Allegation investigated: Facility staff is not allowing resident to see visitors
On 3/23/26, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced visit to this facility. LPA was met by the Administrator, Marylou Namoc and explained the purpose of the visit is to investigate and deliver findings for the allegations mentioned above. LPA was granted access to the facility. The investigation consisted of the following: On 10/2/25 LPA Shirley reviewed copies of the following records: Staff and Resident Roster, Admission Agreement, Physician’s Report, and Visitation list. LPA Felisa Shirley conducted a tour of the facility. LPA Shirley interviewed Staff 1 – Staff 3 (S1 – S3), and Resident -1 – Resident - 4 (R1-R4). R5 is non-verbal. Con'd on 9099-C Unsubstantiated The investigation revealed the following: Allegation: Facility staff is not allowing resident to see visitors It is being reported that R1’s family member is not allowed to visit. On 3/23/26, LPA Felisa Shirley observed the Admission Agreement for R1 and noted that R1 has been a resident of this facility since 10/26/19. On 3/23/26, LPA Shirley observed an approved list of visitors for R1. This list also included the names of persons that are not okay to visit R1. Per interview with the Administrator on 3/23/26, R1’s responsible party requested that specific family members are not allowed to visit R1. Per interview on 3/23/26 with the Administrator, R1’s son came to visit late 2020 and he was allowed to visit. The responsible party was notified and became upset and submitted a list for visitation. LPA interviewed staff 1 – staff 3 (S-1 – S-3). Of those interviewed 3 out of 3 denied the allegation. LPA interviewed resident 1 – resident 4 (R1 – R4). Of those who interviewed 4 out of 4 denied the allegation. R5 is non-verbal. Based on information gathered, LPA did not find sufficient evidence to support the allegation "Facility staff is not allowing resident to see visitors“,” therefore, the allegation is unsubstantiated. No deficiencies were cited for these allegations. Type of visit: Required - 1 Year
On 10/10/2025 at 10:00 AM, Licensing Program Analyst LPA) Jose Calderon conducted an unannounced annual inspection visit at the Best Place Home Care Facility. LPA Calderon was allowed entry into the facility by Administrator Marylou Namoc. The facility is licensed to operate for six (6) non-ambulatory elderly residents ages 60 and above. The facility is approved for one (1) bedridden and two (2) hospices. Currently, there are five (5) residents residing in the facility, age of 60 and above. LPA Calderon explained to Administrator Marylou Namoc the purpose of the one-year Annual Inspection visit, and escorted LPA Calderon on a tour of the entire inside and outside facility grounds. As part of the inspection, LPA Calderon reviewed: Five (5) resident service records, five (5) resident medication records (MAR), three (3) staff records, and inspected the inside facility and outside grounds. The facility’s last fire drill was conducted on 07/24/2025. The one-story residential home consists of five (5) resident bedrooms, two (2) resident bathrooms, living room, dining room, kitchen, staff room, office area, attached garage with washer and dryer/ storage area, backyard with table and chairs. No weapons are stored or found on the premise grounds. Kitchen was inspected and observed to be clean and operational. A two-day supply perishable and seven-day supply of non-perishable foods are present in the facility. The seven (7) day emergency Water Storage is in located in the garage. LPA Calderon observed that all facility rooms are clean and in good repair. A comfortable temperature was observed, and the facility has central air and heating. LPA Calderon observed the following during inspection of client’s rooms: mattresses are in good condition, adequate lighting present, plenty of dresser/closet space is present, and all bed linens present. All bedrooms contain furniture, lighting fixtures and personal storage space as required, all beds have the required amount of linen and mattress covers, LPA Calderon observed fully stocked closet with bedding, towels, and toiletries supplies. Bathroom fixtures are clean, in good repair, and working properly and contain the required nonskid mats and grab bars. LPA Calderon observed bathrooms were found to be within Title 22 regulation. Bathroom #1 hot water temperature was properly measured at 111 degrees Fahrenheit, bathroom #2 hot water temperature properly measured at 112 degrees Fahrenheit, and