Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Graham Gunby arrived on Wednesday May 20, 2026, to conduct the unannounced annual inspection. LPA met with the Executive Director, Neil Torres, and explained the purpose of this visit. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed five (5) resident and three (3) files. All resident files contained the required paperwork. All staff files contained the required paperwork and training. LPA and ED toured the facility together to ensure the health and safety of residents in care. The areas toured included the following areas: resident apartments, resident bathrooms, common areas, kitchen, laundry room, and medication rooms. Facility had fire extinguishers inspected on 12/29/2025. Facility was current on fire drills. Facility had fully stocked first aid kits. All required postings were observed. Facility was clean and well organized. In the areas toured, there were no health or safety violations observed. No deficiencies cited. Exit interview conducted. Allegation investigated: Staff are not following universal precautions to prevent the spread of scabies
On 03/13/2026, Licensing Program Analyst (LPA) Graham Gunby arrived at the facility unannounced to deliver complaint findings into the allegation listed above and met with Executive Director, Neil Torres. Based on observation, record review, and statement reviewed, the facility was following infection control requirements. Public Health was contacted and they stated since it was only one client, they were not going to get involved. The administrator did submit an incident report to the department. As a precaution, during the first sign of a rash, facility puts out PPE outside the resident room, notifies staff of the potential of scabies, and an in-service to staff is reviewed on proper handwashing and universal precautions. Facility encouraged residents to stay in their room during the episode. It was observed facility had required PPE outside the resident room; therefore, the allegation is UNFOUNDED. A finding of unfounded means that the allegation is false, could not have happened and/or is without a reasonable basis. Exit interview conducted. Report left with facility. Licensing Program Analyst (LPA) Graham Gunby conducted a visit at Summerfield of Roseville and met with Executive Director (ED), Heidi Charette, and explained the purpose of the visit. Based on record reviewed it has been determined that Licensee has an overdue license fee. This facility has an outstanding balance for fees due. The current amount owed is $10,860.00 and was billed on 02/05/2025. As of this date, the fees are overdue. LPA explained to ED that proof of payment and/or proof of payment plan needs to be provided to CCL by 11/28/2025 . Allegation investigated: Resident needs are not being met due to insufficient staffing
On 08/26/2025, Licensing Program Analyst (LPA) Lavinia Muscan arrived at the facility unannounced to deliver complaint findings into the allegations listed above and met with Administrator May Ramos. During the investigation, the Department conducted interviews and reviewed documentation pertinent to the investigation. The results of the investigation are as follows: The department has reviewed facility schedules and conducted interviews. Documents reviewed showed that shifts are covered by multiple staff. There is no evidence to support the allegation that needs are not being met due to staffing. The department learned that there are four (4) direct care staff and one (1) Med Tech available during each shift. The department finds no evidence that the current staff level is insufficient. Therefore, the allegation is UNFOUNDED. A finding of unfounded means that the allegation is false, could not have happened and/or is without a reasonable basis. Exit interview conducted. Allegation investigated: Resident sustained multiple unwitnessed falls due to staff neglect
On May 7, 2025 Licensing Program Analysts (LPAs) Lavinia Muscan and Talwinder Bains arrived at the facility unannounced to deliver complaint findings into the allegations listed above and met with Jane Scaparro - Marketing Director. During the investigation, the Department conducted interviews and reviewed documentation pertinent to the investigation. The results of the investigation are as follows: **Report continued on 9099-C** Substantiated ***Report continued from 9099.... Allegation- Resident sustained multiple unwitnessed falls due to staff neglect Based on documentation, between 03/12/2024 and 01/14/2025, R1 sustained nine unwitnessed falls in R1’s bedroom and one witnessed fall in the common area. Based on interviews, neither R1’s responsible party, or facility staff recommended a change in R1’s care plan or provided R1 with some type of ambulatory aid to possibly prevent R1 from falling. R1’s responsible party provided R1 a portable bed rail; however, R1 would remove the rail from the bed. Additionally, interviews revealed that neither R1’s responsible party nor facility staff recommended the use of an ambulatory aid because R1 would have “refused” to use it. Summerfield of Roseville has four neighborhoods on the premises: Garden, Tuscan, Apple, and Seaside. Seaside is a neighborhood for residents who need a higher level of care. Facility staff explained the residents living in Seaside can be considered fall-risks, may be wheelchair bound or receiving hospice services