Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA met with Administrator Hannah Pichon and explained the reason for the visit. Hannah Pichon's Administrator's Certificate expires on December 18, 2026. Facility is licensed for 6 non-ambulatory residents of which 6 can be bedridden and a hospice waiver for 6. The facility is a single story home. LPA and staff toured the facility. LPA observed the See Something. Say Something poster (PUB 475) posted in the main entry of the facility. The last emergency drill was conducted on May 7, 2026. The fire extinguisher in the kitchen is fully charged. The smoke detectors/carbon monoxide detectors tested operational. LPA observed in the shared bathroom in the hallway that there were 2 pairs of scissors unsecured in the vanity drawer. Hot water measured 116.2 degrees Fahrenheit in three (3) shared bathrooms. Two of the rooms have private bathrooms. LPA observed all 5 bathrooms are clean and operational. LPA observed all six 6 resident rooms have the required furnishings. LPA observed clean linens stored in the hall closet across from the shared bathroom. Medications are kept locked in the hall closet next to the dining room. LPA inspected the first aid kit. The First aid kit has all the required elements. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. LPA observed knives are kept locked under the kitchen sink. The 4 burner gas stove lights unassisted. The knobs for the stove are kept secured when it is not in use. The garage is kept locked and used for storage. LPA observed the garage is used for storage of cleaning supplies and old furniture. LPA observed a 3 day supply of emergency food and water in the garage. LPA and staff toured the backyard. No bodies of water observed. There is a covered patio with a shaded seating area for residents to sit outside. Both exit gates are operational and self closing. LPA reviewed 6 resident files, no discrepancies observed. LPA reviewed 6 resident medications. LPA observed Resident 4 (R4) did not have PRN Nystatin powder which has been prescribed and not discontinued. LPA reviewed 2 staff files. Both staff members are background cleared and associated to the facility. Both staff members have the required training and CPR/First Aid training. Facility has a dedicated internet device for resident use only. LPA consulted with the Administrator regarding reporting requirements. Deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct a proof of correction (POC) visit. LPA met with Administrator Hannah Pichon and explained the reason for the visit. LPA observed the smoke/carbon monoxide detector in the hallway between bedroom 5 and bedroom 6 has been replaced. Smoke/carbon monoxide detectors were tested and are operational. LPA observed all resident medications are stored in their original containers. The Administrator provided documentation that an emergency disaster drill was conducted on July 22, 2025. LPA observed the facility now has a 3 day supply of emergency food and water. LPA reviewed Staff 2's file. Staff 2 now has 20 hours of annual training. All deficiencies have been cleared. LPA provided Administrator with proof of correction letters for each deficiency. LPA met with Administrator Hannah Pichon and explained the reason for the visit. Hannah Pichon's Administrator's Certificate expires on December 18, 2026. Facility is licensed for 6 non-ambulatory residents of which 6 can be bedridden and a hospice waiver for 3. LPA and staff toured the facility. LPA observed all six 6 resident rooms have the required furnishings. LPA observed clean linens stored in the hall closet. LPA observed all 5 bathrooms are clean and operational. Hot water measured 120.0 degrees Fahrenheit in both (2) shared bathrooms. Medications are kept locked in the hall closet. LPA observed medication for all 6 residents, for July 21 and July 22 (AM medications only) stored in plastic cups, staff reported that the medication stored in the plastic cups are going to be administered later today and tomorrow morning July 22, 2025. LPA observed medications (for all 6 residents) for July 23 and the rest of July are stored in their original containers. LPA inspected the first aid kit. The First aid kit has all the required elements. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. LPA observed knives are kept locked under the kitchen sink. The 4 burner gas stove lights unassisted. There is no record of a recent emergency drill being conducted in the last 4 months. Smoke detectors/carbon monoxide detectors tested operational except for the smoke detector/carbon monoxide detector in the hallway between bedroom 5 and bedroom 6. LPA observed the garage is used for storage of cleaning supplies and old furniture. LPA observed the lock on the garage door knob is not operational. LPA observed and staff verified there is no 3 day emergency food and water supply at the facility. LPA and staff toured the backyard. No bodies of water observed. There is a covered patio with a shaded seating area for residents to sit outside. Both exit gates are operational and self closing. LPA reviewed 6 resident files, no discrepancies observed. LPA reviewed 6 resident medications, no discrepancies observed. LPA reviewed 2 staff files. LPA observed Staff 2 (S2) did not have any annual training covering, hospice care, postural supports and restricted health conditions. No obstacles or hazards observed inside or outside of the facility. Deficiencies are being cited (see LIC809Ds) per Title 22, Division 6 of the California Code of Regulations. Facility is a one story home with 6 bedrooms, 5 bathrooms, living room, kitchen, dining room, 2 staff rooms and a 2 car garage. Facility is licensed for 6 non-ambulatory residents of which 6 can be bed ridden and a hospice waiver for 3. There is a screened fireplace in bedroom 1. LPA observed the See Something, Say Something Poster (PUB 475) posted in the main entry way of the facility. LPA and staff toured the facility. LPA observed all resident bedrooms had the required furnishings and bed linens. LPA observed the kitchen is clean and organized. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. Knives are kept locked in a kitchen drawer. LPA observed the 4 burner gas stove lights unassisted. LPA observed the fire extinguisher in the kitchen is fully charged. LPA observed medications are kept locked in the hall closet. All 5 bathrooms are clean and operational. Hot water measured 108.0 to 110.0 degrees Fahrenheit in all 5 bathrooms. LPA observed a clean supply of linens and towels in the hall closet. LPA and staff toured the garage. The garage is kept locked. Cleaning supplies and emergency food and water are stored in the garage. LPA and staff toured the backyard. There is a covered patio that provides shaded seating. No bodies of water observed. The exit gates on each side of the house are operational. Smoke detectors/carbon monoxide detectors tested operational. Facility has a tablet that can connect to the internet for resident use. The last facility fire drill took place June 1, 2024. Administrator Fez Jon left the facility during the visit due to an previously scheduled appointment. LPA reviewed 2 staff files. LPA observed Staff 1 (S1) did not have the required 20 hours of annual training. Both staff files reviewed had current CPR training. LPA reviewed 6 resident files, no discrepancies observed. LPA reviewed 6 resident medications, no discrepancies observed. LPA inspected the first aid kit. The first aid kit has all required elements. Deficiencies are being cited per Title 22 Division 6 of the California Code or Regulations.
