Type of visit: Required - 1 Year

Licensing Program Analyst Audrey Jeung met with manager and toured this 2-story facility, consisting of 15 ground floor and 13 second floor client rooms; all rooms have private half bathrooms and some have sliding doors that exit to the walkway or enclosed patio on the ground floor and to private balconies on the second floor. There are Arial emergency call devices in each bedroom and bathroom, so residents can summon staff for assistance. The auditory alert is transmitted to a central monitor at the front reception desk and to each caregivers' handheld device. Random client rooms were inspected, including common areas--TV/activity room, dining room, public restrooms, and 2 shower rooms. There is a front and rear patio, and 9 locked storage cabinets or sheds contain building supplies, decorations, and supplies. No accessible bodies of water or hazards were observed. Smoke detectors and carbon monoxide detectors were observed to be in working condition. Adequate lighting and comfortable indoor temperature were observed in the facility. Emergency exit routes are marked, and an emergency evacuation plan was posted. Carbon monoxide detectors are tested and operable. Food supplies are maintained. Medications are stored in locked medication room. Irene Matijas and Theresa Pamintuan (x 2/27) are certified RCFE administrators that oversee facility operations. Due to time constraints, client and staff files will be reviewed at a later time. This requirement is not met, as hot water temperature tested at 136 degrees F in shower room on ground floor. Type of visit: Required - 1 Year

On September 30, 2025, Licensing Program Analyst (LPA) Komal Curley conducted an unannounced annual inspection. LPA met with Front Desk Team/Co-managers, Ernalynn Ancheta and Theresa Pamintuna and explained the purpose of the visit. LPA toured the facility inside and outside including a random sample of resident rooms, common areas & kitchen. The indoor and outdoor passageway was free of obstruction. No accessible bodies of water of fire safety hazards observed. This is a two story facility. Resident rooms were observed to be clean with all required furniture. All resident rooms were observed to have half bathrooms. LPA observed three shower rooms, all equipped with non-skid mats. Water temperature throughout the facility measured between 110-118 degrees F. Extra linen was observed to be present. First aid kit was observed to be complete. Dining room/common area was observed free from tripping hazards. A comfortable temperature of 70 degrees F is maintained and lighting is sufficient for comfort. LPA observed two day perishables and seven day non-perishables. Extra food supply was observed to be present. Medications, sharps, and chemicals were observed locked an inaccessible to residents in care. Carbon monoxide monitors are working properly. All fire extinguishers have been checked and current as of April 2025. Emergency drills are logged and done every three month. LPA reviewed 5 resident records and 5 staff records. Resident records are updated, complete and signed. Staff records are complete, with training logs that have met the basic requirement. Medication review was done, and all medications are accounted for, and centrally stored medication records are updated. No citations are issued during the visit. Type of visit: Required - 1 Year

On September 03, 2024, Licensing Program Analysts (LPAs) Grace Donato and Kiran Jain arrived at the facility at 01:15 PM to conduct the Annual 1-year required inspection. LPAs met with Irena Matijas, Licensee/Administrator, and Therisa Pamintuan, Lead Staff, and explained the purpose of the visit. LPAs toured the physical plant. The facility is a two-story, clean, and well-maintained building. All bedrooms, living room, dining room, common area, elevator, and backyard were observed to be in good condition. No accessible bodies of water or other hazards were observed. Fire extinguishers were fully charged and last serviced on April 2024. Smoke detectors and carbon monoxide detectors were observed to be in working condition. Adequate lighting and indoor temperature readings at 75°F were observed in the common room. LPAs inspected five resident rooms and two resident bathrooms. Rooms were observed to be clean with sufficient furniture and lighting. The bathrooms were observed to be mold-free and contained liquid soap and paper towels. The bath area had a shower curtain and a shower chair. The hot water temperature in the bathroom sink faucet was measured at 107.8°F. The kitchen was observed to be clean without any knives, sharp objects, or chemicals seen accessible to persons in care. No expired food items were observed. The facility had the required 7 days of non-perishables and 2 days of perishables items. LPA reviewed five resident records and five staff records. All were observed to be complete. The client’s medications were securely stored in a locked room inaccessible to the residents. The First Aid kit was checked and found to have the required items. Updated LIC 500: Personnel Report form is requested to be submitted to CCLD by 09/06/2024. No deficiencies were cited during today's visit. An exit interview was conducted.
