Allegation investigated: Staff do not prevent bedbug infestation Staff do not prevent scabies infestation Staff do not report unusual incidents to residents' authorized representatives
Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to deliver the findings of the allegations cited above. LPA met with Executive Director and explained the purpose of the visit. During this course of this investigation, LPA conducted file reviews and interviews. The result of the investigation is as follow. For the allegation of Staff do not prevent bedbug infestation, file review revealed that pest control has treated the facility for bedbugs on a couple of occasion since the sighting of "a bug". Interview reveals that actual bedbug sightings were not observed, and residents in the room did not have any bites, but in precaution, residents were relocated out of the room for treatments to be conducted. There are no reports of any biting and/or bedbug activity in any other area in the facility. Invoices reviewed revealed that treatment was conducted in June 2026 and August 2026. Therefore the allegation is unfounded. Please continue on LIC 9099-C. Unfounded LIC 9099-C For the allegation of Staff do not prevent scabies infestation, interview revealed that only one resident (R1) in care was diagnosed with scabies. R1 enjoyed keeping to themselves and did not socialized much with other residents in care. Close observations of the community was conducted and no other residents in care reported of a rash. Incident report review revealed that R1 was diagnosed with scabies at the hospital and was discharged to the community with treatment. R1 was isolated and placed in quarantine in their room. Medical hospital paperwork revealed that R1 has since been medically cleared of scabies. The allegation is unfounded. Allegation of Staff do not report unusual incidents to residents' authorized representatives, interviews were conducted. Interview with authorized representative of the resident with alleged bedbugs (R2), it revealed facility had informed them on resident relocation and pest control services. In interview conducted with authorized representative of the resident with scabies (R1) revealed that they are aware of the resident having scabies and that treatment was completed. Both R1 and R2's authorized representatives stated "Nate is great" and reported no issues with the facility in regards to communication. The allegation is unfounded. A finding of unfounded means that the allegation is false, could not have happened and/or is without a reasonable basis. Allegation investigated: Staff do not respond to resident's call for assistance in a timely manner
Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to complete investigation into allegation listed above. LPA met with assistant administrator Anelisa Fusneica during today's inspection. LPA investigated allegation, "Staff do not respond to resident's call for assistance in a timely manner." LPA interviewed staff, residents, and reviewed facility documentation. Relevant party stated R1 is waiting up 2 hours to be transfer to and from bed to wheelchair or to receive continence care. LPA interviewed R1 in which they stated they are waiting 40 or more minutes to get help from caregivers. R1 stated that in July 2026 a caregiver never answered their call light and R1 missed lunch and remained in bed until the PM shift. LPA reviewed R1's call light log from May 2026 to July 2026 and observed R1 waited over 40 minutes 51 times. LPA interviewed caregivers in which they stated they are having a hard time meeting residents care needs with their current staffing levels. 87411 Personnel Requirements - General. (a) Facility personnel shall at all times be sufficient in numbers, and competent to provide the services necessary to meet resident needs. In facilities licensed for sixteen or more, sufficient support staff shall be employed to ensure provision of personal assistance and care as required in Section 87608, Postural Supports. Additional staff shall be employed as necessary to perform office work, cooking, house cleaning, laundering, and maintenance of buildings, equipment and grounds. The licensing agency may require any facility to provide additional staff whenever it determines through documentation that the needs of the particular residents, the extent of services provided, or the physical arrangements of the facility require such additional staff for the provision of adequate services. Allegation investigated: Facility staff neglect resulting in wound deterioration Facility staff are not meeting resident's incontinence care needs
Licensing Program Analyst (LPA) Graham Gunby arrived unannounced on 04/28/2026 to complete and deliver findings to a complaint received on 03/02/2026. LPA met with Executive Director, Nathan Condie and explained the purpose of the visit. Throughout the course of the investigation, the department conducted interviews and record reviews. Please continue to LIC9099C.. Unfounded Allegation: Facility staff neglect resulting in wound deterioration During the course of the investigation, interviews were conducted as well as records reviewed. Interviews indicated that R1 was transferred from a Skilled Nursing Facility with a pressure injury near their lower back. S1 and F1 both confirmed R1 was receiving care from a home health agency that lacked communication. With the help of the facility, R1 transferred to a new home health agency that is able to provide the necessary care needed for R1. R1 confirmed their health has improved while at the facility and has no problems with the care they are currently receiving from the facility or home health. Allegation: Facility staff are not meeting resident's incontinence care needs An interview conducted with R1 revealed R1 does get assistance whenever needed. R1 emphasizes the satisfaction of the care staff and feels safe living at the facility. Interview conducted with F1 revealed that R1 feels comfortable at the facility and gets assistance with changing and toileting. R1 denied any lack of care and supervision. F1 and F2 expressed their concern for the previous home health agency as there was little to no communication, but is happy with the new agency. Interview conducted with F2 revealed that the care at the facility is amazing and that they receive full assistance with activities of daily living. This agency has investigated the above complaint allegations. We have found that the complaint is UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. We have therefore dismissed the complaint. Type of visit: Prelicensing
On September 30, 2025, Licensing Program Analyst (LPA) Lavinia Muscan met with administrator, Sheri Kimbro, to conduct a Pre- Licensing visit. Administrator holds a current administrator certificate (#7014585740 with expiration date 01/26/2026). LPA toured the facility with Administrator. LPA toured resident bedrooms and observed that they were properly furnished and maintained. LPA toured common bathrooms and observed the bathrooms to be clean and sanitary. LPA toured kitchen, common areas, and outside areas. Smoke detectors are operational. There are carbon monoxide detectors which are functioning. The Fire extinguisher was charged, serviced and functional. All exits were unobstructed. All toxins, medications, and sharps were locked and stored away. Component III was waived as the current administrator is an administrator for an RCFE Facility licensed by CCLD . LPA will forward findings to the Centralized Application Bureau (CAB) that facility met all the pre-licensing components. Applicant has satisfied all requirements in accordance to Title 22, California Code of Regulations on today's pre-licensing inspection. A copy of this report was provided to the facility.
