Type of visit: Required - 1 Year

On 11/14/15 Licensing Program Analyst (LPA) Javina George made an unannounced visit to conduct a 1 year required visit. LPA met with Administrator Eloisa Karatas, and explained the purpose of the visit. The facility has an approved fire clearance for (6) ambulatory of which (3) may be non ambulatory and (1) bedridden. The facility currently has 3 residents that are receiving hospice services, there are no residents receiving home health services and there is (1) bedridden resident. The facility was observed to be clean and clutter free. The resident medications, chemicals and sharps were observed to be locked and inaccessible to residents in care. The emergency disaster drills are being conducted on a quarterly basis with the last drill being held on 08/06/25, and there is a drill due by the end of this month. The fire extinguishers were last serviced July 2025. The hot water was tested and measured to be 11.5 degrees Fahrenheit. The smoke and carbon monoxide detectors were tested and found to be operable. The resident bedrooms were observed to have the required furniture (bed, chest of drawers, lamp. There was extra linen and personal hygiene supplies. The food supply was adequate meeting the staple of 2 day supply of perishable and a 7 day supply of non perishable food items. A file review was conducted of staff and resident files. Staff files reviewed were observed to have criminal record clearance and to be associated to the facility. The staff have received the initial and ongoing training with valid CPR certification. The facility annual fees and the governing body was observed to be in good standing. The resident files had completed admissions agreements and proper assessments. Based on today's inspection there were no deficiencies observed. Type of visit: Case Management - Incident

On 09/12/25 Licensing Program Analyst (LPA) Javina George made an unannounced case management Incident visit in regards to an Unusual Incident/Injury report that was received on 09/09/25. LPA met with Administrator Eloisa Karatas and informed her of the purpose of visit. The department received an incident report which revealed that on 09/08/25, Resident #1 (R1) returned from a home visit on 09/07/25. The next day R1 had discharged the fire arm, as there was a sound heard, smell burn and half of a bullet observed on the floor by facility administrator. Through the administrator's investigation revealed that R1 brought the hand gun back from their home visit on 09/07/25. There were no injuries sustained by R1, other residents or facility staff. The gun was removed from the premises on 09/09/25, by R1s responsible party, and the necessary cross reports were completed. During today's visit LPA verified with R1s responsible party that they did in fact pick up the hand gun. In addition per the administrator there are no other known guns or ammunition on the premises. There were no health and safety concerns observed during today's visit. Type of visit: Required - 1 Year

On November 08, 2024, Licensing Program Analyst (LPA), Venus Mixson arrived at the facility unannounced to conduct the Required Annual Inspection and met with the Administrator, Eloisa Karatas. The facility file review was conducted at the Regional Office and additional records were requested and reviewed on site. The facility is licensed for six Elderly Adults and is currently operating at a capacity of (5) five Elderly Adults (740). LPA Mixson toured the facility along with the Administrator and made observations pertaining to the annual visit. LPA inspected the facility inside and outside there were no obstructions or debris to the indoor or outdoor passageways at the time of this visit. Additionally, there were no bodies of water on the premises. The facility is a single-story home located at 21 Via Alta Vista Bonsall, CA. 92003. Physical Plant: The facility phone number is (760) 294-4141 and it is operable. LPA Mixson observed the residents’ bedrooms, and each was equipped with required furniture as per Title 22. LPA Mixson inspected facility bathrooms, and the hot water temperature tested within regulations at. The bathrooms were clean, and appliances were operating appropriately currently at the time of this visit. The facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguishers. LPA Mixson observed required postings such as "If you See Something, Say Something,” the "Personal Rights," and the PUB 475. The cleaning supplies and sharp items were kept locked and inaccessible to the residents in care. There was a designated storage space for the residents and staff files, and it was locked and inaccessible to residents in care currently at the time of this visit. Medications: Were locked and inaccessible to residents in care, and there was a sufficient supply of medication for each resident. The overall facility is clean, the furniture is in good condition. The facility cooling system and other appliances were operable currently at the time of this visit. Administrator informed LPA there were safety lights for night throughout the facility. Food Service: Non-perishable and perishable food supply is sufficient per regulations, and there are a variety of food types available for residents. Dishes and utensils were in sufficient supply and stored properly, and sharp items are locked. Care & Supervision/Administration: Adequate staff are present for the supervision of resident in care. Floor plans, telephone numbers and personal rights were found posted in the facility. The listed administrator possesses a current administrator’s certificate with an expiration date of 08/28/2026. Records Reviewed and Resident/Staff Files: LPA reviewed staff files and reviewed the facility's staff schedule. The staff files reviewed has criminal clearance and updated training along with First Aid Certification. LPA reviewed five resident files files reviewed were current and up to date on required paperwork. Disaster preparedness: LPA Mixson reviewed the facility's emergency and disaster plan as well as disaster training binder. LPA observed the last fire drill met the department standards, and was completed every three months 08/02/2024. Was conducted by Yusuf Karatas. Infection Control: LPA Mixson observed the hand washing stations in the facility restrooms. LPA observed PPE equipment and cleaning supplies to do regular cleaning of the facility. LPA reviewed the facility's infection control plan and found all required infection control measures. There were no deficiencies observed or cited per Title 22, Division 6 of the California Code of Regulations at this time. On November 07, 2023, Licensing Program Analyst (LPA), Venus Mixson made an unannounced visit to the facility to conduct an annual licensing inspection, and met with the Administrator, Eloisa Karatas The LPA introduced herself and stated the purpose of the visit. LPA Mixson toured the facility along with the Administrator, and inspected the facility inside and outside, and there were no obstructions to the indoor or outdoor passageways at the time of this visit. The facility is licensed to serve six residents; ages 60 and above; three of whom may be non-ambulatory and ambulatory, currently there are five residents and two staff. The facility has an approved hospice waiver for one resident. Physical Plant: The facility is a two story town home type with seven bedrooms, and five bathrooms, located at 21 Via Alta Vista, Bonsall, CA. 92003 The facility phone number is (760)295-4141, and is operable. The LPA observed the residents bedrooms, and they are equipped with required furniture as per Title 22. The LPA inspected the facility bathrooms, and the hot water temperature tested within regulations. The bathrooms were clean and appliances were operating appropriately at the time of this visit. The facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguishers. The LPA observed required postings such as; the Ombudsman poster, "If you See Something, Say Something" and the "Personal Rights" postings were posted at the front door. The cleaning supplies and sharp items were kept locked and inaccessible to the residents. There was a designated area for the resident and staff files. Medications: were reviewed with the Administrator and were locked and inaccessible to residents. The overall facility is clean, welcoming, and the furniture is in good condition. The facility air conditioning and other appliances were operable currently at the time of this visit. Food Service: Non-perishable and perishable food supply is sufficient per regulations, and there are a variety of food types available for residents. Dishes and utensils were in sufficient supply and stored properly. Care & SupervisionThe facility has sufficient staff, two caregivers at the time of this visit, and the staff were attending to the residents. Records Review: The LPA reviewed five resident files, two staff files, and reviewed previous CCL forms. There were no Title 22, Division 6 Regulation violations observed and/or cited during todays visit.
