On April 17, 2026, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPA was greeted and granted entry into the facility by care giving staff after explaining the purpose for the visit. Compliance Manager (CM) Michelle Nesbitt and Assistant Administrator (AA) Kimberly Arnett were notified via telephone and later arrived to assist with the inspection. LPA observed that Kimberly Arnett has a valid administrator certificate which expires on July 21, 2026. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents and has a hospice waiver for six. The facility is a single story home with five resident bedrooms, one of which is shared, one staff bedroom, four and a half resident bathrooms, a living room, a dining room, a kitchen, and an attached two car garage. LPA, accompanied by the CM and the AA, conducted a tour of the interior portion of the facility. On today's visit, LPA observed six residents in care and four care giving staff present. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall by the entryway of the facility. LPA inspected the five resident bedrooms and they were observed to be free of hazards. LPA observed resident bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA observed additional linens to be stored in a hallway cabinets. LPA inspected the four and a half resident bathrooms and they were observed to be clean. Resident bathrooms are equipped with grab bars and nonskid floor mats. Faucets and toilets were operational. Hot water temperature measured between 114 and 115.1 degrees Fahrenheit. LPA observed the staff bedroom to be kept locked and inaccessible to residents in care. CONTINUED ON LIC809-C LPA observed the kitchen has a two day perishable and a seven day nonperishable food supply on hand. LPA observed kitchen appliances to be clean and operational. LPA observed knives and sharps to be stored in a locked kitchen cabinet. LPA observed chemicals and toxins to be stored in a locked kitchen cabinet under the sink. LPA observed fire extinguishers to be mounted on the wall in the kitchen and in the resident hallway. LPA observed fire extinguishers to be charged and serviced as of April 8, 2026. LPA tested the smoke detectors and carbon monoxide detectors which tested operational. LPA observed the facility conducted their last emergency disaster drill on March 1, 2026. LPA observed the centrally stored medication to be kept in a locked storage cabinet located in the kitchen. LPA observed a first aid kit mounted on the wall in the kitchen and it had all the required components. LPA observed the garage to be inaccessible to residents in care. LPA observed the garage to be used for storage. LPA observed the facility has a three day emergency food and water supply to be stored in the garage. LPA, accompanied by the CM and the AA, conducted a tour of the exterior portion of the facility. LPA observed the exterior to be clear of any obstructions and hazards. LPA observed a shaded outdoor seating area with furniture for resident use. The perimeter gates of the facility are self-latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all six resident files. All the required documentation were present and current in the resident files reviewed. LPA reviewed all six residents' medication and medication administration records. LPA reviewed five staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, no deficiencies are being cited per the Title 22 of the California Code of Regulations. Allegation investigated: Inadequate toileting care provided by facility staff resulted in multiple occurrences of urinary tract infections among residents Facility staff is not following the Infection Control Plan Administrator made a false statement about the whereabouts of a former employee

On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of following up on the investigation of the three allegations listed above. LPA was greeted and granted entry by facility caregiving staff after introducing himself and stating the purpose of the visit. Administrator staff Kimberly Arnett, Michelle Nesbitt and Jozef Springer were notified via telephone and arrived later to assist with the visit. The initial complaint investigation visit was conducted on February 6, 2025. During the visit, LPA reviewed the resident records for four currently admitted residents. Four staff interviews conducted during the visit. Additional records requested to be provided by staff at the earliest convenience. Additional record review and witness interviews were conducted during the investigation. CONTINUED ON FORM LIC9099-C Unsubstantiated CONTINUED FROM FORM LIC9099 Regarding the allegation that Inadequate toileting care provided by facility staff resulted in multiple occurrences of urinary tract infections among residents, the following has been concluded: Based on staff and witnesses interviews along with review of records provided, the investigation evidenced the presence of a procedure for the reporting of suspected urinary tract infections to the residents' primary care provider accompanied by demonstrated prophylactic measures as well as diagnostic tests being ordered. A review of medical records did not evidence a higher rate of infections that could be attributed to a failure to provide adequate care and supervision Regarding the allegation that Facility staff is not following the Infection Control Plan, the following has been concluded: Cases of upper respiratory infections occurred in late December/early January and were adequately reported to the Department by the infection prevention staff designated on the Infection Control Plan. Based on records reviewed, preventative treatment was offered to residents. Personal Protective Equipment was confirmed to be available for staff and visitors and mask-wearing was confirmed to be enforced at that time. Follow-up visits did not evidence any ongoing outbreaks at the time. Regarding the allegation that Administrator made a false statement about the whereabouts of a former employee, the following has been concluded: The termination of staff member S1 was confirmed to have occurred following a verbal altercation with another staff member effective on October 24, 2024. The final pay stub for the staff member was provided to the investigation and the separation was made effective in Guardian. Based on the investigation conducted, all three allegations are determined to be Unsubstantiated, meaning