and require two-staff assistance. It was not recommended by facility staff that R1 move into this neighborhood for the higher level of care and supervision until 01/15/2025. Based on the findings, staff were neglectful in preventing R1 from sustaining falls while in care, therefore, the preponderance of evidence standard has been met, this allegation of Neglect/Lack of Care and Supervision is SUBSTANTIATED. Per California Code of Regulations, Title 22 Division 6, Chapter 8, a deficiency is being cited on the attached 9099-D page. Exit interview conducted. Appeal rights provided. Report left with facility staff. Resident sustained a fracture due to staff neglect Based on records reviewed, on 01/04/2025, around 1100 hours, R1 sustained an unwitnessed fall in R1’s bedroom. S1 located R1 on the floor next to R1’s bed. R1 complained of having back pain. S2 called emergency services and R1 was transported to the hospital by ambulance. Per medical records, R1 arrived at the hospital on 01/04/2025, at 1115 hours. X-Ray images revealed that R1 had sustained an acute fracture of the transverse process of vertebra L1. Based on staff interviews and employee timesheets, R1 was last checked on by S1 on 01/04/2025, around 1000 hours, before S1 clocked out for S1’s lunch break at 1001 hours. Upon S1 returning from S1’s lunch at 1033 hours, S1 conducted another check on S1’s assigned residents, to prepare them for their lunch at 1130 hours. Considering the approximate one hour and fifteen-minute timeframe between R1 being checked on and R1 arriving at the hospital, I determined staff’s supervision of R1 was appropriate, and staff were not neglectful in their care; therefore, this allegation of Neglect/Lack of Care and Supervision is UNSUBSTANTIATED. A finding that the complaint is UNSUBSTANTIATED means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Observation of the Resident - The licensee shall ensure that residents are regularly observed for changes in physical, mental, emotional and social functioning and that appropriate assistance is provided when such observation reveals unmet needs. When changes such as unusual weight gains or losses or deterioration of mental ability or a physical health condition are observed, the licensee shall ensure that such changes are documented and brought to the attention of the resident's physician and the resident's responsible person, if any. This requirement was not met based on facility failed to reassess R1 after resident sustained multiple falls. R1 sustained 9 falls between 03/12/24 and 01/14/25. Licensing Program Analysts (LPAs) Lavinia Muscan and Talwinder Bains arrived on 05/07/25 to conduct the annual inspection. LPAs met with Marketing Director, Jane Scaparro and explained the purpose of the visit. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPAs reviewed five (5) residents, five (5) staff files and found them to be complete. All the required postings were observed. Water temperatures during visit was within the required range of temperature. LPAs observed the facility's emergency food and water storage. LPAs observed fire extinguisher is ready for emergency use. In the areas toured, there were no health or safety violations observed. LPAs reviewed fire drills. LPAs requested the facility to send updated LIC500, LIC610E and current liability insurance to be sent to the Department by 05/31/25. No deficiencies were observed or cited per Title 22 Regulations during this visit. Type of visit: Case Management - Other
Licensing Program Analyst (LPA) Cassie Yang conducted unannounced case management visit on 09/13/2024. This visit is to confirm ORDERS TO INDIVIDUAL FOR IMMEDIATE EXCLUSION FROM ALL FACILITIES. LPA met with med tech, and additionally LPA spoke with Marketing Director, Jane Scaparro, and stated the purpose of visit. Facility understands this is an Immediate Exclusion effective 09/13/2024 and S1 is excluded and cannot be allowed to work, live in, and/or have contact with clients in any residential facility licensed by the California Department of Social Services. Therefore, the Department orders the facility to remove S1 from any contact with clients and not allow this employee to be physically present in the facility. Type of visit: Required - 1 Year
Licensing Program Analysts (LPAs) Lavinia Muscan and Talwinder Bains arrived on 4/24/24 to conduct the annual inspection. LPAs met with Administrator May Tate and explained the purpose of the visit. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPAs reviewed resident (5) and staff (5) files. All the required postings were observed. LPAs and Administrator May Tate toured the facility together to ensure the health and safety of residents in care. The areas toured included kitchen, hallways, memory care apartments, memory care dining room/kitchen, and memory care common areas. Water temperatures in the apartments toured were within the required range of temperature. LPAs observed the facility's emergency food and water storage. LPAs observed fire extinguisher is ready for emergency use. In the areas toured, there were no health or safety violations observed. LPAs reviewed fire drills. LPAs requested the facility to send updated LIC500, LIC610E and current liability insurance to be sent to the Department by 5/15/24. Deficiencies were observed per Title 22 regulations from today's visit as indicated on 809-D. Exit interview conducted.
