Licensing Program Analyst Hiratsuka, conducted this unannounced case management visit. The beginning of May 2026, LPA called and requested a copy of the facility's initial application that they submitted in June 2020. LPA emailed the request to Ms. Mireles several times who stated she was informed each time the paperwork was found and going to be sent over. As of today LPA has not received the paperwork.. The licensee has until June 5, 2026, to submit the paperwork to Community Care Licensing Division. Failure to do so shall result in a deficiency being cited. Community Care Licensing Division (CCLD) conducted the investigation into the allegations above. The allegation was denied to have happened by those involved and they said they would report abuse if anything did occur. Based on information above, the department concluded that the allegations are Unfounded. LPA Hiratsuka, conducted this unannounced annual visit. LPA toured with Administrator in Training Gurpreet Kaur. The main entrance opens to the main hallway. The administration offices are to the right of the main entrance. This facility has a hallway on the left that leads to the kitchen, main dining room, and leads back to another hallway. There are two hallways on the right that are not connected to each other. The second hallway in the back that goes to the right leads to the activity area. There are three short hallways that lead off from the very back hallways with the ends leading to the outside. There are two outdoor areas and they are both enclosed. This facility has 59 resident rooms; three have full private bathrooms and the rest have half private bathrooms. There is a medication room to the right past the administration offices. Multiple topics discussed. Several resident records were reviewed Several staff files were reviewed. The following shall be updated and submitted to licensing within 30 days; LIC 500- facility personnel or staff schedule LIC 308- designation of administrative responsibility LIC 610- emergency disaster plan LPA did not observe any issues of concern during tour and review of resident files. Type of visit: Case Management - Deficiencies
Licensing Program Analyst (LPA) Hiratsuka, conducted this unannounced case management visit in response to a resident who left the facility without being noticed on July 19, 2025, and not noticed gone until the early morning of July 20, 2025. Assistant Administrator (AA) stated they were able to determine the resident left out the front doors around 7:30pm on July 19, 2025, and the staff didn't notice until about 1:30am. The local sheriff's office was contacted and a search was conducted and the resident was found a couple of blocks away. The resident was sent to the hospital and has returned without ill effects. AA stated the staff did not conduct the checks they are assigned to do for residents. AA stated there are now elopement drills for the staff over the next thirty days, the staff are to monitor and document they have checked on residents, and there are going to be further instructions based on resident needs to be given to staff. Based on the above, the following citation was issued today from the California Health and Safety Code. Deficiencies cited from Title 22 Regulations and or the California Health and Safety Code. Licensing Program Analyst (LPA) K. Hiratsuka conducted this visit to complete the complaint. LPA met with Assistant Administrator Ivy Garner. LPA interviewed the resident in question, staff, and reviewed the resident's file. The item in question is a bottom set of dentures. LPA observed the resident wearing the top ones and moving it around with their tongue during an interview. The resident in question does have a diagnosis of dementia and it is difficult to determine if the resident is capable to know when they take out the dentures and where they put it. LPA also cannot determine how much the resident comprehends regarding keeping the dentures clean. Staff stated the resident is able to clean them with assistance and that the dentures are put in a solution when not in use per the instructions. LPA cannot determine how much cleaning the dentures require. Unsubstantiated Interview with staff also stated the resident had a tendency to put on dirty clothes before the staff were able to take the dirty clothes away. LPA observed clean clothes in the resident room. The resident has the right to wear what they want and the facility staff needs to try to work with the resident to ensure the resident wears clean clothes. Therefore, LPA finds the allegation to be "unsubstantiated." A finding of unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the violation occurred. Licensing Program Analyst (LPA) K. Hiratsuka conducted this visit to complete the complaint. LPA met with Assistant Administrator Ivy Garner LPA has spoken with residents, staff, reviewed documentation. Title 22 regulations requires an alert system for residents to request assistance. The regulations do not state a specific time frame the calls for assistance have to be answered. LPA was unable to determine if the resident was able to articulate time. Staff stated there were times they were notified by a witness the resident used the call button and the resident did not. LPA interviewed a couple of residents and the response times varied. Staff stated they do frequent checks as well as respond to call lights within ten minutes. The facility is not required to have a call system log and this facility does not. LPA cannot determine whether staff responded to the resident alerts or not or in what time frame. Unsubstantiated Through interviews the resident did get up on their own instead of waiting for staff assistance without using the call button to request assistance. LPA cannot determine if the resident used the call button to request assistance or not. Staff stated they do frequent checks of the residents. LPA cannot prove or disprove because each side has their own version of events. Therefore, LPA finds the allegation to be "unsubstantiated." A finding of unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the violation occurred. Regarding the allegation that staff are mistreating residents, LPA has interviewed staff involved with the incident mentioned in the complaint, spoke with residents involved and briefly with other residents. At this time, there is no proof that the staff actually mistreated the residents involved. One resident did fall when staff tried to open the bedroom door, but it's not completely clear if staff pushed the door hard enough to push the resident; or if she just lost her balance and fell. Stories related by staff differ from story related by residents involved. LPA has spoken with other residents and they have not seen any staff mistreat other residents or themselves, and related that staff is helpful and respectful. At this time, since stories differ from one witness to another, LPA is not able to say with certainty that staff in any way "mistreated" residents. Therefore, LPA finds the allegation to be "unsubstantiated." A finding of unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the violation occurred. Type of visit: Required - 1 Year
LPA Hiratsuka, conducted this unannounced annual visit. LPA wore a surgical mask and observed all staff wearing surgical masks. LPA was also screened by the front desk. The main entrance opens to the main hallway. The administration offices are to the right of the main entrance. This facility has a hallway on the left that leads to the kitchen, main dining room, and leads back to another hallway. There are two hallways on the right that are not connected to each other. The second hallway in the back that goes to the right leads to the activity area. There are three short hallways that lead off from the very back hallways with the ends leading to the outside. There are two outdoor areas and they are both enclosed. This facility has 59 resident rooms; three have full private bathrooms and the rest have half private bathrooms. There is a medication room to the right past the administration offices. Multiple topics discussed. Several resident records were reviewed Several staff files were reviewed.
