Type of visit: Required - 1 Year

On August 07, 2026, Licensing Program Analyst (LPA), Venus Mixson made an unannounced visit to the facility to conduct the Annual Required visit, and met with Administrator, Caroline Armstrong. LPA introduced herself and stated the purpose of the visit. The facility has a fire clearance to serve six non-ambulatory elderly residents and an approved hospice waiver for two. During today's visit, there was two staff and six residents present. During today's visit, LPA toured the facility, along with Administrator and observed the facility is made up of a single-story home with six resident bedrooms, three Jack and Jill bathrooms, which connect several of the residents bedrooms. LPA observed the kitchen, dining area, living room and laundry room. The resident bedrooms possess the required furnishings per Regulations. LPA observed the bathrooms to have grab bars and non-skid mats in the showers at the time of this visit. LPA did not observe any bodies of water on the premises currently. Indoor and outdoor pathways were free of obstruction, clutter and debris at this time. LPA toured the kitchen and observed the door with a lock leading to the kitchen having been removed and knives are secured in a kitchen drawer. LPA also observed the facility has more than a two-day supply of perishable foods and seven-day supply of non-perishable foods. Medications and disinfectants are secured in different locked closets. LPA observed the smoke alarms, carbon monoxide detectors, and LPA observed them to operational. LPA also observed fire extinguishers mounted near the kitchen and living room that were in the green and serviced currently. LPA conducted staff and resident file reviews and found the required documents at the time of this visit. Additionally current staff possess current criminal record clearance, are associated with the facility, and have a valid first aid/CPR certification. POC were cleared with this visit. There were no observable health and safety challenges at the time of this visit. An exit interview was conducted. Type of visit: Required - 1 Year

On 7/10/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by Assistant Administrator (AA), Zoraida Margate who was informed of the purpose of the visit. Licensee, Bella Margate was reportedly unavailable at the time of LPA's visit. The facility has a fire clearance to serve six (6) non-ambulatory elderly residents and an approved hospice waiver for two (2). LPA was informed one (1) resident is currently receiving hospice services at the facility. During today's visit, there was two (2) staff and six (6) residents present. A record review revealed a change of ownership application has been submitted for this facility and currently pending approval by the Department. During today's visit, LPA toured the facility with AA and observed the facility is made up of a one-story home with six (6) resident bedrooms, three (3) bathrooms, a kitchen, dining area, living room and laundry room. Resident bedrooms had the required bedding, furniture, and lighting. Bathrooms had grab bars and non-skid mats in the showers. No bodies of water were observed on the premises. Indoor and outdoor pathways were free of obstruction. LPA toured the kitchen and observed the door with a lock leading to the kitchen has been removed and knives are secured in a kitchen drawer. LPA also observed the facility has more than a two-day supply of perishable foods and seven-day supply of non-perishable foods. Medications and disinfectants are secured in different locked closets. AA tested one (1) of the smoke alarms/carbon monoxide detectors and LPA observed it to be operational. LPA also observed two charged fire extinguishers mounted near the kitchen and living room that were last serviced on 11/26/2024. Staff present have a criminal record clearance, are associated with the facility, and have a valid first aid/CPR certification. LPA reviewed the facility's Earthquake and Fire Drill log noting the facility's last emergency disaster drill was conducted on 4/20/2025. The facility's certificate of liability insurance expires on 12/13/2025 and their licensing fees are up-to-date. Long Term Care Ombudsman's contact information, residents' personal rights, complaint procedures, facility sketch and emergency disaster plan are visibly posted near the front entrance and dining room. During today's visit, LPA did not observe any issues or concerns. Type of visit: Required - 1 Year

On 7/22/24 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility for the purpose of conducting a 1 year required visit. LPA met with Caregiver Mary Rose Delos Santos and explained the purpose of the visit. At the time of the visit there were (2) staff and (6) residents present. The facility is licensed to serve 6 non ambulatory senior residents. The facility has an approved hospice waiver for (2). There are currently (0) residents receiving hospice services. LPA was informed that the facility will be undergoing a change of ownership. The facility administrator was unavailable during LPAs visit to verify the status. LPA toured the facility interior and exterior of the facility. LPA observed the facility to be clean and in good repair. The facility is maintained at a cool and comfortable temperature. There are no known guns or ammunition, or pools or bodies of water on the premises. All outdoor and indoor passageways are free of obstruction. Food prep and storage areas are clean and organized. Food supply meets the requirement of one (1) week supply of nonperishable and two (2) day supply of perishables. The medications are centrally stored medication in a locked closet by the dining area. The facility has seven (7) dual fire and carbon monoxide detectors, 1 central alarm for the outside, they were tested and observed to be operable. The facility was observed to have the required postings. However there was no proof of liability insurance available for LPAto review. Deficiency cited. LPA conducted a review of (3) resident and (3) staff records/files. LPA observed for the files to have the required documentation such as a medical assessment, appraisal, and admission agreement. Regarding the staff files all staff present were observed to have obtained criminal record clearance and to be associated to the facility. The administrator Bella Margate does possess a valid administrator certificate. LPA observed Staff # 1, Staff #2 and Staff # 3 to all have expired Cardio Pulmonary Resuscitation (CPR). Deficiency cited. In addition LPA conducted a facility file review prior to conducting the visit and observed for the facility to have not paid their annual fees that were due on or before 07/01/24. As of today 7/22/24 the current amount due is $742.00. Deficiency cited. A review of the facilities emergency disaster drills that the facility is not conducting the drills on a quarterly basis, as the last documented drill is noted to have occurred on 6/3/23. Deficiency cited. Based on today's inspection deficiencies were cited on the attached 809D, in accordance with the California Code of Regulations (Title 22, Division 6, Chapter 8).
