Licensing Program Analyst (LPA) Jessica Cho arrived at the facility unannounced for the purpose of conducting the Required 1-Year annual inspection. LPA was greeted and granted entry by Caregiver Jun Lumague after stating the reason for the visit. Administrators (Admins) Wilma Fuentes has a valid certificate expiring January 4, 2027 and December 6, 2026 for Jonalyn Mejia. The following was observed during the inspection: This is a single story residential home. The facility is clean, sanitary, and in good repair. LPA observed six residents of which LPA verified four are receiving hospice services. LPA observed two caregivers on duty and verified fingerprint clearances and association statuses. The facility is operating within the conditions and limitations specified on the license. There are two private resident bedrooms, two shared resident bedrooms, one staff bedroom, and two resident bathrooms. All common areas were inspected including the attached two car garage. All residents' bedrooms have the required furnishings, and bathrooms are found to be in compliance equipped with grab bars and slip resistant mats. The hot water temperature measured at 119.8 degrees Fahrenheit in both bathrooms. Toxins, disinfectants, sharps, and medications were secured and inaccessible. The fireplace is appropriately screened in the living room. LPA observed ample two day supply of perishables and seven-day supply of non-perishable food. LPA toured the exterior portion of the facility. LPA observed the outdoor passageway is free of obstruction. The exit gate is operational and there were sufficient seating and shading. The fire extinguisher was purchased per receipt date of June 17, 2026. The auditory devices and smoke/carbon monoxide detectors were tested and operational. LPA observed at least three days of emergency food and water supplies including disaster supplies. Fire drills are conducted quarterly with the last date on August 11, 2026. The Emergency Disaster Plan (LIC610E) was reviewed and updated annually as required. LPA observed the required 'See Something, Say Something' (PUB475) poster posted in the correct size. LPA audited six residents' files and two personnel files in which no discrepancies were found. Medications were audited for two out of six residents. No discrepancies found. LPA consulted on the following: to ensure type of emergencies covered varies from quarter to quarter as per page 1 of LIC610E. Based on the observations made during today's visit, no deficiency is being cited. An Advisory Note is being issued. Allegation investigated: Staff is repeatedly yelling at resident Facility is not following Physical Therapy orders resulting in worsening of pressure ulcer.

Licensing Program Analyst (LPA) Celine Rodriguez conducted an unannounced continuation visit to the facility for the complaint and to deliver the findings. LPA Rodriguez explained the purpose of today's visit, was greeted by staff on duty, and met with Facility Administrators (AD) Jonalyn Mejia & Wilma Fuentes. During the investigation, LPA De Perio toured the physical plant of the facility, conducted interviews, and requested copies of pertinent records reviewed. It was alleged that staff is repeatedly yelling at resident. LPA Rodriguez conducted a total of 5 interviews that consisted of residents and staff, of which all 5 interviews did not corroborate with the allegation. Interviews with residents described staff as "nice", and denied of ever being yelled at, and denied of observing staff yell at residents. Per record review, staff are trained on how to care for residents, along with resident rights. Upon entering the facility, LPA observed staff engaging in activities with residents in the living room, and that residents were observed to be happy. Unsubstantiated It was alleged that facility is not following Physical Therapy orders resulting in worsening of pressure ulcer. LPA Rodriguez conducted a total of 5 interviews that consisted of residents and staff, of which all 5 interviews did not corroborate with the allegation. Per interviews and record review, resident 1 (R1) was admitted to facility, after getting discharged from the hospital due to a hip fracture that was sustained. During the investigation, an interview was conducted with R1's responsible party, and it was revealed that although R1 had redness and soreness on R1's tailbone, there was never an instance where R1 had a pressure ulcer. In addition, there were no documents presented from the facility, the hospital, and from home health that R1 ever had a pressure ulcer. Based on LPA’s interviews which were conducted, review of documents obtained, and observations, LPA is unable to ascertain if the allegation occurred as reported. