Allegation investigated: Staff did not provide 90-day notice prior to rent increase.
On 05/04/26 Licensing Program Analyst (LPA) Mario Leon conducted an initial, unannounced, complaint visit at the facility. California Department of Social Services (CDSS) was met by staff one, Stephen Gradney - Licensee (S1), and the purpose of the visit was explained. S1 and CDSS toured the facility. The investigation consisted of the following: On 05/04/26 CDSS requested and reviewed facility documents and toured the facility. CDSS interviewed four (4) out of four (4) residents (R1-R4) and three (3) out of seven (7) staff. The investigation revealed the following: Regarding the allegation, “Staff did not provide 90-day notice prior to rent increase.", it is being alleged that a resident received a 60-day notice of rent increase. Report continues, please see LIC9099-C. Substantiated Between 09:00AM and 11:00AM, CDSS interviewed (4) residents (R1-R4) and three (3) staff (S1-S3). Interviews revealed that one (1) out of four (4) residents (R1) and two (2) out of three (3) staff (S1-S2) have agreed the allegation has taken place. Two residents (R2-R3) were not familiar with an increase in rent and one (1) resident (R4) was not available for interview due to medical condition. When an increase in rent, or any changes regarding a resident, their responsible person(s) have been made aware through Nino Echevarra (S2) or S1. Record reviews have indicated that a 60-day notice has been provided to R1. Based on record reviews and interviews conducted, the preponderance of evidence standard has been met. Therefore, the above allegation is found to be Substantiated. California Code of Regulations, Title twenty-two (22), Division six (6) is being cited on the attached LIC 9099D. There has been one (1) deficiency cited during today's inspection, please see LIC9099-D. Type of visit: Required - 1 Year
On 5/04/26, Licensing Program Analyst (LPA) Mario Leon conducted an unannounced annual required visit using the CAREs Inspection Tool. California Department of Social Services (CDSS) met with Nino Echevarra - House Manager. CDSS explained the purpose of today’s visit. The facility is licensed to serve (6) residents ages 60 and above, of which (6) may be non-ambulatory and (1) Bedridden in bedroom #3. Approved hospice waiver for (6). Currently the facility has (4) residents. The facility is a single-story structure located in a residential neighborhood. It consists of (5) bedrooms, (2 -1/2) full bathrooms, back yard with shaded seating, front yard, and a laundry room in the attached garage. CDSS and the licensee toured the physical plant. There were no bodies of water or obstructions on the premises. LPA inspected a total of (5) bedrooms and (2) bathrooms. The beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the residents’ personal belongings was observed. The bathrooms were found to be within Title 22 regulations and were operational and showers were without mold or mildew. Smoke and carbon monoxide detectors were in operable condition. The water temperature properly measured between 105F°. and 120F° (degrees Fahrenheit), and the room temperature was measured at 74.1°F. During the visit, CDSS observed that the facility was clean, sanitary, and appropriately furnished. Storage areas for personal hygiene were in place. Cleaning supplies, toxins, and sharp objects were stored in a way that made them inaccessible to residents in care. Report continues, please see LIC809-C. The kitchen was inspected, and there was sufficient 2-days' perishable and 7-days' non-perishable food available, all of which was adequately maintained. All fire extinguishers were charged and operable, last inspected 09/12/25. A review of four (4) residents' service files and two (2) staff personnel files was maintained in order. CDSS found no discrepancies. During today's inspection, there have been zero (0) deficiencies cited. On 5/14/2025, Licensing Program Analyst-LPA Alfonso Iniguez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Stephen Gradney/ Licensee. LPA explained the purpose of today’s visit. The facility is licensed to serve (6) residents ages 60 and above. Of which (6) may be non-ambulatory and (1) Bedridden in bedroom #3. Approved hospice waiver for (6). Currently the facility has (5) residents. The facility is a single-story structure located in a residential neighborhood. It consists of (5) bedrooms, (2 -1/2) full bathrooms, shaded back yard, front yard, laundry room in the attached garage. LPA Iniguez and the licensee toured the physical plant. There were no bodies of water or obstructions on the premises. LPA inspected a total of (5) bedrooms and (2) bathrooms. The beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the residents’ personal belongings was observed. The bathrooms were found to be within Title 22 regulations and were operational. Smoke and carbon monoxide detectors were in operable condition. The water temperature properly measured between 105F°. and 120F°., and the room temperature ranged from 76°F to 78°F. The evaluation Report continues on the next page, LIC 809-C, providing further details of the inspection findings. During the visit, LPA Iniguez observed that the facility was clean, sanitary, and appropriately furnished. Storage areas for personal hygiene were in place. Cleaning supplies, toxins, and sharp objects were stored in a way that made them inaccessible to residents in care. The kitchen was inspected, and there was sufficient perishable and non-perishable food available, which was adequately maintained. All fire extinguishers were charged and operable. The last Fire/Disaster Drills were conducted on 4/1/25. A review of (4) residents' service files and (4) staff personnel files was maintained in order. LPA reviewed (4) Medication Administration Records (MARs) and found no discrepancies. LPA observed the facility's infection control practices. All mandated inspection control posters were displayed throughout the facility. A copy of liability insurance will be email to LPA. Facility Annual Fess current. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA did not observe deficiencies during this visit; therefore, no citations were issued. On 3/26/2024, Licensing Program Analyst (LPA) Alfonso Iniguez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Cathy Espino/staff. LPA explained the purpose of today’s visit. The facility is licensed to serve (6) residents ages 60 and above. Of which (6) may be non-ambulatory and (1) Bedridden in bedroom #3. Approved hospice waiver for (6). The facility is a single-story structure located in a residential neighborhood. It consists of (5) bedrooms, (2 -1/2) full bathrooms, shaded back yard, front yard, laundry room in the attached garage. LPA toured the physical plant with staff. There were no bodies of water or obstructions on the premises. A total of (4) rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the resident’s personal belongings was observed. Bathrooms were found to be within Title 22 regulations and were operational. LPA inspected the carbon monoxide detectors combo were in operable conditions. The water temperature properly measured between 105F. and 120F.: Kitchen 109.4°F, Bathroom #1:108.2°F and Bathroom #2:107.4°F. Evaluation Report Continues LIC 809-C LPA Iniguez observed the facility to be clean, sanitary, and appropriately furnished at the time of the visit. Storage areas for personal hygiene were observed. LPA found cleaning agents and poisons unlocked in the bathroom and kitchen. Sharps objects were locked and not accessible to residents. The kitchen was inspected and there is not sufficient perishable and non-perishable food available. All fire extinguishers were charged and were operable. A review of (3) residents' service files, (2) staff personnel files were checked. (3) Medication Administration Records (MAR) were reviewed discrepancies were found. First AID kit was checked. Last fire disaster drill was on:2/24/2024. LPA observed the facility's infection control practices. Liability insurance was given to LPA during the visit. Facility Annual Fees Current. Deficiency cited under California Code of Regulations, Title 22, Division 6, Chapter 8.
