On April 24, 2026, Licensing Program Analyst (LPA) Komal Charitra conducted an unannounced annual visit. LPA met with Caregiver, Arsenia Villadarez and explained the purpose of the visit. LPA toured the facility inside and outside including all of resident rooms, common areas & kitchen. The indoor and outdoor passageway was free of obstruction. No accessible bodies of water of fire safety hazards observed. This is a single story facility. There are four resident rooms; two of which are single private rooms and two that are shared rooms. Resident rooms were observed to be clean with all required furniture. LPA observed 3/4 resident beds to have half bed rails. Two full bathrooms were observed to be clean, odor-free and in good repair. Water temperature throughout the facility measured between 123.6-129 degrees F. First aid kit was observed to be complete. Dining room was observed free from tripping hazards. A comfortable temperature of 74 degrees F is maintained and lighting is sufficient for comfort. LPA observed two day perishables and seven day non-perishables. Medications, sharps, and chemicals were observed locked and inaccessible to residents in care. LPA observed all four residents' PM medication to be pre-poured into another container. Carbon monoxide monitors are working properly. All fire extinguishers have been checked and current as of August 2025. Emergency drills are logged and done every 4 months. LPA reviewed 4 resident records and 4 staff records. Medication review was done, and all medications are accounted for, and centrally stored medication records are updated. Deficiencies of the California Code of Regulations, Title, 22 cited on the LIC809-D. Failure to correct the deficiencies may result in civil penalties. On October 31, 2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced Case Management-Other visit. LPA met with caregiver, Arsenia Villadarez and LPA explained the purpose of today's visit. Caregiver informed the administrator, Karin Lastimosa via phone of LPA's visit. LPA delivered an immediate exclusion letter for a staff member who is currently associated in the facility. The administrator confirmed that this staff does not work at the facility. On April 11, 2025, Licensing Program Analyst (LPA) Komal Charitra conducted an unannounced annual visit. LPA met with Caregiver, Arsenia Villadarez and explained the purpose of the visit. LPA toured the facility inside and outside including all of resident rooms, common areas & kitchen. The indoor and outdoor passageway was free of obstruction. No accessible bodies of water of fire safety hazards observed. This is a two story facility. There are four resident rooms; two of which are single private rooms and two that are shared rooms. Resident rooms were observed to be clean with all required furniture. Two full bathrooms were observed to be clean, odor-free and in good repair. Water temperature throughout the facility measured between 109-114 degrees F. Extra linen was observed to be present. First aid kit was observed to be complete. Dining room was observed free from tripping hazards. A comfortable temperature of 70 degrees F is maintained and lighting is sufficient for comfort. LPA observed two day perishables and seven day non-perishables. Medications, sharps, and chemicals were observed locked and inaccessible to residents in care. Carbon monoxide monitors are working properly. Emergency drills are logged and done every three month. LPA reviewed 3 resident records and 4 staff records. Resident records are updated, complete and signed. Staff records are complete, with training logs that have met the basic requirement. Medication review was done, and all medications are accounted for, and centrally stored medication records are updated. No citations are issued during the visit. On March 15, 2024, Licensing Program Analyst (LPA) Komal Charitra conducted an unannounced annual inspection. LPA met with Caregiver, Arsenia Villadarez and explained the purpose of the visit. LPA toured the facility inside and outside including all of resident rooms, common areas & kitchen. The indoor and outdoor passageways were free of obstruction. No accessible bodies of water of fire safety hazards observed. Extra linen was observed. LPA toured four resident rooms; two of which are shared rooms and two are observed to be private resident rooms. All resident rooms were observed with all required furniture. LPA observed two full bathrooms. Bathrooms were observed clean and odor-free; equipped with liquid soap, paper-towels, and non-skid mats. LPA observed staff room located next to the bathroom. Extra linen supply was observed. First aid kit was present and complete. Living room and dining room was observed to be free from