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove or refute the alleged violation occurred; therefore, this allegation is deemed UNSUBSTANTIATED. LPA Rodriguez explained the purpose of today’s visit, and was greeted and granted entry by staff on duty, who informed facility administrators (AD) Jonalyn Mejia & Wilma Fuentes about visit. For this visit, LPA Rodriguez met with AD Mejia. For today’s visit, LPA Rodriguez observed a total of 5 residents in care and 2 staff members on duty. LPA Rodriguez observed the Administrator's Certificate for Jonalyn Mejia which expires on 12/6/2026 and Wilma Fuentes which expires on 1/4/2027. LPA Rodriguez observed the PUB 475 "See Something Say Something" poster located in the hallway. LPA Rodriguez toured the interior and exterior portions of the facility with staff on duty. The facility is a single level structure and is licensed for 6 non-ambulatory residents, of which 4 may be on hospice, but has an approved waiver for 6 residents to be on hospice, and 1 bedridden. There are a total of 4 bedrooms of which 2 are private resident rooms, 2 shared resident rooms, and 1 room designated for caregivers. LPA Rodriguez toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detector and auditory exit alarms were tested and operational. There are a total of 2 restrooms in the facility. Restrooms were observed to be in good repair, toilets were operational, and grab bars and non-skid floor mats were provided. Water temperature tested between 110.3 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked and inaccessible to residents in care. Fire extinguisher was mounted, charged, tagged for the date 2025, and was located in the kitchen. LPA Rodriguez observed the emergency disaster and evacuation plan, which is located in the hallway. Facility had back-up emergency food and water supply located in the garage. LPA Rodriguez observed that First Aid Kit had all the required components. LPA Rodriguez observed that medications and toxins were locked and inaccessible to residents in care. For the exterior portion, LPA Rodriguez observed a shaded area, patio furniture, and the grounds were free of any hazards. There is 1 gate in the backyard, which was observed to be self-closing and self-latching. No bodies of water were observed. LPA Rodriguez reviewed all five resident files and observed that the staff currently present at the facility were background cleared and associated via Guardian. For today's visit no deficiencies were issued per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with AD Mejia and AD Fuentes. The Licensing Program Analyst (LPA Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA met with Administrator Jonalyn Mejia and explained the reason for the visit. The Administrator's certificate expires on December 6, 2024. LPA and the Administrator toured the facility. The last fire drill was conducted on April 2, 2024, Smoke detectors and the carbon monoxide detectors tested operational. The facility is a single story home with 5 bedrooms, 2 bathrooms, living room with a screened fireplace, dining room, kitchen and an attached two car garage. LPA observed the see something say something sign is posted in the hallway and measures 8 1/2 by 11 inches. LPA observed the garage is used for storage of supplies and old furniture. LPA observed the kitchen is clean and organized. The knives and sharp objects are kept locked in a kitchen drawer. Cleaning supplies are kept locked under the sink. LPA observed a two day perishable and a seven day non-perishable food supply on hand in the kitchen. The fire extinguisher in the kitchen is fully charged. The facility has 5 bedrooms, 4 are for residents and one bedroom is for staff. LPA observed all resident rooms had the required furniture and bed linens. Extra linens are stored in the hall closet. LPA observed both bathrooms are clean and operational. Hot water measured 109.0 degrees Fahrenheit in both bathrooms. No obstacles or hazards observed inside the facility. LPA and Administrator toured the backyard. No bodies of water observed. There is a shaded seating area for residents to sit outside. The exit gate is operational and is self closing. No obstacles or hazards observed in the backyard. LPA reviewed 6 out of 6 resident records, no discrepancies observed. LPA reviewed 6 out of 6 resident medications, no discrepancies observed. LPA reviewed 4 staff files, no discrepancies observed. LPA inspected the first aid kit. The first aid kit has all the required elements. LPA consulted with Administrator concerning reporting requirements. No deficiencies observed during the visit. No deficiencies are being cited as a result of this visit.
