On 10/10/25 Licensing Program Analyst (LPA) Mario Leon conducted an unannounced annual visit at the facility, using the CARE tool. LPA was met by staff one, Josie Guiao - Assistant Administrator (S1), and the purpose of the visit was explained. The facility is licensed to serve (4) elderly adults ages 60 and above, of which (4) can be non-ambulatory. The facility has an approved hospice waiver for (2). There are three (3) clients, sourced from Harbor Regional Center. Staff to client ratio is (1:2) The facility is a single-story structure located in a residential neighborhood. It consists of four (4) bedrooms, two (2) full bathrooms, back yard, front yard, laundry room, family room, living room/office, kitchen, covered empty pool, sun room and an attached two (2) car garage. During the visit, LPA observed that the facility was clean, sanitary, and appropriately furnished. Storage areas for personal hygiene were in place. Cleaning supplies, toxins, and sharp objects were properly stored, inaccessible to clients in care. The kitchen was inspected, and there was sufficient perishable and non-perishable food available, which was adequately maintained. All fire extinguishers were charged and operable, last maintained on 11/05/24. Water temperature was measured at an average of 116.3°F (degrees Fahrenheit) and the internal temperature was at a comfortable 74.3°F. A review of (3) residents' service files were conducted, no discrepancies were observed. LPA observed the facility's infection control practices, with a sign-in sheet and all inspection control posters were displayed throughout the facility. Report continues, please see LIC809C. Allegation investigated: Licensee does not ensure enough night staff to meet the needs of residents.

On 10/02/2025 Licensing Program Analyst (LPA) Troy Watson conducted an initial complaint visit to the facility listed above. LPA met with the Direct Support Professional (DSP) Alberta Caldiz and the purpose of the visit was explained. LPA Watson was granted entry into the facility. The investigation consisted of the following: On 10/02/2025 Licensing Program Analyst (LPA) Watson requested, reviewed, and obtained a copy of the Resident Roster, Staff Schedule, Behavioral Reports, and Medical Administration Records (MAR’s). LPA Watson was later greeted by the Assistant Administrator Josefina Guiao. LPA Watson toured the facility with the assistant administrator and found the facility clean and in good repair. CONTINUED ON LIC9099-C Unsubstantiated The investigation revealed the following: Allegation: Licensee does not ensure enough night staff to meet the needs of residents. On 10/02/2025 LPA Watson conducted interviews with Staff #1 – Staff#4 (S1-S4) about the allegation listed above. Out of those interviewed 4 out of 4 staff interviewed denied the allegation. On 10/02/2025 LPA Watson conducted interviews with Residents #3 - Residents #4 (R3-R4) about the allegation listed above. Out of those interviewed 2 out of 2 residents denied the allegation. An attempt to interview both Resident #1- Resident #2 (R1-R2) was made, but Resident #1(R1) was not in the facility at the time of visit. An attempt to conduct an interview R1 was made by calling the Lomita Post Acute Care Center Hospital. R1 was unable to intelligibly respond to the interview questions. An attempt to conduct an interview with Resident #2 was made at the facility but R2 refused to answer any interview questions. LPA Watson requested, obtained and reviewed the Staff Schedule and found that the facility was adequately supplied with night staff to meet the resident’s needs. Based on the information gathered, interviews conducted, and review of records LPA Watson found no evidence to support the allegation mentioned above. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation, did or did not occur, therefore the allegation is Unsubstantiated. On 9/12/2024, Licensing Program Analyst (LPA) Alfonso Iniguez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Josefina Guiao /House Manager. LPA explained the purpose of today’s visit. The facility is licensed to serve (4) elderly adults ages 60 and above, of which (4) can be non-ambulatory. The facility has an approved hospice waiver for (2). The clients are from Harbor Regional Center. (1) Restricted Health Care Conditions, and (1) utilizes postural support or protective devices. Staff to client ratio is (1:2). The facility is a single-story structure located in a residential neighborhood. It consists of four (4) bedrooms, two (2) full bathrooms, shaded back yard, front yard, laundry room, family room, living room/office, kitchen, empty pool covered, sun room and attached two (2) car garage. LPA Iniguez and the house manager toured the physical plant. There are bodies of water but covered no obstructions on the premises. LPA inspected a total of (4) bedrooms and (2) bathrooms. The beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the residents’ personal belongings was observed. The bathrooms were found to be within Title 22 regulations and were operational. Smoke and carbon monoxide detectors were in operable condition. The water temperature ranged from 108.5°F to 115.2°F, and the room temperature ranged from 76°F to 78°F. During the visit, LPA Iniguez observed that the facility was clean, sanitary, and appropriately furnished. Storage areas for personal hygiene were in place. Cleaning supplies, toxins, and sharp objects were stored in a way that made them inaccessible to residents in care. The kitchen was inspected, and there was sufficient perishable and non-perishable food available, which was adequately maintained. All fire extinguishers were charged and operable. The last Fire/Disaster Drills were conducted on 8/24/24. A review of (3) residents' service files and (3) staff personnel files was conducted. LPA reviewed (3) Medication Administration Records (MARs) and found no discrepancies. LPA observed the facility's infection control practices. All mandated inspection control posters were displayed throughout the facility. A copy of liability insurance was emailed to LPA. Facility Annual Fees are not current, LPA Iniguez provided PIN #583489 to House Manager. Deficiency cited under California Code of Regulations, Title 22, Division 6, Chapter 8. On 11/11/2023 at 2:00 pm Licensing Program Analyst (LPA) David España conducted an unannounced Required-1-year annual visit. Upon arrival at the facility, LPA España conducted a risk assessment at the front door. Based on the assessment, the facility is clear of Covid-19 infection (No COVID-19 cases). LPA verified that the facility has an approved mitigation plan report. LPA was granted access and allowed to enter the facility to conduct the inspection. LPA was met with S#1 and the purpose of today’s visit was explained. The Residential Care Facilities for the Elderly (RCFE) is licensed to serve (4) non ambulatory residents age (60) sixty and above, approved hospice waiver for (2) two residents. There are currently (4) residents in the facility. Harbor Regional Center placement, three (3) residents are ambulatory, one (1) are non-ambulatory, zero (0) bedridden. The facility is a single-story structure located in a residential neighborhood. It consists of four (4) bedrooms, two (2) full bathrooms, shaded back yard, front yard, laundry room, family room, living room/office, kitchen, empty pool, and attached two (2) car garage.LPA and S#1 toured the entire facility inside and out. Documents are posted as mandated. Bedrooms #1-#4 are occupied by residents and contain the mandated furniture. The (2) bathrooms have grab bars and non-skid mats and are clean and operational. First aid kit is fully stocked with manual; smoke detectors and carbon monoxide detectors were in compliance and operational. No firearms are stored at facility and no bodies of water present. A reviewed of Medication Records Administration (MAR) was observed to be maintained in order and accurate. Medications are stored, locked and inaccessible to residents. Resident files along with medications are current. Staff files is current. Ample supply of perishable and nonperishable food, hot water temperature is (107.1) degrees Fahrenheit, linens and personal hygiene supplies are adequate, hazardous toxins and/or sharp items are inaccessible to residents, (2) fire extinguisher is fully charged. Continued on LIC-809C Exit, walkways and/or passageways, front and back yard are free of debris and/or hazards. The facility is in good repair. During the visit, LPA observed the facility infection control practices. LPA observed a sanitizing station at the facility entry & visitors and temperatures are logged and checked, sanitizer/soap, paper towels, in all the bathrooms and additional sanitation supplies are stored in the garage. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA did not observe any deficiencies, therefore no citations were issued at this time.
