Allegation investigated: Staff does not safeguard resident's property. Staff does not adequately supervise residents, resulting in residents going into other resident's rooms. Staff does not ensure that the residents have a way to contact staff for assistance or in an emergency. Staff does not ensure that resident's room was at a comfortable temperature. Staff does not ensure that resident's clothing was clean and free of stains. Staff does not frequently wash resident's clothing.
On 12/05/2025 at 10:20AM Licensing Program Analyst (LPA) Renese Howell-Small conducted an unannounced visit to the facility in order to deliver findings for the above allegations. LPA discussed the purpose of the visit with Executive Director, Deborah Staggs. The investigation consisted of interviews, observation and record review. Resident 1 (R1) passed away on 11/10/2023. In regards to the allegation of staff does not safeguard resident's property: At the request of family, a lock was installed on R1's room door. Staff denied the allegation and stated that if resident items are taken by other residents, those items are returned. Based on interviews and record review, this allegation is UNSUBSTANTIATED. In regards to the allegation of staff does not adequately supervise residents, resulting in residents going into other resident's rooms: Staff stated that residents are checked on hourly. The resident that was wandering was relocated. The Admission's Agreement states that there is a risk of residents wandering. Based upon interviews and record review, this allegation is UNSUBSTANTIATED. Unsubstantiated In regards to the allegation of staff does not ensure that the residents have a way to contact staff for assistance or in an emergency: LPA observed that there were pull cords mounted to the wall in the resident's rooms. Staff stated that some of the residents also have safety pendants and are checked on hourly. Based on observation and interview, this allegation is UNSUBSTANTIATED. In regards to the allegation of staff does not ensure that resident's room was at a comfortable temperature: LPA observed the hallway thermometer to read 70 degrees Fahrenheit. LPA observed two (2) operable fire places. Residents in care appeared to be content, comfortable and wearing appropriate clothing. Staff stated that they will assist residents with securing blankets or sweaters and will adjust the overall temperature when needed. Based upon observation and interview, this allegation is UNSUBSTANTIATED. In regards to the allegation of staff does not enure that resident's clothing was clean and free of stains: LPA observed an operational washer and dryer in the laundry room. LPA toured resident bedrooms and clean laundry was observed in closets. Staff stated that laundry is completed several times during each shift. Based upon observation and interview, this allegation is UNSUBSTANTIATED. In regards to the allegation of staff does not frequently wash resident's clothing: Staff stated that laundry is completed when a resident has an accident, after eating, after showering/grooming and other times as needed. Based upon interview, this allegation is UNSUBSTANTIATED. UNSUBSTANTIATED is defined as the allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation(s) occurred. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Michelle Echeverria made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA met with Receptionist, Candice Tapia and was granted entry to the facility. Executive Director, Debbie Staggs later arrived and LPA introduced self and stated the purpose of the visit. The facility is a Residential Care Facility for Elderly (RCFE) licensed capacity for (70) nonambulatory, hospice waiver for (35), bedridden for (35 out of the 70), fire clearance for delayed egress and locked perimeters and advertising in dementia care. The facility has (3) wings, (39) bedrooms, bathrooms, reception area, living rooms, tv rooms, kitchen, laundry room, staff rooms, offices and shaded patios. LPA was accompanied by Executive Director, Debbie Staggs and Director of Resident Services, Lureen Trujillo to conduct a general overall inspection, which included, but was not limited to the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at comfortable temperatures 73, 72, 74, 73, 72, 72, 71 and 71 degrees Fahrenheit. Water temperatures measured at 113, 111, 107, 112, 107 and 111 degrees Fahrenheit. LPA inspected resident bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. The facility is equipped with operating fire extinguishers, smoke detectors, signal alarms and carbon monoxide alarms. Posters such as personal rights, CCL complaint poster, CCL license, ombudsman, and facility sketch were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. There was a designated storage space for residents/staff files. Medications were kept inaccessible to residents. Overall, the facility is clean, in good repair, and operating in safe conditions for residents in care. Food Service: Non-perishable and perishable food supply is sufficient for number of residents in care. Pesticides and other toxin's are not stored in food areas. