On 03/04/2026 at 09:00 AM, Licensing Program Analyst (LPA) Jose Calderon conducted an unannounced annual inspection visit at the Global Elderly Care Facility. LPA Calderon was allowed entry into the facility by Administrator Joel Morales. The facility is licensed to serve 6 non- ambulatory residents, 60 and over and an approved hospice waiver for 4 residents. None of the residents are receiving home health. Two residents are in hospice. Currently, there are five (5) residents residing in the facility, 60 and older. LPA Calderon explained to Administrator Morales the purpose of the one-year Annual Inspection visit, and escorted LPA Calderon on a tour of the entire inside and outside facility grounds. As part of the inspection, LPA Calderon reviewed: Three (3) client service records, three (3) client medication records (MAR), two (2) staff records, and inspected the inside facility and outside grounds. The facilities’ last fire drill was conducted on 11/28/2025. The one-story residential home consists of four (4) client bedrooms, three (3) client bathrooms, living room, dining room, kitchen, staff room, office area, attached garage with washer and dryer/ storage area, backyard with table and chairs. No weapons are stored on the premises. Kitchen was inspected and observed to be clean and operational. A two-day supply perishable and seven-day supply of non-perishable foods are present in the facility. Emergency Water Storage is in the garage and kitchen area. LPA Calderon observed that all facility rooms are clean and in good repair. A comfortable temperature was observed, and the facility has central air and heating. LPA Calderon observed the following during inspection of client’s rooms: mattresses are in good condition, adequate lighting present, plenty of dresser/closet space is present, and all bed linens present. All bedrooms contain furniture, lighting fixtures and personal storage space as required, all beds have the required amount of linen and mattress covers, LPA Calderon observed fully stocked closet with bedding, towels, and toiletries supplies. Bathroom fixtures are clean, in good repair, and working properly and contain the required nonskid mats and grab bars. LPA Calderon observed bathrooms were found to be within Title 22 regulation. Bathroom #1 hot water temperature properly measured at 106 degrees Fahrenheit, bathroom #2 hot water temperature properly measured at 107 degrees Fahrenheit, and bathroom #3 hot water temperature properly measured at 105 degrees Fahrenheit. Kitchen hot water temperature is properly measured at 105 degrees Fahrenheit. Facility (7) Carbon Monoxide and (7) Smoke Detectors hard wired operated and connected were tested and are working properly. The facility four (4) Fire Extinguishers were checked and found to be fully charged and accessible. All exit doors in the facility have alarm systems. The facility has a working landline telephone. All toxins and knives are locked/secured and inaccessible to clients. Medications are centrally stored and in a locked storage cabinet. Facility 2 first aid kit is fully stocked with manuals checked and in order. Outside grounds were toured and there is no water feature observed. All Exits/ Walkways around the home were free of debris and hazards. Outside patio accessible to clients. Three (3) client files were reviewed and found to be complete. LPA Calderon reviewed three (3) resident medications (MAR), and they were all found to be administered according to doctor's orders. Two (2) staff files were checked, and they have the required documents. LPA Calderon noted the Administrator Joel Morales Certification # 7028807740 expiration date of 09/08/2026 was valid at time of inspection. The facility does not handle clients’ money/cash resources, and no surety bond is needed. Commercial General Liability Policy #0100000026500 policy period from 01/08/2026 to 01/08/2027 underwritten by Lloyds London Insurance Company, coverage 1,000,000/3,000,000 is valid at time of inspection. All the required documents are posted in the facility in a clearly visible area. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA Calderon did observe deficiencies therefore citations were issued at this time. Annual Licensing Fee is CURRENT. Licensing Program Analyst (LPA) Sparkle Day conducted an unannounced visit to the above facility. The purpose of today’s visit was to conduct the one-year inspection. LPA met with Joel Morales , Administrator and the purpose of the visit was discussed. The facility is licensed to serve 6 non- ambulatory residents 60 and over and an approved hospice waiver for 4 resident. None of the residents are receiving home health. Two residents are on hospice. The facility does not handle any of the residents’ money. This home is a single story home consisting of: (4) resident bedrooms, (2) Full bathroom, 1 half restroom, living room, kitchen with dining area, laundry room (located in the attached garage) and an outdoor shaded patio area. LPA toured the Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, shower was free of mold/mildew and a non-skid mat was in place, water temperature measured between 118.1F. