Type of visit: Post Licensing
On 3/13/2026 LPA Grace Donato made an unannounced post-licensing visit to the facility. LPA met with Administrator Leila Legarda. LPA explained the purpose of the visit. LPA toured the facility inside and outside including all of resident rooms & kitchen. The indoor and outdoor passageways were free of obstruction. Carbon monoxide monitor is working properly. All fire extinguishers have been checked and current. Resident bedrooms & bathrooms were observed to be in good repair equipped with grab bars and non-skid mats. LPA reviewed 1 resident record and 1 staff record. Everything is complete. No deficiencies are cited at this time. Nov 13, 2025Facility evaluation reportReport on file
Type of visit: Prelicensing
On November 13, 2025, Licensing Program Analyst (LPA) Komal Curley conducted an announced Pre-Licensing inspection. LPA met with Administrator, Leila Legarda and explained the purpose of the visit. LPA toured facility and grounds. No accessible bodies of water or fire safety hazards observed. LPA observed the indoor and the outdoor passageways are free of obstruction. This is a single story facility with four resident bedrooms; two of which are private rooms and two of which are shared rooms. All rooms were observed to have required furniture and lighting. Two full bathrooms were observed to be clean, odor-free, equipped with grabbed bars and non-skid mats. Living room, dining room, and the enclosed deck were observed to be spacious, comfortable, and free from any tripping hazards. A comfortable temperature is maintained and lighting is sufficient for comfort throughout the facility. LPA toured the kitchen area and observed it to be clean and in good repair. Facility refrigerator temperatures are within regulatory standards. Water temperature throughout the facility was observed to be between 106-109 degrees F. First aid kit was observed in place and new. Chemical, sharps, and medication cabinets were observed to be locked. LPA observed 2-day perishables and 7-day non perishables present at the facility. All fire prevention systems such as smoke detectors, carbon monoxide detectors, fire extinguishers, and fire control panel have been inspected in place. Component III is also conducted on this day. Facility is clean and in good repair based on observations made today. Facility is in compliance with Title 22 regulations. Immediate Licensure is recommended pending final approval from the Central Applications Bureau. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed the understanding of the California Code Title 22 Regulations. Signed LIC 809 with copy of photo ID have been obtained.