kitchen hot water temperature properly measured at 110 degrees Fahrenheit. Facility one (1) Carbon Monoxide and six (6) Smoke Detectors, hard-wired and connected, were tested and are working properly. Facility three (3) Fire Extinguisher was checked and found to be fully charged and accessible. All exit doors in the facility have alarm systems. The facility has a working landline telephone. All toxins and knives are locked/secured and inaccessible to residents. Medications are centrally stored and in a locked storage cabinet. Facility one (1) first aid kits are fully stocked with manuals was checked and in order. Outside grounds were toured and no bodies of water were observed. All Exits/ Walkways around the home were free of debris and hazards. Outside patio accessible to residents. Five (5) resident files were reviewed and found to be complete. LPA Calderon reviewed five (5) resident medications (MAR) and they were all found to be administered according to doctor's orders. Three (3) staff files were checked, and they have the required documents. LPA Calderon noted the Administrator Marylou Namoc Certification # 7009268740 expiration date of 06/25/2026 was valid at time of inspection. Commercial General Liability Policy #PCI8082234604 policy period from 07/30/2025 to 07/30/2026 underwritten by Primary Care Insurance Company, coverage 1,000,000/3,000,000 is valid at time of inspection. All the required documents are posted in the facility in a clearly visible area. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA Calderon did not observe deficiencies therefore no citations were issued at this time. Annual Licensing Fee is CURRENT. Allegation investigated: Staff do not provide adequate food service for residents Staff do not treat resident with dignity or respect
On 05/22/2025 Licensing Program Analyst (LPA) Jose Calderon conducted an unannounced visit to Best Place Home Care and was greeted by Administrator Marylou Namoc (S1). LPA Calderon spoke to S1 prior to entering the facility to conduct a risk assessment. LPA Calderon explained the purpose of this visit is to deliver the finding pertaining to the above-mentioned allegations. The investigation consisted of the following: LPA Calderon interviewed Staff S1-S3, resident R1-R5. LPA Calderon obtained the following records: Admission agreement (dated 02/06/2025), physician report (dated 02/11/2025) for R1, facility food menu. LPA Calderon toured the facility with S1 to include the kitchen area. The investigation revealed the following: Regarding the Allegation: Staff do not provide adequate food service for residents. Unsubstantiated This complaint alleged that the facility did not provide quality food for R1. The facility menu has a mix of foods served to residents on a weekly basis. LPA Calderon toured the facility with S1, inspected the kitchen area and noted the following: LPA Calderon noted a menu in the kitchen area, noted chicken and vegetables were being cooked for lunch. LPA Calderon checked the refrigerator and noted fresh apples and oranges and vegetables. The kitchen appears to have a good mix of fresh and canned food for resident’s needs. Records review indicate the following: Physician report indicate R1 is verbal and R1 can feed R1 self. Reviewed the facility menu for 4 weeks. Appeared to be a mix of food served for breakfast, lunch and dinner. Admission agreement indicates that resident moved into the facility on 02/06/2025. Admission agreement page 2 part 5 indicates “food services” three nutritious meals daily and snacks will be served. Interviews indicate the following: 3 out of 3 staff deny the allegation. R1 indicates that the facility does not serve enough food, and the quality of the food could be better. 3 out of 5 residents indicate that the facility does provide 3 meals per day and provides quality food. R5 could not answer any questions. Based on interviews and supporting documentation, the preponderance of evidence standard has NOT been met therefore, the allegation of “staff do not provide adequate food service for residents” is found to be UNSUBSTANTIATED. Regarding the Allegation: Staff do not treat resident with dignity and respect. This complaint alleged that the facility staff yells at R1. LPA Calderon toured the facility with S1, and did not note any negative interaction between staff and residents. Records review indicate the following: Physician report indicate R1 is verbal and able to care for R1 own needs with help from staff. Interviews indicate the following: 3 out of 3 staff deny the allegation. R1 indicates that the facility staff R2 yells at R1 when R1 complains about the food served in the facility. 