On August 18, 2026, Licensing Program Analyst (LPA), Venus Mixson, without out notice to conduct the Annual Inspection and met with Facility Manager, Aidanne Galapon. Facility file review was conducted at the Regional Office and additional records were requested and reviewed on site. Facility is licensed for six, Elderly Adult Residents and is currently operating at a capacity of five, Elderly Adults; for a (740), facility type. The facility is operating within the scope of the license at the time of this visit. LPA Mixson toured the facility along with Facility Manager, Aidanne Galapon, and made observations pertaining to the annual visit. LPA inspected the facility inside and outside there were no obstructions or debris to the indoor or outdoor passageways at the time of this visit. The facility is a single-story home located at 1152 Sal Lane Vista, CA. 92084.The facility phone number is (760) 295-3523 and it is operable. LPA Mixson observed residents’ bedrooms, and each was equipped with required furniture as per Title 22 at the time of this vist. LPA inspected facility bathrooms, and hot water temperature tested within regulations. Bathrooms were clean, and appliances were operating appropriately currently at the time of this visit. Facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguishers. LPA observed required postings such as "If you See Something, Say Something" and the "Personal Rights." Cleaning supplies and sharp items were locked and inaccessible to the residents in care at the time of the visit. There was a designated storage space for facility files, and it was locked. Medications were locked and inaccessible to residents in care, and there was a sufficient supply of medication for each resident. The overall facility is clean, the furniture is in good condition. Facility cooling system and other appliances were operable currently at the time of this visit. Non-perishable and perishable food supply is sufficient per regulations, and there are a variety of food types available for residents. Dishes and utensils were in sufficient supply and stored properly, and sharp items were locked. Allegation investigated: Facility refusing to allow Resident additional care

On 04/26/2025 Licensing Program Analyst (LPA) Jose Calderon conducted an unannounced visit to Kelly’s Foothill Villa and was greeted by Staff Clara Bactad (S1). LPA Calderon spoke to S1 prior to entering the facility to conduct a risk assessment. LPA Calderon explained the purpose of this visit is to deliver the finding pertaining to the above-mentioned allegation. The investigation consisted of the following: LPA Calderon interviewed Staff S1-S2, resident R1-R6. LPA Calderon obtained the following records: Admission agreement (dated 05/20/2024), physician report (dated 06/07/2024), incident report (dated 09/18/2024), Takara Hospice skilled nursing notes (dated 02/18/2025), Takara Hospice visit notes (10/15/2024 to 01/17/2025) for R1. The investigation revealed the following: Unsubstantiated Regarding the Allegation: Facility refusing to allow resident additional care. This complaint alleged that the facility did not allow R1 hospice care. Records review indicate the following: Physician report indicate R1 nonverbal and has health issues. Incident report indicates that resident had a fall and hospice nurse was called. Admission agreement indicates that resident moved into the facility on 05/20/2024. Takara hospice visit notes indicate that resident was given hospice services from 10/15/2024 to 01/17/2025. Takara Hospice nursing notes indicates that resident passed away on 01/18/2025. Interviews indicate the following: 2 out of 2 staff deny the allegation. R1-R6 could not answer any questions due to health issues and was non-verbal. Based on interviews and supporting documentation, the preponderance of evidence standard has NOT been met therefore, the allegation of “facility refusing to allow resident additional care” is found to be UNSUBSTANTIATED. No deficiencies cited during today's visit. The facility file review was conducted at the Regional Office and additional records were requested and reviewed on site. The facility is licensed for six, Elderly Adult Residents and is currently operating at five, Elderly Adults (740), Facility Type. LPA Mixson toured the facility along with Lead Caregiver, Clara Castro, and made observations pertaining to the annual visit. Food Service: Non-perishable and perishable food supply is sufficient per regulations, and there are a variety of food types available for residents. Dishes and utensils were in sufficient supply and stored properly, and sharp items are locked. Care & Supervision: Facility has sufficient staff, currently two staff and five of five residents participating in family visits, meeting with Hospice Nurse, and other activities. Administration: Emergency exiting plans, telephone numbers and Ombudsman information and other required signage are posted throughout the facility. Drills are conducted monthly and logged. Facility tests and logs water regularly: Water was tested and logged on today and tested within regulations. Fire extinguishers are charged and in the green. Training and Administrator Certificate: LPA reviewed administrator’s certification for Kelsey B. Rosas, and it was current at the time of this visit with an expiration date of 01/27/2025. Training is done and documented. Records Review: LPA Mixson reviewed resident and staff files, conducted two staff interviews and two resident interviews. LPA Mixson reviewed current staff records and those reviewed have current Criminal Background Clearance, current First Aid and CPR certification, and training's are current. Client records were reviewed and contained required documents. Physician reports are current and TB test were completed at the time of admission. There was no Title 22, Division 6 Regulation violations observed or cited during today’s visit.
