Type of visit: Case Management - Other

Licensing Program Analyst (LPA) Angelica Boyles conducted an unannounced case management visit regarding licensee-initiated facility closure. LPA was greeted by, identified herself to, and discussed the purpose of the visit with Challyn Gross. During today's visit, LPA toured the facility and verified that there were no longer residents in care. The Department verified that all residents were relocated. Per, staff the original licenses were shredded. LPA advised that the licensee is required to maintain staff and resident files for at least three years after facility closure. No deficiencies were issued, and the facility is ready for closure. An exit interview was conducted with Challyn Gross, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided at the conclusion of the visit.the state’s words, verbatim · CDSS document, Jul 16, 2026
Mar 24, 2026Facility evaluation reportReport on file

Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Correia arrived unannounced to conduct the facility’s Required Annual Inspection. Upon arrival, LPA met with Executive Director (ED) Watkins, confirmed her identity, and explained the purpose of the visit. Per the facility license, the home is approved to serve up to six (6) residents, all of whom may be non ambulatory. The facility also has delayed egress approval and maintains a hospice waiver for six (6) residents. LPA reviewed staff files, resident records, and facility documentation. All required documents were present, complete, and current. The most recent disaster drill was held in February 2026, and liability insurance coverage was verified as active. A comprehensive tour of the interior and exterior areas was conducted with ED Watkins. The facility appeared clean, well maintained, and free of hazards. Walkways were clear of obstructions. Resident bedrooms contained the necessary furnishings and were observed in good condition. All doors, windows, bathrooms, and shower areas were functional and equipped with appropriate safety features. The home provided adequate space and equipment for dining, activities, laundry, and visitation. Kitchen and dining equipment were available and in good working order. No sharps or toxic items were accessible to residents. Medications were properly labeled and secured inside a locked medication cart. Hot water temperature measured with in regulation. There were no pools or other bodies of water on the property. Smoke detectors and carbon monoxide detectors were tested and observed to be operational. ED confirmed that no firearms or ammunition are stored at the facility. Emergency lighting and facility telephones were functional. Fire extinguishers were present, fully charged, and had current service tags. Based on observations and a review of facility records, the facility was found to be operating in compliance with Title 22 requirements. No deficiencies were cited during today’s inspection. An exit interview was completed with ED Watkins. A copy of this report and the Licensee/Appeal Rights were provided. Feb 11, 2026Complaint investigation reportSubstantiated

Allegation investigated: Staff did not allow resident visitation at the facility.

Licensing Program Analyst (LPA) Correia conducted an unannounced visit to conclude a complaint investigation. LPA Correia was greeted by Executive Director (ED) Watkins, identified herself, and explained the purpose of the visit. On December 31, 2025, the Department received a complaint alleging that the Licensee did not allow visitation with Resident 1 (R1). The Department’s investigation included interviews with staff, residents, and outside sources, as well as a review of resident records. It was alleged that facility staff restricted Outside Source 1 (OS1) from visiting R1 for approximately three weeks following R1’s admission. Substantiated A review of R1’s records revealed that their Power of Attorney (POA) was Outside Source 2 (OS2) for both health and fiduciary matters; however, R1 was not conserved. Interviews with outside sources (OS1, OS2, and OS3) corroborated that, at the time of admission, OS2 requested staff to prohibit OS1’s visitation with R1. An interview with OS2 confirmed this request, stating that R1 was still acclimating to the new environment and that OS1’s visits appeared to upset R1. Additionally, OS1 reported being denied visitation on two occasions by facility staff, who stated they were following management’s directive. OS3 also corroborated that OS1 was denied visitation by facility staff [see LIC 811 for confidential names list].Allegation Findings: Based on interviews and record review, the allegation that the Licensee did not allow visitation with R1 is SUBSTANTIATED. An exit interview was conducted with Health Services Director (HSD) Anding, A copy of this reports (LIC 9099/LIC 9099D) and the Licensee Rights (LIC 9058) will be provided at the conclusion of the visit. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Debbie Correia conducted a Pre-licensing and Component III Visit to observe the physical plant for compliance per Title 22, Division 6 of the CA Code of Regulations and Health & Safety codes. LPA Correia was met by Executive Director (ED) Williams, identified herself, and was granted entry into the facility. LPA Correia conducted a resident and staff facility records reviews and an interior and exterior facility tour of the physical plant. LPA Correia observed resident accommodations including required furnishings and linens. Resident bathrooms were equipped with non-skid flooring and grab bars and the water temperature read at approximately 110.6 F in bathroom faucets, a secure location for administrative records was observed; as well as a locked medication cart. A first aid kit and manual were present. Toxins, cleaning supplies, and sharps are kept in a locked room or cabinet. Fire extinguishers are affixed with current tags. Facility staff conducted their last disaster drill on February 26, 2024; smoke and carbon monoxide detectors are present and operable; facility posting requirements were present in a common area and the facility administrators certification is current until June 22, 2024. Staff and resident records were complete. LPA observed 7 days of non-perishable and 2 days of perishable food. Facility employs a Life Style Director with scheduled activities for residents. The facility meals are based on a dietician approved menu to ensure proper nutrition. LPA observed shaded outdoor areas, and a large common room for resident activities. Per ED Williams there are no firearms or weapons on the premises. Fire clearance has been received and approved. The facility is ready for licensure once reviewed and approved by the program manager.