3 out of 5 residents indicate that the facility staff does not yell at residents and treats residents with dignity and respect. R5 could not answer any questions. Based on interviews and supporting documentation, the preponderance of evidence standard has NOT been met therefore, the allegation of “staff do not treat resident dignity or respect” is found to be UNSUBSTANTIATED. No deficiencies cited during today's visit. On 10/17/2024 at 11:00 AM, Licensing Program Analyst LPA) Jose Calderon conducted an unannounced annual inspection visit at the Best Place Home Care Facility. LPA Calderon was allowed entry into the facility by Administrator Marylou Namoc. Administrator Marylou Namoc asked infection control questions and took LPA Calderon temperature prior to entering the facility. The facility is licensed to operate for six (6) non-ambulatory elderly residents ages 60 and above. The facility is approved for one (1) bedridden and two (2) hospices. Currently, there are five (5) residents residing in the facility, age of 60 and above. LPA Calderon explained to Administrator Marylou Namoc, the purpose of the one-year Annual Inspection visit, and escorted LPA Calderon on a tour of the entire inside and outside facility grounds. As part of the inspection, LPA Calderon reviewed: Five (5) client service records, five (5) client medication records (MAR), two (2) staff records, and inspected the inside facility and outside grounds. LPA Calderon interviewed five (5) clients and two (2) staff members for visit. The facilities’ last fire drill was conducted on 10/10/2024. The one-story residential home consists of five (5) client bedrooms, two (2) client bathrooms, living room, dining room, kitchen, staff room, office area, attached garage with washer and dryer/ storage area, backyard with table and chairs. No weapons are stored or found on the premise grounds. Kitchen was inspected and observed to be clean and operational. A two-day supply perishable and seven-day supply of non-perishable foods are present in the facility. The seven (7) day emergency Water Storage is in located in the garage. During the visit, LPA Calderon observed the facility infection control practices. LPA Calderon observed screening protocols for visitors, staff, and clients, sanitizing stations (Located in common areas and restrooms). LPA Calderon observed staff and clients were NOT wearing face coverings. LPA Calderon observed the facility has a 30-day supply of Personal Protective Equipment (PPE). LPA Calderon advised the Administrator Marylou Namoc to continuously monitor the Centers for Disease Control (CDC) website and Community Care Licensing Provider Informational Notices (PIN) for any updates relating to COVID-19 guidance. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA Calderon did not observe deficiencies therefore no citations were issued at this time. Annual Licensing Fee is CURRENT. An exit interview was conducted, and a copy of the Facility Evaluation Report was provided to Administrator Marylou Namoc. LPA Calderon observed that all facility rooms are clean and in good repair. A comfortable temperature was observed, and the facility has central air and heating. LPA Calderon observed the following during inspection of client’s rooms: mattresses are in good condition, adequate lighting present, plenty of dresser/closet space is present, and all bed linens present. All bedrooms contain furniture, lighting fixtures and personal storage space as required, all beds have the required amount of linen and mattress covers, LPA Calderon observed fully stocked closet with bedding, towels, and toiletries supplies. Bathroom fixtures are clean, in good repair, and working properly and contain the required nonskid mats and grab bars. LPA Calderon observed bathrooms were found to be within Title 22 regulation. Bathroom #1 hot water temperature properly measured at 111 degrees Fahrenheit, bathroom #2 hot water temperature properly measured at 112 degrees Fahrenheit, and kitchen hot water temperature properly measured at 110 degrees Fahrenheit. Facility one (1) Carbon Monoxide and six (6) Smoke Detectors hard wired and connected were tested and are working properly. The facility three (3) Fire Extinguisher was checked and found to be fully charged and accessible. All exit doors in the facility have alarm systems. The facility has a working landline telephone. All toxins and knifes are