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. All staff members working in the facility have criminal record clearance through the department. Record Review: LPA reviewed residents files for admission agreements, physician reports, and needs and services plans. LPA also reviewed staff files for First Aid/CPR certification, criminal record clearance, trainings, and health screenings. Medication was audited at random and appeared to be dispensed appropriately by staff. LPA reviewed facility's file for emergency drills, emergency disaster plan, annual fire inspection, infection control plan, and insurance policy. LPA observed that the facility did not have emergency kits assembled in order to shelter in place during a disaster. Technical violation issued. No deficiencies and one technical violation was cited during this visit. LPA met with Director of Community Relations, Jennifer Bohner and was granted entry to the facility. The facility is a Residential Care Facility for Elderly (RCFE) licensed capacity for (70) nonambulatory, hospice waiver for (25), bedridden for (35), fire clearance for delayed egress and locked perimeters and advertising in dementia care. The facility has (3) wings, (39) bedrooms, bathrooms, reception area, living rooms, tv rooms, kitchen, laundry room, staff rooms, offices and shaded patios. LPA was accompanied by Jennifer to conduct a general overall inspection, which included, but was not limited to the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at comfortable temperatures 75, 74, 71, 74, and 76 degrees fahrenheit. Water temperatures measured at 105, 109, 107.4, and 105.9 degrees fahrenheit. LPA inspected resident bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. The facility is equipped with operating fire extinguishers, smoke detectors, signal alarms and carbon monoxide alarms. Posters such as personal rights, CCL complaint poster, CCL license, ombudsman, and facility sketch were posted in a common area. LPA met with Director of Resident Services, Shelia Orozco and was granted entry to the facility. The facility is a Residential Care Facility for Elderly (RCFE) licensed capacity for (70) nonambulatory, hospice waiver for (15), bedridden for (35), fire clearance for delayed egress and locked perimeters and advertising in dementia care. The facility has (3) wings, (39) bedrooms, bathrooms, reception area, living rooms, tv rooms, kitchen, laundry room, staff rooms, offices and shaded patios. LPA was accompanied by Shelia to conduct a general overall inspection, which included, but was not limited to the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature 74, 73, 75, 68.2, 73, 72 and 75 degrees fahrenheit. Water temperature measured at 109.1, 106.8, 105, 105.4 and 105.2 degrees fahrenheit. LPA inspected resident bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. The facility is equipped with operating fire extinguishers, smoke detectors, signal alarms and carbon monoxide alarms. Posters such as personal rights, CCL complaint poster, CCL license, ombudsman, and facility sketch were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. There was a designated storage space for residents/staff files. Medications were kept inaccessible to residents. Overall, the facility is clean, in good repair, and operating in safe conditions for residents in care. Food Service: Non-perishable and perishable food supply is sufficient for number of residents in care. Pesticides and other toxin's are not stored in food areas. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. All staff members working in the facility have criminal record clearance through the department. Record Review: LPA reviewed (4) residents files for admission agreements, physician reports, and needs and services plans. LPA also reviewed (4) staff files for First Aid/CPR certification, criminal record clearance, trainings, and health screenings. Medications were not audited due to time management. No deficiencies were cited during this visit. Type of visit: Case Management - Health Checks
Licensing Program Analysts (LPA) Paola Guerrero conducted an unannounced visit to the facility on 11/9/2023 at 11:14 AM for the purpose of a Health & Safety check. LPA Guerrero identified herself to Director of Resident Services Shelia Orozco and discussed the purpose of the visit. Residents in care were present during visit. No imminent health and/or safety concerns observed at the time of visit. LPA Guerrero observed no health and/or safety hazards inside the facility. LPA Guerrero inspected the outside perimeter of the facility and observed no health and/or safety hazards. LPA Guerrero observed sufficient staff present at the facility to provide care. LPA Guerrero inspected facility food supplies and observed three (3) day supply of perishable and seven days (7) supply of non-perishable food. The needs of the residents in care appear to be met during this inspection.
