrator’s certificate expiring 04/16/2027 . Continued with 809C LPA reviewed files for three (3) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. Three (3) resident files were reviewed and contained all required documentation. All residents’ medications were securely locked in medication room. LPA observed medications were checked and listed ensuring accuracy and proper documentation by the facility and staff. LPA reviewed the facility’s emergency and disaster plan, including documentation of the last fire/earthquake drill conducted on 07/09/2026, which met department requirements. All facility exits were clear of obstructions. No deficiencies were cited during the visit. Allegation investigated: Staff violated a resident's personal rights.

On 10/20/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to commence an investigation in regards to the allegation noted above. On 10/17/25 Community Care Licensing received a complaint alleging staff violated the personal rights of Resident#1 (R1). During today’s visit LPA conducted a walk through of the facility in an attempt to locate R1 to conduct an interview. LPA observed for (R1) to not be in any of the designted resident bedrooms. After further investigation LPA conducted a records review of the facility census, which revealed that R1 was residing at the Sister facility therefore the complaint allegation of staff violated a resident's personal rights is unfounded. A finding that the complaint is unfounded means that the allegation is false, could not have happened, and/or is without a reasonable basis. On today's date 07/11/24 at 10:00am Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility for the purpose of conducting a 1 year required visit/annual inspection. LPA met with Executive Director Brigitta Lofvendahl and explained the purpose of the visit. At the time of the inspection there were 4 staff and 11 residents present. The facility is licensed to serve residents age 60 and over, 15 non ambulatory of which 15 may be bedridden. The facility has an approved hospice waiver for six (6). There is currently one (1) resident receiving hospice services. Below are the observations made during today's visit: The facility is a single story structure with 7 combined bedrooms and bathrooms, dining area, front and back patio, kitchen, living room, activities room, medication closet, and garage which is used to store the laundry appliances, generator and extra food. The facility was observed to be clean and clutter and odor free. The call lights were tested and observed to be operable. LPA observed all toxic chemicals and other hazards/sharps to be inaccessible to residents. The medications are locked inside the mediation closet, that is accessed via a key pad. The facility food supply met the requirements of a 2 day supply of perishable and a 7 day supply of non perishable food items. There were expired canned goods, that were discarded at the time of LPAs visit, therefore no citations will be issued. Staff present were observed to have obtained criminal record clearance and to be associated to the facility. Staff present were observed to have current CPR/First Aid training, and possess a valid Administrator certificate which expires on 04/16/2025. Resident files were observed to have the required documentation such as physician's report, appraisal, and admissions agreement. The hot water temperature was tested and found to be within regulatory limits, measuring at 107.9 degrees Fahrenheit. The facility has (2) fully charged fire extinguishers fully charged. The emergency drills are conducted quarterly, the last drill was conducted on 4/16/24. The last fire inspection was conducted on 12/14/23. The smoke and carbon monoxide detectors were tested and found to be operable. Based on today's inspection, no deficiencies were observed during today's visit. The facility was found to be in compliance with California Code of Regulations (Title 22, Division 6, Chapter 8).
