On 2/23/2026, LPA Grace Donato made an unannounced annual visit to the facility. LPA met with Administrator Lily Pena. LPA explained the purpose of the visit. LPA toured the facility inside and outside including resident rooms and kitchen area. LPA observed residents resting in their bedrooms with visiting families and other residents are having an activity in the living room. While touring the facility it was observed that the room temperature was at 74 deg F. Hot water was also tested in the bathrooms and the temperature was 112 deg F. The facility is observed to be clean, odorless, and well maintained. Sharps in the kitchen have a lock box accessible by code. Food supply was observed with an adequate two day perishable and seven day non-perishable. Carbon monoxide/ smoke detectors, sprinklers, and fire extinguisher were present throughout the facility. Emergency drills are done every quarter. Five resident records and two staff records were reviewed. All records are complete and updated. Centrally stored medication was locked in the medicine cart and inaccessible by residents. All medication was labeled and sorted by resident name. All medication logs are complete and updated. LPA toured the facility inside and outside including resident rooms and kitchen area. LPA observed residents resting in their bedrooms and other residents are having an activity in the living room. While touring the facility it was observed that the room temperature was at 71 deg F. Hot water was also tested in the bathrooms and the temperature was 110 deg F. The facility is observed to be clean, odorless, and well maintained. Residents bedrooms were observed to be well organized and fully furnished with adequate lighting. Sharps in the kitchen have a lock box accessible by code and toxic materials were observed locked. Food supply in kitchen was observed with an adequate two day perishable and seven day non-perishable. LPA toured the facility inside and outside including all of resident rooms, garage, and kitchen area. LPA observed all residents resting in their bedrooms and other residents are having an activity in the living room. While touring the facility it was observed that the room temperature was at 71 deg F. Hot water was also tested in the bathrooms and the temperature was 110 deg F. Licensing Program Analyst (LPA) Victoria Brown arrived unannounced to conduct a Required – 1 Year inspection on 10/15/23 at 3:30pm. LPA met with Haziel Romeromariano and stated the purpose of today’s visit. LPA was allowed entry into the facility that is licensed to serve a total capacity of 22 bedridden residents of which 5 may receive hospice care services. Annual Fees are current. The administrator certificate for Lorena Jose expires 4/30/24. Facility staff roster provided during this visit. There are 2 residents receiving hospice care services during this visit. The caregiver(s) are fingerprint cleared and associated to the facility. LPA toured and inspected the physical plant inside and outside to ensure there are no safety hazards to residents. The temperature inside the facility was observed to be at 70*F which is within the required range of 68-85*F. The hot water temperature was measured at 111.8 *F which is within the required range of 105-120*F. LPA observed fire extinguisher(s), pull alarm system, smoke and carbon monoxide detectors in the facility. LPA observed the centrally stored medications area to be locked and inaccessible to residents. The first aid kit contained the required items such as sterile dressings, bandages, adhesive tape, scissors, tweezers, thermometers, antiseptic solution and guide. LPA observed 2-day perishables and 7-day non-perishables. LPA observed 2 staff file and 2 resident files and conducted interviews during this visit. Upon a file review the following items were discussed to be submitted with any changes annually: Any addendums to the Infection Control Plan, Designation of Facility Responsibility (LIC308), Personnel Report (LIC500) to include the Administrator presence in the facility, Administrator Certificate-Updated, Control of Property, Liability Insurance Per California Code of Regulations (CCRs) - Title 22, Division 6, Chapter 8, no deficiencies are being cited An exit interview was conducted, a copy of the report was given.the state’s words, verbatim · CDSS document, Oct 15, 2023