that although the allegations may have happened or are valid, there is no preponderance of evidence to prove the alleged violations did or did not occur. On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by facility caregiving staff after introducing himself and stating the purpose of the visit. Administrator staff Kimberly Arnett, Michelle Nesbitt and Jozef Springer were notified via telephone and arrived later to assist with the visit. There are currently six residents in care, one of which is receiving hospice care. LPA observed residents relaxing in the facility's common living areas or in their respective bedrooms. LPA accompanied by facility staff toured the physical plant. The facility is a one-story house with a detached garage. The facility has four private rooms, one shared room along with one locked staff room. There is one shared bathroom as well as two and a half en-suite bathrooms throughout the facility. Bedrooms appear clean and sanitary. Four residents are currently using half-rails for postural support along with an additional resident using full rails. Appropriate physician orders and/or hospice plan of care on file for all five relevant residents. All resident bedrooms have the required furnishings. Bathrooms appear clean and sanitary. Hot water temperature measured at 130F in three separate bathrooms with faucets used for personal grooming. Type B deficiency issued and cleared later during the visit. LPA observed the kitchen has a minimum two (2) day perishable and seven (7) day non-perishable food supply. Sharp items, cleaning products and the medication central storage all verified to be secured. Fire extinguishers are charged and mounted, with up-to-date maintenance documented on the attached tags. LPA tested the smoke and carbon monoxide detectors which were found to be operational. The detached garage is inaccessible to residents and used for laundry as well as additional storage of food and supplies. CONTINUED ON FORM LIC809-C CONTINUED FROM FORM LIC809 LPA and facility staff toured the outside of the facility. LPA observed an shaded outdoor seating area with furniture for resident use. There are self-latching gates on each side of the property and the route of egress is free of clutter and obstructions. There are no bodies of water on the premises. Facility does not utilize locked perimeters or delayed egress. LPA reviewed six resident records which included all necessary components. LPA reviewed resident medication records and prescription orders for all residents with no discrepancies observed. Oxygen is currently in use with adequate signs on display. There are no bedridden residents present on the premises. LPA reviewed staff records for all three caregivers present. Records were found to be complete. Training and CPR/First aid training reviewed and up-to-date. All staff are background cleared and associated to the licensed location accurately. Disaster drills are conducted quarterly. Based on the observations made during today’s visit, one type B deficiency is being cited per Title 22 Division 6 of the California Code of Regulations. Allegation investigated: Staff got in a verbal altercation in front of residents

On this day, LIcensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of following up on the investigation of the allegation listed above. LPA was greeted and granted entry by facility caregiving staff after introducing himself and stating the purpose of the visit. Administrator staff were notified by telephone and arrived later to assist with the visit. An initial complaint investigation visit was conducted on November 4, 2024. LPA accompanied by facility staff conducted a tour of the physical plant. LPA requested and obtained resident records which were reviewed during the visit. A joint interview was conducted with administrative staff during the visit. During the present visit, LPA conducted or attempted six resident interviews along with two staff interviews. CONTINUED ON FORM LIC9099-C Unsubstantiated CONTINUED FROM FORM LIC9099 Regarding the allegation that Staff got in a verbal altercation in front of residents, the following has been concluded: Based on records reviewed and interviews conducted, it has been confirmed that a verbal argument has taken place between two staff members on October 24, 2024. Facility staff however confirmed that professional coaching was immediately provided to one staff member involved and that one of the two staff members involved was terminated and no longer works at the facility. Additionally, resident interviews conducted did not corroborate that the argument resulted in effectively creating an unfavorable or unsafe environment for the residents in care. As a result, the allegation is found to be Unsubstantiated, meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit for the purpose of conducting a Required Annual Inspection. LPA was greeted and granted entry by facility caregiving staff after introducing himself and stating the reason of the visit. Administrator staff was notified of the visit and arrived later to assist. During the inspection, LPA and administrator conducted a tour of the physical plant and observed the following: The facility is a one-story home with five resident bedrooms, one staff room and three bathrooms. All resident bedrooms had the required furnishings. LPA observed all beds had linens and blankets and an adequate additional supply is present. The backyard has a shaded area and the route of egress is free of clutter and obstructions. There are currently four residents in care at the facility, two of which are receiving hospice care. Residents are observed to be clean and appear well taken care of. Bathrooms faucets and toilets were operational. Water temperature was verified to be within acceptable range. LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Combined smoke and carbon monoxide detectors tested operational. There are two fire extinguishers present, both observed to be fully charged with up-to-date maintenance. Medication, sharp items and cleaning supplies were confirmed to be inaccessible throughout the physical plant. The medication central storage was also observed to be secure and reviewed for accuracy during the visit. LPA reviewed four resident files and three staff files. LPA conducted three staff interviews. Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations.
