There is a medication area in the middle of the wings. There are common areas at the end of each wing and at the entrance. Type of visit: Case Management - Deficiencies

LPA Hiratsuka conducted this unannounced case management visit in response to several residents leaving without supervision. The occurrences were 09/09/2024, and LPA was informed the resident is returning this evening; 08/23/2024, and returned; 05/14/2024, resident returned,\; one resident who left twice in May 2024, and one who left in April 2024. All residents were deemed to not be able to leave the facility unassisted by their physicians. The residents were not injured and they all returned. Most of the residents here have high functioning mental capabilities and are all physically ambulatory. From the amount of residents who were able to leave the facility without notice and without permission it has been determined the facility shall submit a written plan to address this issue. CA Health and Safety Code 1569.312 (e) states, "Every facility required to be licensed under this chapter shall provide at least the following basic services; "Monitoring the activities of the residents while they are under the supervision of the facility to ensure their general health, safety, and well-being." Because multiple residents have made it out of the facility without anyone noticing, the following deficiency was cited. The following deficiency is cited on LIC 809D and cited from the California Health and Safety Code. Failure to correct the deficiency may result in civil penalties. Allegation investigated: Facility air conditioner in disrepair and not being addressed

LPA Hiratsuka, conducted this unannounced complaint visit. LPA toured the facility, interviewed two staff, and interviewed Administrator Angie Karis. This facility has two wings for residents and the wings meet in the middle where there is a central enclosed area for staff. The central enclosed area for staff has an office, staff lounge, medication/charting area, and linen closet. This is the area that has a broken motor for the air conditioner. The air conditioner broke on Monday, June 24, 2024, a repair company was called the same day and came on Tuesday, June 25, 2024 and a part was ordered for the unit. An update for the part that was ordered should tentatively be coming in five to ten days and that should include installation. If there is a delay the facility does have fans and has other accommodations. The resident areas are not affected. LPA was also informed the air conditioner and heater units are checked quarterly by the facility maintenance team. Unfounded LPA toured the facility and observed fans blowing in the areas affected. There fans are placed to not be tripping hazards that are blowing cooler air into the affected areas. The facility always has hydration stations for everyone. Staff have been allowed to take breaks in other areas of the facility to avoid being in the staff lounge. Medications that have to be kept cold are stored in a locked refrigerator and any medications that require specific temperatures have been checked and stored in appropriate areas. The resident wings and rooms are not affected. Because there was no warning for the air conditioner dying, the repair service being called the day the air conditioner died, the facility placing fans in cooler areas to blow air into the warm areas to try to cool the affected areas, always having hydration stations, and medication storage adjustments if required have been done, the allegation is UNFOUNDED. A finding that an allegation is UNFOUNDED means that the allegation is false, could not have happened, and/or is without a reasonable basis. No deficiencies are cited at this visit. Type of visit: Case Management - Incident

LPA Hiratsuka conducted this unannounced case management visit. This is in response to one resident who physically attacked another resident and the staff on duty 06/15/2024. LPA interviewed Administrator Angie Karis. The resident and staff who were assaulted had minor injuries and are doing well. The resident who attacked the others was removed from the facility by the police, taken to the hospital, and discharged by the conservator. LPA was here to get information about staff supervision and did not observe any lack of supervision on the part of the staff. Allegation investigated: Staff is overmedicating resident in care. Staff confiscated resident's inhaler.

On 5/20/2024 LPA Tryon visited the facility to complete the complaint. LPA met with Angie Karis. LPA has spoken with resident involved, has spoken with staff, reviewed medical and medication records. Regarding the allegation that staff is overmedicating resident in care, through the team care process, it had been decided that an increase in one particular medication may be beneficial to the resident. Doctor had ordered an increase, and medication was given as prescribed by physician. By all accounts the resident was doing well, and the medication had obviously been effective. Since the medication was appropriately precribed through the team process, and medications were given as per order, LPA finds that the allegation that the staff is overmedicating resident is care is UNFOUNDED. Regarding the allegation that the staff confiscated the resident's inhaler, it was learned that staff had informed the doctor that the resident had made a statement about misusing the medication; and therefore the doctor had discontinued it. Resident still had a regular inhaler, but it was a "rescue" inhaler that was discontinued. Resident stated that she felt the rescue inhaler was still needed; a follow-up doctor appointment was scheduled; and the doctor re-instated the medication. This took place in January 2024. Unfounded Therefore, the inhaler was not "confiscated" by the staff, but rather discontinued by the doctor; staff was following doctor orders. When the resident complained that she was having issues breathing, a doctor appointment was scheduled and it was re-instated. Therefore, the allegation is UNFOUNDED. A finding that an allegation is UNFOUNDED means that the allegation is false, could not have happened, and/or is without a reasonable basis. No deficiencies are cited at this visit. Type of visit: Case Management - Incident

LPA Hiratsuka conducted this unannounced case management visit in response to a resident who has left the facility twice and is deemed by a physician not able to leave the facility without assistance. Both times facility followed their protocol for elopements. LPA toured the facility with Administrator Angie Karis. The first time the resident left the resident was not deemed an elopement risk. When the resident was returned to the facility the resident was put on fifteen minute checks. The resident left the facility shortly after one of the fifteen minute checks. The resident was found the second time by facility staff and was brought back to the facility. The resident now has a one-on-one staff. However, the facility staff are not allowed to physically restrain someone from leaving the facility without assistance and if the resident shows signs of wanting to leave and scales the perimeter fence the staff shall call the local police department to report the resident leaving. Administrator is currently in touch with responsible party for appropriate placement. Further investigation is required.