tripping hazards. A comfortable temperature is maintained and lighting is sufficient for comfort. Hot water throughout the facility measured at 115 degrees F throughout the facility. Sharps, toxins and medication were locked and inaccessible to residents. Carbon monoxide monitors are working properly. All fire extinguishers have been checked and current as of November 2023. LPA observed 2 days for perishables and 7 days non-perishables. Emergency drills are logged and done every four months. LPA reviewed 3 resident records and 3 staff records. Resident records are updated, complete and signed, however during record review and tour, LPA observed that one resident (R1) has a foley catheter, however an exception was not approved by CCL. In addition, although LPA observed home health documents and doctor's discharge documents, LPA did not observe a service plan to show what home health is responsible for and what the facility staff will be responsible for. Staff records are complete, with training logs that have met the basic requirement. Medication review was done, and all medications are accounted for, and centrally stored medication records are updated. Deficiencies of the California Code of Regulations, Title, 22 cited on the LIC809-D. Failure to correct the deficiencies may result in additional civil penalties. On 12/15/2023, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to conduct an annual visit. LPA was greeted by staff member (SM), Arsenia Villadarez and explained the purpose of the visit. It was asked at this time that SM Villadarez call the Facility Designated Administrator (FDA) to inform them that CCL was present at this time. LPA was able to speak to FDA Karin Lastimosa via telephone and it was learned that they were unable to come to the facility at this time due to other obligations. LPA was able to continue the visit with SM Villadarez. Current census was 2. 1 out 2 residents were out at their respective day program at this time. This facility is licensed to serve residents who are 60 and older. All 6 residents may be non-ambulatory. This facility also has a hospice waiver on file. LPA reviewed 2 resident files. It was observed that 2 out 2 residents did not have an updated physicians report and did not have a Needs and Appraisals Plan on file. LPA reviewed 3 staff files. It was observed that 1 out 3 staff files did not have an active First Aid Certificate at this time. The current administrator has a current administrator certificate #6018146740 and expires until 07/07/2024. A tour of the facility was conducted. Fire extinguisher was observed to be annual inspected on 11/14/2023 by Arrow Fire Protection Company. Smoke detectors and carbon monoxide was in good repair. A tour of the kitchen was conducted. LPA observed a sufficient amount of 2-day perishable and 7-day non-perishable food supply at this time. LPA observed a washer and dryer in the kitchen. Laundry soap was accessible at this time. LPA observed dish soap and toxins under the sink and was not locked and made inaccessible. LPA also observed a large part of the counter chipped off near the dishwasher. A medication cabinet was observed in the kitchen. Along with SM Villadarez, medication and medication dispensing logs were reviewed and compared. First aid kit was present and contained all the required components. A tour of 3 resident bedrooms were conducted. Furniture and other furnishings were observed to be in good repair at this time. A tour of 2 resident restrooms were conducted. LPA observed that one resident restroom did not have knob and observed water damage near the grab bar in the bathtub. It was stated that no residents use the tub at this time. Hot water temperature was taken to ensure it was within Title 22 regulations at this time. A tour of one staff room was conducted. A tour dining room, living room and all other areas intended for resident use were toured. Furniture and furnishings were observed to be in good repair. A tour of the yard was conducted. Perimeter fence was observe to be stable and in good repair. Gates were observed to be in stable and in good repair. LPA observed a yellow string connected to the perimeter gate. LPA advised that the yellow string be taken off as it is a fire clearance issue. LPA informed the Administrator that their annual fees are due and was provided a copy of their pin number. Administrator stated that annual fees will be paid by 12/18/2023. The following forms were asked to be submitted and provided to CCL: -LIC308 -LIC400 -LIC500 -LIC610E A technical violation is being provided for Sections 87303(a) 87458(a), and 87506(b). Per California Code of Regulations (CCR) – Title 22 – Division 6, Chapter 6, no deficiencies were observed during today’s visit.