locked/secured and inaccessible to clients. Medications are centrally stored and in a locked storage cabinet. Facility one (1) first aid kits is fully stocked with manuals was checked and in order. Outside grounds were toured and no bodies of water were observed. All Exits/ Walkways around the home were free of debris and hazards. Outside patio accessible to clients. Five (5) client files were reviewed and found to be complete. LPA Calderon reviewed five (5) resident medications (MAR) and they were all found to be administered according to doctor's orders. Two (2) staff files were checked and have the required documents. LPA Calderon noted the Administrator Marylou Namoc Certification # 6031372740 expiration date of 06/25/2024 was NOT valid at time of inspection. The facility does not handle client's money/cash resources and no Surety bond is needed. Commercial General Liability Policy #PCI8082234604 policy period from 07/30/2024 to 07/30/2025 underwritten by Primary Care Insurance Company, coverage 1,000,000/3,000,000 is valid at time of inspection. Administrator Marylou Namoc to email LPA Calderon a full copy of the commercial insurance policy including all endorsements no later than 10/25/2024. Type of visit: Required - 1 Year
On 10/12/2023, Licensing Program Analyst (LPA) Lourdes Montoya conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Caregivers Letecia Barber and Susan Adeva and explained the purpose of the visit. At around 11:00 AM, Administrator Marylou Namoc arrived and joined the visit. The facility is licensed to operate for six (6) non-ambulatory elderly residents ages 60 and above. The facility is approved for one (1) bedridden and two (2) hospice. The facility is a single-story structure located in a residential neighborhood. The facility consists of the following: five (5) resident's rooms, two (2) bathrooms, a living room area, dining area, and kitchen. There is an attached garage with access through the door prior to entering the facility or the garage door. The washer and dryer are located in the garage. There is a shaded patio area with ample seating area for the residents. The facility does not handle residents’ cash resources. Facility Annual Fees are current during today’s visit. Administrator's certificate expires 6/25/2024. At around 10:00 AM, LPA toured the inside and outside grounds of the facility with Caregiver Letecia Barber and Marylou Namoc. Carbon monoxide and smoke detectors were tested and found to be operable. Fire extinguishers were last serviced on 7/5/2023. The last facility fire drill was on May 20, 2023. LPA observed the facility to be sanitary and appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, and toxins were stored and not accessible to clients. The kitchen was inspected and there are sufficient two-day perishable and seven-day non-perishable food supplies. There are no pools or bodies of water on the premises. There are no firearms on the premises or other dangerous weapons. Centrally stored medications are locked in a cabinet located in the medication room. The first aid kit has all required supplies. The facility has a written emergency disaster plan posted in the living room. The facility is maintained at a comfortable temperature. LPA observed the hot water temperature in the common bathroom was 116.1 degrees Fahrenheit. There are working lights or lamps in each room at the time of visit. There are grab bars for each toilet and shower used by residents. Showers have non-skid floors. Report Continued in LIC 809-C LPA requested a copy of the facility's liability insurance certificate. Administrator will email the binder. LPA observed the following deficiencies: 1. LPA Lourdes Montoya observed a caregiver (S3) is not associated to the facility. The administrator verbally confirmed S3 is not associated to the facility. S3 stated S3 began working at the facility in September 2023. 2. At around 10:08 AM, LPA Lourdes Montoya observed expired hamburger buns, Kroeger soft wheat bread, hotdog buns and guerrero tortillas inside the pantry. The hamburger buns expired on 9/21/23. The Kroeger soft wheat bread expired on October 6, 2023. The hotdog buns expired on 9/17/23. And the Guerrero tortillas expired on May 12, 2023 Deficiencies are being cited based on LPA observations, interviews conducted and records review in accordance with the California Code of Regulations, Title 22, see LIC809D. Civil penalty is being assessed. An exit interview was conducted, Plans of Corrections were reviewed and developed with the Licensee.
