Licensing Program Analyst (LPA) Jessica Cho arrived at the facility unannounced for the purpose of conducting the Required 1-Year annual inspection. LPA was greeted and granted entry by Caregiver Lupita Romero after stating the reason for the visit. Administrator Cristo Forero arrived shortly after and assisted with the investigation. The application to renew administrator's certificate for Admin Forero was received on July 1, 2026 and is pending review. Olga Moreno has a valid certificate expiring December 18, 2026. The following was observed during the inspection: This is a two story residential home. The facility is clean, sanitary, and in good repair. The first floor has two shared resident bedrooms and one resident bathroom. The second floor has three bedrooms and two bathrooms used by one live-in staff. No residents occupy the second floor. An evacuation chair was observed by the stair. LPA observed one staff on duty providing care to two residents of which one is on hospice. LPA verified fingerprint clearance and association status of the staff. The facility is operating within the conditions and limitations specified on the license. All common areas were inspected including the attached two car garage. All residents' bedrooms have the required furnishings, and the bathroom on the first floor is found to be in compliance equipped with grab bars and a slip resistant mat. The hot water temperature measured at 119.4 degrees Fahrenheit in the resident bathroom on the first floor. Toxins, disinfectants, sharps, and medications were secured and inaccessible. The fireplace is appropriately screened in the living room. LPA observed ample two day supply of perishables and seven-day supply of non-perishable food. LPA toured the exterior portion of the facility. LPA observed the outdoor passageway is free of obstruction. The exit gate is operational, and there were sufficient seating and shading. The fire extinguisher was purchased per receipt date of August 3, 2026. The auditory devices and smoke/carbon monoxide detectors were tested on both levels and operational. LPA observed at least three days of emergency food and water in the red backpack stored in the garage and additional disaster supplies stored in a tub also in the garage. Disaster drills are conducted quarterly with the last date on June 30, 2026. The Emergency Disaster Plan (LIC610E) was reviewed and updated annually as required. LPA observed the required 'See Something, Say Something' (PUB475) poster posted in the correct size. LPA audited two residents' files and one personnel file in which no discrepancies were found. Medications were audited for two out of two residents. No discrepancies found. Based on the observations made during today's visit, no deficiency is being cited. An Advisory Note is being issued. Licensing Program Analyst (LPA) Celine Rodriguez conducted an unannounced visit for the required annual inspection. LPA Rodriguez explained the purpose of today’s visit, and was greeted and granted entry by staff on duty, who informed facility administrators (AD) Olga Moreno and Cristo Forero about visit. For today’s visit, LPA Rodriguez observed a total of 4 residents in care, of which 1 is on hospice and 3 staff members on duty. LPA Rodriguez observed the Administrator's Certificate for Olga Moreno which expires on 12/18/2026 and Cristo Forero which expires on 6/26/2026. LPA Rodriguez observed the PUB 475 "See Something Say Something" poster located in the hallway. LPA Rodriguez toured the interior and exterior portions of the facility with staff on duty. The facility is a two level structure and is licensed for 4 residents of which 1 may be on hospice. On the first level, are a total of 2 bedrooms of which both are shared resident rooms, and 1 bathroom. On the second level, there are 3 rooms designated for staff and 2 bathrooms. LPA Rodriguez confirmed and observed that there are no residents residing on the second floor, and the second floor bathrooms are not for resident use. LPA Rodriguez toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detector and auditory exit alarms were tested and operational. Restrooms were observed to be in good repair, toilets were operational, and grab bars and non-skid floor mats were provided. Water temperature was tested and observed to be at 112.1 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked and inaccessible to residents in care. Fire extinguisher was mounted, charged, tagged for the date 2025, and was located in the living room. LPA Rodriguez observed the emergency disaster and evacuation plan, which is located in the hallway. Facility had back-up emergency food and water supply located in the garage. LPA Rodriguez observed that First Aid Kit had all the required components. LPA Rodriguez observed that medications and toxins were locked and inaccessible to residents in care. For the exterior portion, LPA Rodriguez observed a shaded area, patio furniture, and the grounds were free of any hazards. There is 1 gate in the backyard, which was observed to be self-closing and self-latching. No bodies of water were observed. LPA Rodriguez reviewed all four resident files and observed that the staff currently present at the facility were background cleared and associated via Guardian. For today's visit no deficiencies were issued per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with AD Moreno and AD Forero. Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA met with Administrator Olga Moreno and Cristo Forero and explained the reason for the visit. The facility is a two story home with 5 bedrooms, 3 bathrooms, living room, dining room, kitchen and a 2 car garage. Facility has a capacity for 4 non-ambulatory residents and a hospice waiver for 1. The second floor is for live in staff. The second floor has 3 bedrooms and 2 bathrooms. Residents only reside on the first floor. LPA observed the See Something, Say Something poster posted in the hallway measures 8 1/2 by 11 inches. LPA observed the Ombudsman poster posted next to the stairway. The Administrator's Certificate expires on December 18, 2024. LPA Observed the fireplace in the living room is screened. The smoke detectors tested operational. The carbon monoxide detector in the living room tested operational. The facility does not have internet service at this time. LPA observed the bathroom downstairs for residents is clean and operational. Hot water measured 116.2 degrees Fahrenheit. LPA observed the kitchen is clean and organized. Knives are kept locked in the kitchen cabinet. LPA observed medications are kept locked in the kitchen cabinet. LPA observed a 2 day perishable and 7 day non-perishable food supply on hand in the kitchen. The 4 burner gas stove lights unassisted. LPA reviewed 2 staff files. No discrepancies observed. LPA reviewed 3 resident files. LPA observed Resident 3 (R3) did not have a current appraisal/needs and service plan. No other discrepancies observed. LPA reviewed 3 resident medications, no discrepancies observed. LPA and Administrator toured the backyard. No bodies of water observed. There is a shade patio area to sit outside. The exit gate is operational. LPA inspected the garage. The garage is kept locked and used for storage. LPA inspected the first aid kit. The first aid kit has all the required elements. LPA consulted with the Administrator concerning reporting requirements. Deficiencies are being cited per Title 22 Division 6 of the California Code of Regulation.