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards; doorways were free of obstructions. Kitchen was checked and observed to be within Title 22 regulations. Perishable and non-perishable food supply was checked. All cleaning solutions, hazardous items, and medications were securely locked and inaccessible to residents. Smoke detectors were working properly and fire extinguisher was fully charged. Carbon monoxide detector was operational. First Aid kit was available. Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises. During todays visit LPA did not observe any deficiencies. On 2/15/2024, Licensing Program Analyst (LPA) Alfonso Iniguez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Maricel Querol/ House Manager. LPA explained the purpose of today’s visit. Facility is licensed for (6) non-ambulatory residents and (1) bedridden resident. The facility has an approved hospice waiver for (4) residents. The facility consists of (4) resident bedrooms, (3) bathrooms, living room, dining room, kitchen. LPA Iniguez toured the physical plant with house manager. There were no bodies of water or obstructions on the premises. A total of (3) rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the resident’s personal belongings was observed. Bathrooms were found to be within Title 22 regulations and were operational. LPA inspected the carbon monoxide detectors combo were in operable condition. The water temperature properly measured between: 105°F-120°F: Kitchen 111.1°F, Bathroom #1:110.7°F and Bathroom #2:113.7°F. Evaluation Report Continues LIC 809-C LPA Iniguez observed the facility to be clean, sanitary, and appropriately furnished at the time of the visit. Storage areas for personal hygiene were observed. Sharps objects and cleaning agents were locked and not accessible to residents. The kitchen was inspected and there is sufficient perishable and non-perishable food available at the property. All fire extinguishers were charged and were operable. A review of (3) residents' service files, (3) staff personnel files were reviewed. LPA checked (3) Medication Administration Records (MAR) and no discrepancies were found. The first AID kit was checked. Last facility disaster drill was: 12/7/2023. LPA observed the facility's infection control practices. A copy of the liability insurance will be email to LPA. Allegation investigated: Staff speaks inappropriately to resident in care.

On 11/15/2023 at 10:30 AM, Licensing program analyst (LPA) Lourdes Montoya conducted an initial 10-day complaint visit to this facility to investigate the allegation mentioned above. LPA met with House Manager Maricel Campbell and LPA explained the purpose of today’s visit. Staff Joel Morales arrived later and joined the visit. The investigation consisted of the following: LPA Montoya toured the facility. LPA obtained copies of the following: Staff roster, resident roster and R1’s service records which include Admission Agreement, Physician’s Report, Appraisal/Needs and Services Plan and other pertinent records. LPA interviewed two staff (S1-S2), one resident (R2), and one witness (W1) at the facility. LPA attempted to interview two residents (R3-R4) but R3 could not maintain a proper conversation and R4 was sleeping. LPA interviewed one resident (R1) and two witnesses (W2-W3) via telephone. REPORT CONTINUED IN LIC 9099C Substantiated INVESTIGATIONS REVEALED: Allegation: Staff speaks inappropriately to resident in care. According to SOC 341, a staff was very angry toward R1, and it hurt R1. On 11/15/2023, LPA interviewed two out of four residents (R1-R2). R1 stated a staff (S1) and R1 had disagreements. R1 stated S1 told R1 “I am tired of pleasing you. You would die if you continued to refuse to eat”. R1 stated S1 speaks to R1 with a raised voice and sounds angry for unknown reasons. R2 stated S1 has a lot of responsibilities in the home and sometimes S1 becomes frustrated but S1 takes good care of the residents. R2 stated R2 heard R1 and S1 argued on several occasions and S1 yelled at R1 because R1 refused to eat. On 11/15/2023, LPA interviewed two out of two staff (S1-S2). S1 and S2 denied that staff speaks inappropriately to resident in care. On 11/15/2023, LPA interviewed three witnesses (W1-W3). W2 stated W2 heard from R1 that R1 and S1 had disagreements. W3 stated W3 heard from R1 that R1 and S1 had arguments. However, W1 denied the allegation. Based on interviews, observations, and supporting documentation, the preponderance of evidence standard has been met; Therefore, the allegation of: "Staff speaks inappropriately to resident in care” is found to be SUBSTANTIATED. According to the California Code of Regulations (Title 22, Division 6, Health and Safety Code), the following deficiency has been observed and citation issued (ref. LIC 9099D).
