On February 13, 2026, Licensing Program Analyst (LPA) Eboni Bentley arrived unannounced for the purpose of conducting the Required 1 Year Inspection. LPA observed three old mattresses in the driveway. LPA Bentley contacted Licensee (LI) Mark Fisk by telephone at 8:16am and stated the purpose of the visit. LI stated that the facility is closed due to renovations and installation of fire sprinklers. There were two residents residing at the facility prior to the renovations. Two residents had agreed to relocate. One was placed at another facility operated by the LI and the other transferred to a different facility through the Assisted Living Waiver program. LI stated that the Department was not notified about the alterations made inside the facility and relocation of residents. At approximately 8:45am,Licensee arrive at the facility to assist with the Required 1 Year Inspection. LPA observed three old mattresses in the driveway. LPA Bentley contacted Licensee Mark Fisk arrived at the facility to assist. LPA conducted a tour of the facility and observed there were no residents/staff on site. LPA observed all floors throughout the facility were replaced and covered, and openings in the ceilings (with sprinkler piping exposed) throughout the facility were made for installation of fire suppression system. Facility is not accepting residents at this time. The annual licensing fee remains current with a balance of $0. Licensee was advised on the following: to submit a written report of any major occurrences which threatens the health, safety, and welfare of residents within 24 hours, to notify the Department when renovations or alterations being made to the facility and upon completion and prior to admitting a new resident. Based on the observations made during today's visit, a deficiency is being cited on the attached LIC9099D. 87211 Reporting Requirements (a) Each licensee shall furnish to the licensing agency such reports as the Department...: (2) Occurrences, such as..., catastrophes or major accidents which threaten the welfare, safety or health of residents, personnel or visitors, shall be reported within 24 hours...to the licensing agency and to the local health officer when appropriate. Plan of correction: Licensee stated they will provide a written incident report of the relocations and renovations, and Death Report for relocated resident by POC due date. Licensee will also submit an Acknowlegement of Understanding of the said deficiency that will be submitted to LPA via email by POC due date.

Licensing Program Analyst (LPA) Jerome Haley conducted an unannounced visit for the purpose of conducting the required one year annual inspection. LPA Haley was greeted and granted entry by staff who contacted the Licensee/Administrator (AD) via telephone. AD Fisk arrived a short time later and was present for the remainder of the visit. Administrator (AD) Mark Fisk has a current Administrators certificate that expires January 27, 2026. During the inspection, LPA Haley observed all resident bedrooms. The bedrooms had all the requirements and were in compliance with regulation guidelines. All resident bathrooms were observed. Hot water temperatures were measured in between 107.6 – 109.9 degrees Fahrenheit. No hazardous items were observed in the resident bathrooms. The kitchen was clean and organized. LPA observed a perishable and nonperishable food supply in compliance with regulation guidelines. Knives and sharp objects are kept locked in a drawer near the stove. The stove was operation and all burners, and the warmer were operational. There’s a locked filing cabinet in the kitchen with resident medications and below the locked medications is a supply of nonperishable food items. A fire extinguisher was observed fully charged and mounted on the wall behind the dining table. A Carbon monoxide detector was observed mounted on the wall near the stairs and tested operational. There’s a locked office area, a staff bedroom, and a bathroom on the second level of the facility. The second level of the facility is off limits and inaccessible to residents in care. Staff and client files are locked in the office. Continued on LIC809C There’s a locked laundry room equipped with a washer, dryer, and hazardous cleaning chemicals. The garage is used for storage and remains off limits and inaccessible to residents in care. The backyard was clean and organized. Walkways are free of obstruction. During the tour of the exterior portion of the facility, LPA Haley observed a shaded patio area with a table and chairs. The side exit gate is self-latching. Smoke detectors were observed in all resident rooms, the living room, and hallways. During the inspection all smoke detectors tested operational. The last emergency drill was conducted November 18, 2024, and are conducted quarterly for staff on each shift. During the inspection, 3 resident files were reviewed, 3 resident medications were reviewed, 2 staff files were reviewed, and 2 staff members were interviewed. Deficiencies are being cited for observations made during the inspection, record review, and a Technical Advisory will be issued. On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit for the purpose of conducting a Required Annual Inspection. LPA was greeted and granted entry by caregiving staff after introducing himself and stating the purpose of the visit. Administrator Mark Fisk were contacted by phone and arrived later to assist with the visit. During the inspection, LPA and administrator conducted a tour of the physical plant and observed the following: The facility is a two-level home with five resident bedrooms and two full bathrooms. There is a second floor with staff dwelling and an administrative office. All resident bedrooms had the required furnishings. LPA observed all beds had linens and blankets and an adequate additional supply is present. The backyard has a shaded sitting area and the route of egress is free of clutter and obstructions. There are currently six residents in care at the facility, three of which are receiving hospice care. Bathrooms faucets and toilets were operational. Water temperature tested at approximately 115F degrees. LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted. Food menu was also posted and visible. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Smoke detectors and carbon monoxide detectors tested operational. Fire extinguisher was observed to be fully charged with up-to-date maintenance. Sharps were observed locked in a drawer in the kitchen. LPA observed cleaning supplies to be stored in a locked cabinet under the kitchen sink. The laundry area is also observed to be secured with a digital lock. The medication central storage was observed to be locked. LPA reviewed six resident files and two staff files. Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations.
